RESUMO

Carbapenem-resistant Acinetobacter baumannii infection is a critically prioritized pathogen by the World Health Organization and a cause for growing concern due to increased mortality among hospitalised patients. Phrenic nerve palsy is a rare complication of herpes zoster infection of the C3, C4, and C5 nerve roots. We present a case of bloodstream carbapenem-resistant A. baumannii infection in a Ghanaian patient with HIV type 1 infection and multiple risk factors, including unilateral diaphragmatic eventration with compression atelectasis likely secondary to phrenic nerve palsy due to herpes zoster infection, consequently leading to recurrent hospital and ICU admission. In this case, we emphasize the need for clinicians in LMICs to be aware of CRAB, in order to advocate for the availability of evidence-based medicines in resource-limited settings for appropriate treatment. In addition, we illustrate the importance of a high index of suspicion for infection with carbapenem-resistant organisms such as A. baumannii and highlight a rare and severe complication of herpes zoster infection in the form of phrenic nerve palsy and consequent diaphragmatic eventration.

Seventeen organochlorine pesticides (OCPs) and seven indicator polychlorinated biphenyls (PCBs) residues in 80 sediment samples from four cage aquaculture farms on the Volta Basin were determined to find out the extent of their contamination as well as their risk to biota in the aquatic ecosystem. The extracted residues of the OCPs and PCBs were analysed on a gas chromatograph equipped with an electron capture detector and mass spectrometer, respectively. Eleven (11) OCPs: methoxychlor, δ-HCH, o,p'-DDD, α-endosulphan, ß-HCH, o,p-DDE, p,p-DDE, p,p'-DDT, ß-endosulphan, endrin, and heptachlor and seven (7) PCBs: PCB 18, PCB 28, PCB 52, PCB 101, PCB 138, PCB 153, and PCB 180 were detected in the sediments from the farms. The OCPs level ranged < LOD - 33.0 µg/kg. δ-HCH (8.154 ± 0.414 µg/kg), α-endosulphan (6.000 ± 1.414 µg/kg), o,p'-DDD (2.010 ± 1.46 µg/kg), endrin (13.867 ± 8.716 µg/kg), and α-endosulphan (0.503 ± 0.398 µg/kg) were predominant with frequencies of detection 100%, 45%, 68%, 92%, and 25% in fish farms A, B, C, D and controls, respectively. PCBs concentrations ranged 0.042-5.320 ng/g wet weight. PCB 153 recorded the highest concentration of 3.328 ± 1.700 ng/g in farm D. PCB 18 and PCB 180 dominated the profiles in the sediment from all the farms. The ecotoxicological risk of the OCPs and PCBs in the surface sediment using the SQGs indicated that ∑HCH in the sediment from all the farms except that the controls may pose a health risk to the benthic organisms. Therefore, a comprehensive remedial intervention is required to arrest the situation.

Monitoring of pesticide residues in food commodities of plant origin is part of the regular controls on food to safeguard consumer's health. This study reports for the first time in Ghana a 3-year (2010-2012) monitoring of pesticide contamination of fruits and vegetables and their health implications. A total of 3483 samples were purchased in notable markets within Accra Metropolis and analysed for pesticide residues, employing the modified quick, easy, cheap, effective, rugged and safe analytical procedure. The results indicated that almost all the fruits and vegetables studied had residues above maximum residue limits (MRLs). The commodities with the greatest concentrations exceeding the European Union (EU) MRLs were long green beans (60.6%) and lettuce (57.1%) with watermelon (10%) and green pepper (8.6%) having the least. The relative occurrence of the pesticides was fenvalerate 11.3%, fenitrothion 5.6%, lambda-cyhalothrin 3.6%, dimethoate 3.2%, permethrin 2.7% and deltamethrin 2.2%. These results will serve as a baseline on which annual or other long-term studies could be compared with, thus emphasizing the need for continuous monitoring programmes to regulate trends of pesticide residues in fruits and vegetables to safeguard the consumers' health.

RESUMO

Pesticides are known to improve agriculture yield considerably leading to an increase in its application over the years. The use of pesticides has shown varying detrimental effects in humans as well as the environment. Presently, enough evidence is available to suggest their misuse and overuse in the last few decades in most developing nations primarily due to lack of education, endangering the lives of farmers as well as the entire population and environment. However, there is paucity of data especially over long durations in Ghana resulting in the absence of effective monitoring programs regarding pesticide application and subsequent contamination in fruits and vegetables. Therefore, this review discusses comprehensively pesticide type and use, importation, presence in fruits and vegetables, human exposure, and poisoning in Ghana. This is to alert the scientific community in Ghana of the need to further research into the potential implications of pesticide residues in food commodities in order to generate a comprehensive and reliable database which is key in drafting policies simultaneous with food regulation, suitable monitoring initiatives, assessment, and education to minimize their effects thereon.

An indicator of the disturbance of natural systems, the landscape development intensity (LDI) index, was used to assess the potential for land-use within watersheds to influence the production/accumulation of methyl-mercury (MeHg) in river sediments. Sediment samples were collected from locations impacted by well-identified land-use types within the Mobile-Alabama River Basin in Southeastern USA. The samples were analyzed for total-Hg (THg) and MeHg concentrations and the obtained values correlated to the calculated LDI indexes of the sampled watersheds to assess the impact of prevalent land use/land cover on MeHg accumulation in sediments. The results show that unlike THg, levels of MeHg found in sediments are impacted by the LDI indexes. Overall, certain combinations of land-use types within a given watershed appear to be more conducive to MeHg accumulation than others, therefore, pointing to the possibility of targeting land-use practices as potential means for reducing MeHg accumulation in sediments, and ultimately, fish contamination.

In this study, "Quick, Easy, Cheap, Effective, Rugged and Safe" 'QuEChERS' method was modified for the determination of 36 pesticides fortified at (0.01-1.0) mg kg(-1) in three vegetables and a fruit (lettuce, carrot, tomatoes and pineapples respectively) from Ghana. The method involved extraction with acetonitrile, phase separation with primary secondary amine and magnesium sulfate; the final injection solution was reconstituted in ethyl acetate. Organochlorine and synthetic pyrethroids residues were detected with electron capture detector whereas organophosphorus, pulsed flame photometric detector was used. The recoveries at different concentration levels (0.01, 0.1 and 1.0 mg kg(-1)) were in the range of 83% and 93% with relative standard deviation ranging from 2% to 10% (n = 5) and the coefficient of determination (R(2)) was greater than 0.99 for all the 36 pesticides. The method was successfully tested on 120 real samples from Accra markets and this proved to be useful for monitoring purposes particularly in laboratories that have no gas chromatography-mass spectrometry and liquid chromatography-mass spectrometry.

A number of pesticide residues in fruits were monitored at five markets in the Accra Metropolis for almost a year. Locally produced fruits (pawpaw and tomato) and imported apple were purchased from these selected markets in the metropolis and analyzed for pesticide residues by gas chromatography equipped with electron capture detector. In all, 320 sampled fruits were extracted and analyzed for pesticide residues, mainly organochlorines (γ-HCH, δ-HCH, aldrin, heptachlor, γ-chlordane, heptachlor epoxide, α-endosulfan, p,p'-DDE, endrin, ß-endosulfan, o,p'-DDT, endrin aldehyde, p,p'- DDT, endrin ketone, and methoxychlor). The data revealed that 32.8% of the fruit samples analyzed contained residues of the monitored insecticides above the accepted maximum residue limit (MRL) whereas 48.7% were below the MRL. Nonetheless, the continuous consumption of such fruits with modest pesticide levels can accumulate and could result in deadly chronic effects.

A detailed study has been carried out on groundwater in rural communities in the Tema District of the Greater Accra region of Ghana to establish the hydrochemistry and identify the various sources of contaminants as well assess the physical and chemical quality of the groundwater to ascertain their wholesomeness and the health impacts of the groundwater on the communities. The groundwater was found to vary considerably in terms of chemical and physical properties. Generally, the water was mildly acidic (pH 4.3-7.4), brackish to fresh, and undersaturated with respect to gypsum and halite. The majority of groundwater clustered toward Ca-Mg-SO(4) and Na-Cl facies. About 70% of boreholes sampled have elevated levels of NO(3)(-) -N, Cl(-), and SO(4)(2-) emanating from anthropogenic activities.

In the past two decades, a great deal of attention has been paid to the environmental fate of mercury (Hg), and this is exemplified by the growing number of international conferences devoted uniquely to Hg cycling and its impacts on ecosystem functions and life. This interest in the biogeochemistry of Hg has resulted in a significant improvement of our understanding of its impact on the environment and human health. However, both past and current research, have been primarily oriented toward the study of direct impact of anthropogenic activities on Hg cycling. Besides a few indirect effects such as the increase in Hg methylation observed in acid-rain impacted aquatic systems or the reported enhanced Hg bioaccumulation in newly flooded water reservoirs; changes in Hg transformations/fluxes that may be related to global change have received little attention. A case in point is the depletion of stratospheric ozone and the resulting increase in solar UV-radiation reaching the Earth. This review and critical discussion suggest that increasing UV-B radiation at earth's surface could have a significant and complex impact on Hg cycling including effects on Hg volatilization (photo-reduction), solubilization (photo-oxidation), methyl-Hg demethylation, and Hg methylation. Therefore, this paper is written to provoke discussions, and more importantly, to stimulate research on potential impacts of incoming solar UV-radiation on global Hg fluxes and any toxicity aspects of Hg that may become exacerbated by UV-radiation.
