RESUMO

Food allergy (FA) is a major health issue for children living in Western countries. At this time the only proven treatment for FA is elimination of offender antigen from the diet. It is becoming clear that the development of gut microbiota exerts a profound influence on immune system maturation and tolerance acquisition. Increasing evidence suggests that perturbations in gut microbiota composition of infants are implicated in the pathogenesis of FA. These findings have unveiled new strategies to prevent and treat FA using probiotics bacteria or bacterial substance to limit T-helper (Th)/Th2 bias, which changes during the disease course. Selected probiotics administered during infancy may have a role in the prevention and treatment of FA. Lactobacillus rhamnosus GG (LGG) is the most studied probiotic in this field. Administration of LGG in early life have a role in FA prevention. Preliminary evidence shows that LGG accelerates oral tolerance acquisition in cow's milk allergic infants. We are understanding the mechanisms elicited by LGG and metabolites in influencing food allergen sensitization. A deeper definition of these mechanisms is opening the way to new immunotherapeutics for children affected by FA that can efficiently limit the disease burden.

Atopy patch tests (APTs) have been proposed for the diagnostic approach in children with non-IgE-mediated cow's milk allergy and gastrointestinal symptoms. We aimed to investigate the benefit of APTs in predicting oral tolerance in these patients. We prospectively evaluated 172 subjects with a sure diagnosis of non-IgE-mediated CMA and gastrointestinal symptoms (97 boys, 56.4%; age, 6.37 m; range, 2-12 m). At diagnosis, 113/172 (65.7%) children had positive APTs to cow's milk proteins (CMP). After 12 months of exclusion, diet APTs were repeated immediately before OFC. APTs significantly correlated (P < 0.001) with the OFC outcome (r 0.579). Diagnostic accuracy was sensitivity of 67.95%, specificity of 88.3%, PPV of 82.81%, NPV of 76.85%, and a +LR of 5.80. APTs are a valuable tool in the follow-up of children with non-IgE-mediated CMA-related gastrointestinal symptoms by contributing in determining whether an OFC can safely be undertaken.

BACKGROUND: Acute diarrhoea is a frequent problem in children with heavy economic burden for families and society. AIM: To test the efficacy of a new synbiotic formulation containing Lactobacillus paracasei B21060, arabinogalactan and xilooligosaccharides in children with acute diarrhoea. METHODS: Double-blind, randomised, placebo-controlled trial, including children (age 3-36 m) with acute diarrhoea who were allocated to placebo or synbiotic group. Major outcome was resolution rate of diarrhoea at 72 h. Total duration of diarrhoea, daily stool outputs, stool consistency, working days lost by parents, adjunctive medications, and hospitalisation were also assessed. RESULTS: We enrolled 55 children in placebo group and 52 in synbiotic group. The two groups were similar for demographic and clinical characteristics. Resolution rate of diarrhoea at 72 h was significantly higher in synbiotic group (67%) compared to placebo group (40%, P = 0.005). Children in synbiotic group showed a significant reduction in the duration of diarrhoea (90.5 h, 78.1-102.9 vs. 109.8 h, 96.0-123.5, P = 0.040), daily stool outputs (3.3, 2.8-3.8 vs. 2.4, 1.9-2.8, P = 0.005) and stool consistency (1.3, 0.9-1.6 vs. 0.6, 0.4-0.9, P = 0.002) compared to placebo group (data expressed as mean, 95% CI). Rate of parents that missed at least one working day (41.8% vs. 15.4%, P = 0.003), rate of children that needed adjunctive medications (25.5% vs. 5.8%, P = 0.005) or hospitalisation (10.9% vs. 0%, P = 0.014) after the first 72 h of treatment, were reduced in synbiotic group. CONCLUSION: The synbiotic formulation studied is effective in children with acute diarrhoea. Australian New Zealand Clinical Trials Registry (ACTRN12611000641998).

Treatments currently employed for plantar warts are often painful (electrosurgery, cryotherapy) and not always effective (keratolytic agents). In this paper we investigate the effect of photodynamic therapy (PDT) with topical delta-aminolaevulinic acid (ALA) on plantar warts. In order to remove the superficial hyperkeratotic layer of the warts an ointment containing 10% urea and 10% salicylic acid was applied for 7 days. After gentle curettage, a cream containing 20% ALA was applied under an occlusive dressing for 5 h on 64 warts, while 57 warts (controls) received only the vehicle. Both the ALA-treated warts and the controls were irradiated using a visible light lamp (with a range of 400-700 nm, peaking at 630 nm). The light dose was 50 J/cm(2). Patients were followed-up for 22 months. Two months after the last irradiation session 48 (75.0%) out of 64 ALA-PDT treated warts had resolved. By contrast only 13 (22.8%) of the 57 control warts had done so. During the treatment a few patients complained of a mild burning sensation. The absorption of ALA by the verrucous tissue was demonstrated by in vivo fluorescence spectroscopy. This study shows that topical ALA-PDT can be an alternative treatment for plantar warts. Further studies will be necessary in order to optimize the concentration of ALA and duration of treatment.

BACKGROUND: Solar urticaria (SU) represents an uncommon skin disorder, characterized by pruritic erythema and wheals after sun exposure, that sometimes restricts normal daily life. OBJECTIVE: To evaluate data concerning sex, age, natural history, associated diseases, and eliciting wavebands of 57 SU cases. METHODS: Questionnaire for anamnestic data, laboratory examinations, phototesting. RESULTS: Sex: 25 (44%) males, 32 (56%) females. Age: The peak age was between 20 and 30 years. Skin type (ST): 12 (21%) ST II, 39 (68%) ST III, and 6 (11%) ST IV. Time between onset and complete disappearance of SU was from 2 to more than 6 years; the main peak (37 patients) between 4 and 6 years. There were histories of atopic dermatitis 12 (21%) and asthma or rhinitis 15 (26%). Association with other urticarias (U): 13 (21%) dermographic U, 2 food U, 3 heat U. Increased immunoglobin E (IgE): 19 (33%). Eliciting wavebands: 38 (67%) visible light (VIS), 16 (28%) long ultraviolet (UVA), 3 natural sunlight. Minimal urticarial dose (MUD): 20-37.5 J/cm2 for VIS-sensitive patients, 5-10 J/cm2 for UVA. CONCLUSIONS: In our series: 1. SU affects both sexes usually when they are under 30 years of age; 2. nearly half the patients are free of disease within 5 years; 3. in about one fourth of cases SU is associated with dermographic urticaria or displays a history of atopic dermatitis; 4. the wheals are elicited mainly by VIS or UVA; 5. SU can be prevented, at least in part, by antihistamines or by PUVA therapy.

To investigate the clinical value of delta agent infection in HBsAg positive chronic hepatitis, we detected anti-delta antibody (anti-delta) in serum and delta antigen (delta-Ag) on sequential liver biopsies of nine patients with HBsAg-positive CPH and 45 patients with HBsAg-positive CAH without cirrhosis observed for at least 2 years. The initial group of patients with CAH was composed of 54 patients who were consecutively either left untreated or treated with 15 mg of prednisolone daily. Nine patients dropped out. Delta-Ag was searched by the direct immunofluorescence technique. HBsAg, anti-delta, HBeAg and anti-HBe were detected by RIA. All CPH patients were delta-Ag negative in the 1st liver biopsy and anti-delta negative in serum. Out of these nine patients, seven remained delta-Ag negative CPH throughout the observation and the remaining two became delta-Ag positive, anti-delta positive and developed CAH. The 73% of patients with CAH were delta-Ag positive on the 1st biopsy and anti-delta positive in serum. The patients in the delta-Ag positive group (24 were always delta-Ag positive and two became delta-Ag positive during the observation) more frequently than those in the delta-Ag negative group (10 were always delta-Ag negative and nine became delta-Ag negative during the study) showed deterioration or died (77 vs. 16%; P less than 0.001). Neither in the delta-Ag positive group nor in the delta-Ag negative group did prednisolone modify the course of the disease.(ABSTRACT TRUNCATED AT 250 WORDS)

Epidemiological research has been carried out on multiple sclerosis in the province of Varese. The index of prevalence of the disease at 31-12-1975 was 24.176%. The geomedical figures for the province are reported and a comparison made with similar research carried out in 1966. The clinical aspects of the disease are discussed in relation to a group of patients from other Italian provinces.

A blood test for M.S., E-UFA test, was reviewed with statistical analysis. The test was positive in 18 out of 22 M.S. patients, negative in the controls and OND. The results confirm the validity of the test.

RESUMO

Electrophysiological examination of the peroneal nerve was performed in a group of individuals exposed for professional reasons to vapours of n-heptane. The results suggest a probably specific role of this hydrocarbon compound in provoking "minimal" peripheral nerve damage.

A hundred and forty-three patients with focal - mainly neocortical - hemiphere-damage (subdivided in 4 groups according to the side of the damaged hemisphere and the presence/absence of visual field defect) and 70 controls, were given a 20-sec delay recognition test of complex spatial visual patterns. There was one free delay condition and two conditions with interference from visual material of either similar or quite dissimilar patterns to that of memoranda. The results provided by two co-variance analyses are: (i) the comparison of the hemisphere-damaged groups with controls show a significant memory decay of both right groups only when there was perceptual interference from similar patterns; (ii) the comparisons between hemisphere-damaged groups fail to elicit any significant difference in the three memory conditions. There appears to be no specific neocortical area involved in the first steps of memory processing of spatial informations and the interference engendered by interpolated perceptual activity is material-specific.
