RESUMO

Fever--the most frequent symptom of diseases in children--points diagnostically to an infection. We report on a child who presented with recurrent high fever of unknown origin. Infectious diseases could be excluded. During a general medical checkup the results of repeated pilocarpine iontophoresis attracted attention by the minimal secretion of sweat. The diagnosis of hypohydrotic ectodermal dysplasia could be confirmed by dermal biopsy since all integumentary appendages were missing. Only 300 cases of this rare hereditary dysplasia can be found in the literature.

RESUMO

Representatives of the working group of "Neuropaediatrics" and "Infectology" like to present recommendations on diagnostics and treatment of infectious diseases on the CNS. Individual opinions can not be considered in recommendations always. Therefore we have to understand these recommendations in this way that individual decisions will not be restricted in special situations.

Serum samples from 1421 domestic cats (561 healthy, 860 sick) were tested for FIV-, FeLV- and coronavirus infection. The results were stored in a computer data base and compared with epidemiologic data and clinical findings. All 3 infections were significantly more prevalent in sick than healthy cats: FIV was found in 0.7% of healthy and 3.4% of sick cats. For FeLV the prevalence was 3.0% and 13.0% and for coronavirus 21% and 36.2%, respectively. FIV-infected cats were mostly male (73%); no sex predilection was observed in FeLV- and coronavirus infection. In sick cats FIV-infection was significantly more prevalent in cats greater than 2 years of age; no age-dependence was found in FeLV- and coronavirus infections. The prevalence of FIV-infection increased significantly with the number of animals per household. In contrast, the frequency of FeLV infection decreased with the number of animals per household. Prevalence of coronavirus infection did not vary with group size or living conditions. The following clinical symptoms were associated with infection: FIV: general depression, diseases of the urinary tract; FeLV: general depression, fever, rough hair coat, lymphadenopathy, impaired functions of heart and circulation and muscle atrophy; coronavirus: lymphadenopathy and alterations in the abdomen. It was concluded that based on the clinical symptoms alone FIV-infection could not be diagnosed nor differentiated from the other 2 infections.

Commercial preparations of streptavidin, a bacterial biotin-binding protein, were analyzed by isoelectric focusing combined with an affinity-based protein blot using biotinylated, protein-saturated nitrocellulose. The colorimetrical detection of streptavidin with biotinylated alkaline phosphatase allows the selective visualization of streptavidin molecules with at least two active biotin-binding sites. Dependent on the preparation, seven to sixteen streptavidin forms were found with isoelectric points ranging from 5 to 8. Molecular weight analysis of the subunits of streptavidin showed that the observed heterogeneity was mainly due to limited proteolysis, which does not destroy the biotin-binding activity. The preparations differed also in the nonspecific reactivity of streptavidin with single-stranded DNA, bovine serum albumin and Tween 20. No relationship was observed between heterogeneity and non-specific binding activity. Data obtained from protein blots onto nitrocellulose saturated with single-stranded DNA showed that it cannot be excluded that streptavidin with only a single active biotin-binding site is mainly responsible for the nonspecific reactivity of some streptavidin preparations.

Manasan is an adapted formula for infants on the basis of cow's milk simulating the mode of action of human milk on faeces-pH and faeces microflora. In two temporally separated tests the effect of Manasan-feeding on faeces-pH, faeces flora, and acid-base-homeostasis was controlled and compared with the effect of human milk and Ki-Na-formula. With Manasan fed up from birth or following breast-feeding faeces-pH was to be found in an acid range, clearly. With Ki-Na the faeces-pH was situated in the neutral range. With Manasan the bacteria flora revealed a noteworthy decrease in the number of Bacteroides germs achieving a non-putrefactive milieu. Concerning the acid-base-homeostasis as with human milk so with Manasan late metabolic acidosis were to meet rarely. The experiments revealed that in important parameters, particularly with microecological signs, Manasan is comparable with human milk.

RESUMO

Fever should be treated according to the individual situation. Securing a sufficient fluid intake is the most important primary measure. The decrease of body temperature by physical means is efficient only in combination with drug therapy and presumes the control of blood circulation. Aminophenazon is the most efficient drug to lower fever. Metamizol should be prefered in children at risk for convulsive disorders. The combination of aminophenazon with phenothiazines can be recommended , prothazine is the drug of first choice in suitable cases a caudal treatment of fever with antiphlogistic drugs as diclophenacsodium (Voltaren) is indicated.
