RESUMO

Objective: To establish and validate a prediction model for early-stage epithelial ovarian cancer based on least absolute shrinkage and selection operator (LASSO) regression. Methods: A total of 509 cases ovarian mass patients who underwent surgical treatment in Tianjin Medical General Hospital from January 2018 to March 2023 were retrospectively analyzed. The patients were randomly divided into modeling group [n=356, M(Q1,Q3) for age were 43 (31, 61) years] and internal validation group [n=153, age 42 (31, 60) years] by 7∶3 ratio. In addition, 86 patients [age 44 (33, 61) years] who underwent surgical treatment for ovarian mass in Tianjin Medical University General Hospital from April to November 2023 were collected as external validation group. The variables were screened by LASSO regression. The nomogram model was established and plotted by multivariate logistic regression. Internal and external validation were then conducted. The model performance and clinical applicability were evaluated using receiver operating characteristic (ROC) curve, calibration curve and decision curve. Results: Five variables including age (OR=1.040,95%CI:1.000-1.050,P=0.002), carbohydrate antigen 125 (CA125) (OR=1.001, 95%CI: 1.000-1.010, P=0.017), human epididymis protein 4 (HE4) (OR=1.020, 95%CI: 1.000-1.030, P=0.002), carbohydrate antigen 199 (CA199) (OR=1.001, 95%CI:1.000-1.020, P=0.023) and lactate dehydrogenase (LDH) (OR=1.020, 95%CI: 1.010-1.022, P=0.001) were screened as risk factors for early-stage epithelial ovarian cancer. The nomogram model was constructed based on these above five risk factors to predict early-stage epithelial ovarian cancer. ROC curves showed the area under curve (AUC) were 0.915(95%CI:0.910-0.932)for modeling group, 0.891(95%CI:0.874-0.905) for internal validation group, and 0.924(95%CI:0.907-0.942) for external verification. The calibration curves and clinical decision curves showed the model exhibited good consistency and clinical practicability. Conclusions: The nomogram model built includes age, CA125, HE4, CA199, and LDH. It can effectively predict early-stage epithelial ovarian cancer and has strong clinical practicability.

RESUMO

Because of the limited effect of traditional treatment methods such as surgical treatment, radiotherapy and chemotherapy,the emergence of immunotherapy has brought new hope for the treatment of patients with bladder cancer. As an immune checkpoint inhibitor, programmed death receptor 1/programmed death receptor-ligand 1 (PD-1/PD-L1) inhibitor has shown good anti-tumor activity and safety in the treatment of advanced bladder cancer, and has been recommended for advanced bladder cancer as second-line treatment by NCCN guidelines. PD-1/PD-L1 inhibitor for the treatment of bladder cancer has covered the first-line and second-line treatment, as well as maintenance therapy after first-line chemotherapy of locally advanced or metastatic bladder cancer, adjuvant and neoadjuvant therapy of muscle-invasive bladder cancer, treatment of high-risk non-muscle invasive bladder cancer failed by Bacille Calmette-Guérin vaccine perfusion, and bladder preservation therapy of muscle-invasive bladder cancer. Some of related studies have achieved certain results, and some are in progress, both of which need to be further examined. Maybe it can provide new guidance and ideas for clinical treatment of bladder cancer.

A general therapeutic strategy to treat breast cancer is attractive as different subtypes of breast cancers often exhibit distinct response to existing cancer therapeutics. To this end, we prepare a catalyst couple of glucose oxidase (GOx) and gallic acid-ferrous (GA-Fe) nanocomplexes, a type of near-infrared (NIR) absorbing Fenton catalyst, to enable NIR-trigger in-situ gelation and enhanced chemodynamic/starvation therapy that appears to be effective for different types of breast cancer cells. In this system, GOx is mixed with GA-Fe in a solution of N,N-dimethylacrylamide (DMAA) and poly (ethylene glycol) double acrylate (PEGDA). Upon intratumoral injection and NIR laser exposure, such GA-Fe show rapid temperature increase, which would simultaneously increase the catalytic efficiencies of GA-Fe and GOx. The cascade production of hydroxyl radicals (•OH) from glucose is then initiated to enable polymerization of DMAA and PEGDA to form a hydrogel at the injection site within the tumor. The continuous production of cytotoxic •OH together with glucose depletion by the intratumorally fixed catalyst couple would further confer effective destruction of breast cancer tumors by such chemodynamic/starvation therapy. Our work presents a hydrogel-based therapeutic strategy for local treatment of solid tumors with high tumor destruction efficacy and low systemic toxicity.

Objective: This study was conducted to analyze the clinical characteristics and pituitary function of patients with primary empty sella (PES). Methods: The clinical data from 123 hospitalized adult patients with PES from January 2010 to May 2016 were retrospectively studied. Results: (1) The average age of the 123 (male 43, female 80) PES patients was (59.2±13.6) years (ranging 24-92 years), among whom 61% patients were in the age group between 50-69 years. (2) The symptoms of the patients included fatigue (56.1%), headache (34.1%), nausea and vomiting (17.9%), gonadal dysfunction (17.1%), visual disturbance (5.7%) and hypopituitarism crisis (3.3%). (3) Hypopituitarism was found in 66 of the 123 patients. Among them, 36.6%, 31.7% and 17.1% were central hypoadrenalism, hypogonadism, and hypothyroidism, respectively.The percentage of hypopituitarism in complete PES was significantly higher than that in partial PES (P<0.05). (4) Sixteen patients were concomitant with other autoimmune diseases including 11 patients with Graves' disease and 2 with Cushing's syndrome due to adrenal adenoma. Conclusions: The incidence of hypopituitarism in PES was 53.7%, in which the pituitary-adrenal axis hypofunction was more common. An overall evaluation of the pituitary function was essential for the patients who had headache and fatigue, or with suspected PES. The patients with hypopituitarism should be given hormone replacement therapy in time and followed up afterword.

This experiment was conducted to investigate the effects of microencapsulted (MEF) and the extract of seed (ECOS) on growth performance, immune functions, and serum biochemical parameters in broiler chickens. A total of 240 1-d-old male broilers were randomly allotted into 6 treatments with 8 replicates/treatment and 5 broilers in each cage. The dietary treatments included 1) a basal diet without antibiotic (group A), 2) the basal diet + 1 g MEF/kg diet (1 × 10 cfu/g MEF; group B), 3) the basal diet + 300 mg ECOS/kg diet (group C), 4) the basal diet + 300 mg ECOS/kg diet + 1 g MEF/kg diet (group D), 5) the basal diet + 500 mg ECOS/kg diet (group E), and 6) the basal diet + 500 mg ECOS/kg diet + 1 g MEF/kg diet (group F). The feeding experiment included 2 phases: the starter phase from Day 1 through 21 and the grower phase from Day 22 through 42. The results showed that a diet supplemented with MEF and ECOS had no significant effect on ADG, ADFI, feed conversion ratio, and average BW during the whole experimental period ( > 0.05), but group F showed an improving trend in growth performance. Serum IL-2, IgA, and IgG levels and spleen index were significantly affected by dietary treatment ( < 0.05). Serum IgA and IgG levels and spleen index in group F were significantly higher than in the group A ( < 0.05), and the IL-2 level was significantly decreased ( < 0.05) on Days 21 and 42. Compared with the group A, diets supplemented with MEF and ECOS can significantly decrease total cholesterol, low-density lipoprotein cholesterol, triglycerides, and blood urea nitrogen levels ( < 0.05) and increase the high-density lipoprotein cholesterol level on Days 21 and 42. Concentrations of serum biochemical parameters were significantly increased in group F ( < 0.05). In summary, the results indicated that dietary supplementation of MEF and/or ECOS had no significant effect on growth performance but significantly increased spleen index and the levels of serum IgA and IgG and improved serum lipid metabolism. The 1 g MEF/kg diet (1 × 10 cfu/g diet) plus 500 mg ECOS/kg diet was the optimum supplemental dose in this experiment.

WHAT IS KNOWN AND OBJECTIVE: Clinical investigations into postoperative intravenous patient-controlled analgesia (PCA) have indicated interindividual differences in fentanyl consumption. Cytochrome P450 3A4 (CYP3A4) is the main metabolism enzyme of fentanyl, and single nucleotide polymorphisms within the CYP3A4 gene may contribute to the variability of fentanyl analgesic efficacy. The aim of this study was to investigate whether the most common genetic variation in Chinese, CYP3A4*1G, has an impact on the fentanyl consumption for intravenous PCA in Chinese Han women undergone abdominal total hysterectomy. METHODS: A total of 79 female patients (American Society of Anesthesiologist physical status I or II) scheduled to undergo elective abdominal total hysterectomy were enrolled. All patients received combined spinal-epidural anaesthesia with bupivacaine. Intravenous fentanyl PCA was provided postoperatively for satisfactory analgesia. The doses of fentanyl consumption were recorded 2, 4, 24 and 48 h after the initiation of PCA postoperatively. Pain at rest and adverse effects were measured with rating scales. CYP3A4*1G was screened by means of direct sequencing and further confirmed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). RESULTS AND DISCUSSION: Forty-six patients were GG homozygotes, 27 patients were GA heterozygotes, and six patients were AA homozygotes, respectively. The distribution of the CYP3A4*1G allele was consistent with Hardy-Weinberg equilibrium (P>0·05). At 2 and 4 h, the doses of fentanyl required for patients with GA/AA genotypes were 80·0 (45·0, 112·5) µg and 120·0 (80, 173·8)µg, respectively, and significantly lower than those for GG homozygotes [91·3 (80·0, 125·0) µg and 169·0 (112·5, 226·3) µg, respectively, P<0·05]. There was trend of decreasing fentanyl consumption at 24 and 48h in patients with GA/AA genotypes, relative to GG homozygotes, but the difference was not statistical significant (P>0·05). WHAT IS NEW AND CONCLUSIONS: CYP3A4*1G has an impact on the analgesic effect of fentanyl in Chinese Han subjects. Further validation of our results in a well-powered study would be helpful.

Based on a facile vapor-phase transport method without any catalyst and template, one-dimensional single-crystalline bismuth oxide (Bi2O3) micro/nanoribbons were fabricated on silicon substrates in large quantities and morphology-controlled fabrication of Bi2O3 was achieved from a single precursor. The widths of Bi2O3 ribbons varied from 0.2 to 20 microm depending on the deposition temperatures. The thickness was in the range of 0.1-2 microm and the length reached several hundred micrometers and even millimeter range. The detailed composition and structural analysis confirmed the single-crystalline nature of alpha-Bi2O3 micro/nanoribbons with monoclinic structure. The photoluminescence spectrum of a single Bi2O3 ribbon showed a broadband emission from 450 to 750 nm in the visible region, consisting two peaks located at 589 and 697 nm which were primarily originated from the impurity ions and crystal defects. A self-catalyzed vapor-solid model was proposed to account for the growth mechanism of Bi2O3 ribbons with different morphologies.

Removal of arsenate (As(V)) from aqueous solution using both nanocrystalline and coprecipitated Mg/Al layered double hydroxides (LDHs) was examined under different sorption/desorption conditions. The surface area, pore volume, and pore size of the nanocrystalline LDH were significantly higher than those of the coprecipitated LDH, thus resulting in a higher As(V) sorption maximum than the coprecipitated LDH. The calculated activation energy (E(a)) value was 24.7 kJ/mol, suggesting the occurrence of anion exchange process for As(V) removal by the nanocrystalline LDH. The predominance of anion exchange process was further supported by the investigation of ionic strength effect, and XRD and FTIR analyses. The effect of aqueous matrix on As(V) sorption by the nanocrystalline LDH was found to increase in the order of nitrate < silica < sulfate < carbonate < phosphate. Regeneration study showed that a secondary sorption mechanism might occur concurrently for the As(V) sorption by nanocrystalline LDH besides the predominant anion exchange process. Prospects and challenges for practical application of the nanocrystalline LDH were also discussed in the latter part of this study.

In this study, Mg/Al layered double hydroxides (LDHs) were investigated for their potential in scavenging several harmful oxyanions from aqueous solution. LDHs could effectively remove oxyanions and the selectivity of LDHs was governed by both the valencies and the ionic radii of the oxyanions. LDHs prepared by the fast coprecipitation with hydrothermal treatment (FCHT) method and sol-gel with solvothermal treatment method (SGST) had higher oxyanion (i.e. arsenate) removal efficiency than those prepared by the conventional routes, owing to their lower carbonate content, higher surface area, larger pore volume, larger pore size, and nanocrystalline characteristic. The sorption of arsenate by FCHT-LDH was found primarily due to anion exchange mechanism and might involve a secondary sorption mechanism. The negative DeltaG degrees for arsenate sorption confirmed the spontaneity of the removal process. The positive values of DeltaH degrees and DeltaS degrees provided further evidence of the anion exchange process in the removal mechanism.

Tubular ZnO microstructural arrays were fabricated by a hydrothermal decomposition method. The dependence of the morphologies on the growth time and temperature was investigated in detail. An experiment was carried out to determine the mechanism of tubular ZnO formation. Our results showed that ZnO microtubes originated from an ageing process from ZnO microrods at a lower temperature (compared to the temperature when hydrothermal deposition of ZnO microrods was dominant) due to the preferential chemical dissolution of the metastable Zn-rich (0001) polar surfaces. A growth model was proposed based on the coexistence of hydrothermal deposition and dissolution of ZnO in the fabrication process.

A cogent understanding of the microstructure, and indeed nano-structure, of hydroxyapatite (HA) and the interface between Ti-6Al-4V and HA is crucial to its appropriateness as a biomaterials. This paper reports the analysis of plasma-sprayed HA/Ti-6Al-4V composites by transmission electron microscopy (TEM) and scanning transmission electron microscopy (STEM) to elucidate the intricate nature of the materials following plasma spray processing and in vitro evaluation. The novel Ti-6Al-4V/HA composite coating, with approximately 48 wt% HA, had demonstrated attractive tensile adhesion strength (approximately 28 MPa) and improved Young's modulus (approximately 55 GPa). Experimental results demonstrated that amorphous calcium phosphate and fine HA grains were formed during rapid splat solidification in the as-sprayed composite coatings. Small Ti-6Al-4V grains were observed adjacent to the amorphous calcium phosphate. The coatings were further heat treated at 600 degrees C for 6 h, and significant crystallisation of the amorphous calcium phosphate phase took place. However, complete crystallisation was not achieved at this temperature, as the coatings invariably contained a small amount of amorphous calcium phosphate phase in some local regions. After immersion in simulated body fluid for 2 weeks and 10 weeks, TEM and STEM confirmed that the interfaces inside the coating maintained good microstructural integrity.

Fr. The structure of (1) was established as (2S,3S,4R,2'R)-2-(2'-hydroxytetracosanoylamino) octadecane-1,3,4-triol by means of spectroscopic and chemical methods.

The distribution and histochemical characteristics of mast cells in the stroma of cervical squamous cell carcinoma were investigated using different staining methods of alcian blue-safranin, berberine sulphate and toluidine blue. The findings suggest that mast cells in the stroma of the squamous cell carcinoma of the cervix might contain a glycosaminoglycan different from that of typical connective tissue mast cell (CTMC) and mucosal mast cell (MCC). They thus, might be a specific subset of mast cells though sharing some of the staining properties of both CTMC and MMC. In the cervical carcinoma in situ, mast cells were distributed closely around the tumor or the involved glands with great density and the number was significantly higher than that of the invasive carcinoma (P < 0.01). In invasive carcinoma, mast cells were mainly distributed in the deep stroma and the stroma around the tumor, only a few in the stroma within the tumor (P < 0.01). The significance of mast cell reaction in the stroma of squamous cell carcinoma of the cervix is discussed.
