RESUMO

OBJECTIVE: We present a registration-based semiautomatic mandible segmentation (SAMS) pipeline designed to process a large number of computed tomography studies to segment 3-dimensional mandibles. METHOD: The pipeline consists of a manual preprocessing step, an automatic segmentation step, and a final manual postprocessing step. The automatic portion uses a nonlinear diffeomorphic method to register each preprocessed input computed tomography test scan on 54 reference templates, ranging in age from birth to 19 years. This creates 54 segmentations, which are then combined into a single composite mandible. RESULTS: This pipeline was assessed using 20 mandibles from computed tomography studies with ages 1 to 19 years, segmented using both SAMS-processing and manual segmentation. Comparisons between the SAMS-processed and manually-segmented mandibles revealed 97% similarity agreement with comparable volumes. The resulting 3-dimensional mandibles were further enhanced with manual postprocessing in specific regions. CONCLUSIONS: Findings are indicative of a robust pipeline that reduces manual segmentation time by 75% and increases the feasibility of large-scale mandibular growth studies.

There is a growing interest in understanding how amyloid-ß (Aß) accumulation in preclinical Alzheimer's disease relates to brain morphometric measures and cognition. Existing investigations in this area have been primarily conducted in older cognitively-normal (CN) individuals. Therefore, not much is known about the associations between Aß burden, cortical thickness, and cognition in midlife. We examined this question in 109, CN, late-middle-aged adults (mean age=60.72±5.65 years) from the Wisconsin Registry for Alzheimer's Prevention. They underwent Pittsburgh Compound B (PiB) and anatomical magnetic resonance (MR) imaging, and a comprehensive cognitive exam. Blinded visual rating of the PiB scans was used to classify the participants as Aß+ or Aß-. Cortical thickness measurements were derived from the MR images. The Aß+ group exhibited significant thinning of the entorhinal cortex and accelerated age-associated thinning of the parahippocampal gyrus compared with the Aß- group. The Aß+ group also had numerically lower, but nonsignificant, test scores on all cognitive measures, and significantly faster age-associated cognitive decline on measures of Speed & Flexibility, Verbal Ability, and Visuospatial Ability. Our findings suggest that early Aß aggregation is associated with deleterious changes in brain structure and cognitive function, even in midlife, and that the temporal lag between Aß deposition and the inception of neurodegenerative/cognitive changes might be narrower than currently thought.

The hyoid bone anchors and supports the vocal tract. Its complex shape is best studied in three dimensions, but it is difficult to capture on computed tomography (CT) images and three-dimensional volume renderings. The goal of this study was to determine the optimal CT scanning and rendering parameters to accurately measure the growth and developmental anatomy of the hyoid and to determine whether it is feasible and necessary to use these parameters in the measurement of hyoids from in vivo CT scans. Direct linear and volumetric measurements of skeletonized hyoid bone specimens were compared with corresponding CT images to determine the most accurate scanning parameters and three-dimensional rendering techniques. A pilot study was undertaken using in vivo scans from a retrospective CT database to determine feasibility of quantifying hyoid growth. Scanning parameters and rendering technique affected accuracy of measurements. Most linear CT measurements were within 10% of direct measurements; however, volume was overestimated when CT scans were acquired with a slice thickness greater than 1.25 mm. Slice-by-slice thresholding of hyoid images decreased volume overestimation. The pilot study revealed that the linear measurements tested correlate with age. A fine-tuned rendering approach applied to small slice thickness CT scans produces the most accurate measurements of hyoid bones. However, linear measurements can be accurately assessed from in vivo CT scans at a larger slice thickness. Such findings imply that investigation into the growth and development of the hyoid bone, and the vocal tract as a whole, can now be performed using these techniques.
