RESUMO

The investigation of the individual anatomical variability of organs of the abdominal cavity in newborns has shown that the variant of fixation of the duodenum, structure of the small intestine mesenterium, pattern of the organization of the peritoneal lining as well as the disposition and fixation of the large intestine are of major practical importance in urgent operations for congenital ileus. Variants of the shape and disposition of the large intestine in neonatals should be taken into consideration in roentgenological examinations and in mobilization of the large intestine.

RESUMO

Newborns with the ileus caused by developmental defects of the digestive tract have, as a rule, suprastenotic dilatations of all the intestine or a considerable part of it higher than the ileus zone and a great difference in the width of the lumen of the intestine portions left after resection of the injured area. In the Center of Developmental Defects of Newborns 190 patients with ileus have been treated for the recent 10 years. In 37 cases T-shaped bypass anastomosis was used which has considerable advantages over "dead" anastomosis. T-shaped ileostomy allows unloading the dilated adducting part of the intestine overfilled with the contents in early terms after operation and simultaneously fulfilling "the feeding" into the abducting part of the intestine.

RESUMO

Malformations of the structure and fixation of the small intestine mesentery in nenates constitute essential morphologic predisposing factors for the occurrence of an "isolated" volvulus and are responsible for clinico-radiological peculiarities of the pathology. The children, operated upon for "isolated" small intestine volvulus, who had shown this pathology, should be under a prolonged medical supervision.

RESUMO

The systematization of malrotations according to their morphological patterns depending on topographic peculiarities of the duodenum and colon permits to ascertain an anatomical variant of the malformation in each case, which helps to make an adequate choice of the procedure of surgical correction. The author is convinced that in time of operation for any type of fixation and rotation abnormality it is necessary to explore the whole intestine, to evaluate the status and development of the superior mesenteric artery, to free the duodenum from abdominal formations along the outside curvature and to relieve the tissue pressure from the vessels of the pathologically developed mesentery.

RESUMO

The anatomical structure of the upper and lower segments of the esophagus and tracheoesophageal fistula in esophageal atresia in newborn in newborn infants conditions the clinical picture, character of surgical intervention and postoperative course. The gravity of pneumonic changes in the lung is dependent not only on terms of child's admission, but also on the width of esophagotracheal fistula, while bifurcation fistulas, sometimes, cause diagnostic erros.
