[Surgery of insulinomas using an artificial pancreas]. / La chirurgie des insulinomes sous pancréas artificiel.

The risks of failure of conventional treatment of insulinomas should be reduced, it seems, when the surgical operation is carried out using an artificial pancreas. The Biostator, used with feed back controlled glucose infusion in 32 cases of which two personal effectively allows one firstly to operate in complete security since it is programmed to maintain a pre-determined glycaemia during the various stages of the operation; it also allows one to put into evidence an occult tumour in a segment rendered suspect due to the sudden hypoglycaemia observed during its mobilisation; it finally allows one by the appearance of a post-excision hyperglycaemia peak to find out whether the procedure has been complete.

[Pheochromocytoma and its diagnostic pitfalls]. / Le phéochromocytome et ses pièges diagnostiques.

A sixty-year-old man experienced successively over two years several episodes of circulatory collapse and meningeal hemorrhage, a myocardial infarction, an episode of ketoacidosis and a seizure before acute abdominal pain with fever related to the sudden, partial, necrosis of his tumor, led to the discovery of a pheochromocytoma. This observation exemplifies the multiple clinical aspects and diagnostic pitfalls of this secreting tumor. It underscores the misleading nature of normotensive pheochromocytomas.

[Pelvic-peritoneal recurrences after amputation of the rectum for cancer. Report of 23 cases (author's transl)]. / Les récidives pelvi-périnéales après amputation du rectum pour cancer. A propos de 23 observations.

Of 79 patients surviving after amputation of the rectum for cancer, local and regional recurrences occurred in 23 cases, in 3 out of 4 of these during the 2-year period following operation. Only 3 patients survived. This illustrates the frequency and serious nature of these recurrent lesions. Based on their experience, the authors consider the risk of recurrence to be doubled if one of the following factors is present : patient under 60 years of age, cancer located in the lower rectum, tumour over 4 cm in diameter, stages B or C according to Dukes' classification, an anaplastic or colloid structure. Enzyme assay and computed tomography are unfortunately not determining factors for early diagnosis and effective therapy of pelvic-peritoneal recurrences. the authors suggest, therefore, though it was never employed in their own series, that pre-operative irradiation could be of value, in those patients that have a high risk of developing local or regional recurrences. The general use of this technique is not suggested because its known inconveniences.

[Statistical analysis of results of operative treatment of 223 cases of left colon cancer (author's transl)]. / Réflexions statistiques sur une série de 223 interventions pour cancer du côlon gauche.

Statistical analysis of results of 223 operations for left colon cancer over a period of ten years is presented. The patients (age, condition), and the clinical expression (urgent or cold surgery) are discussed. The authors then give the lesions a sort of identity card of macroscopic findings (topography, local extension, invasion of glands, metastatic spread) to enable interpretation of the long-term survival results as a function of each parameter. Their conclusions are that, apart from left hemicolectomy conducted for oncological reasons or for safety of technique, no element demonstrates the superiority of standard left hemicolectomy over segmentary colectomy. They also adopt the principle of a proximal colostomy in certain cases that are difficult to treat, this procedure being for them a definite factor of security, but also a risk factor. An urgent bypass operation is conducted except when the lesion requires excision. If conditions are suitable for immediate re-establishment of continuity, which rarely happens, they carry out a systematic protective subjacent colostomy.

Fragments of foetal pancreatic buds taken before the 10th week of gestation, stored at - 196 degrees C and cultivated on artificial media were implanted in an adult, insulin-dependent diabetic patient. Following a 2 weeks latent period, the clinical symptoms of diabetes regressed within 2 months, although all insulin treatment had been withdrawn. These results have now been maintained for 11 months, but major overloads in carbohydrates are imperfectly controlled.

[Abrikossof tumor in the esophagus]. / Tumeur d'Abrikossof à localisation oesophagienne.

Among the benign tumours of the oesophagus, the discovery of an Abrikossof tumour is exceptional. Although modern methods of investigation now permit recognition of this tumour and suspect its benign nature, only histology permits one to determine its precise nature, either by biopsy or by frozen section. Histogenetically, the schwann cell and histiocytic theories are not completely incompatible to the extent that they are considered from the point of view of general pathology, where there is evidence in favour of the mesoectodermic origin of the granular cell myoblastomas. In all cases, surgical excision is necessary. This resection of the myoblastoma permits a permanent cure.
