RESUMO

Concrements were found in the ducts of 172 patients subjected to secondary surgical interventions on bile ducts. The detailed analysis of the data failed to answer the question whether the stones could be formed immediately in the ducts without such conditions as a mechanical obstacle for the outflow of bile, the presence of foreign bodies and a big stump of the cystic duct. Such a formation of stones can take place in very few cases. The authors have shown that control cholangiography and especially choledochoscopy can reduce the danger to leave the stones in the ducts, but does not eliminate the danger completely. Nevertheless they think that now it is possible to lessen the indications for biliodigestive anastomoses in choledocholithiasis since the retained stones can be revealed within the first days after surgery and, as a rule, removed with instruments or by other methods using the drainage left in the ducts.

RESUMO

Within 30 years of their work the authors observed 151 cases of postoperative peritonitis, which made 1.1% of all the laparotomies carried out during this time. None of the known symptoms of peritonitis can be taken as a constant sign of this dangerous complication. In the early recognition of postoperative peritonitis and the determination of the indications for repeated laparotomy the experience of a surgeon, well acquainted with the diagnosis of postoperative complications in the abdominal organs, is the most essential. The diagnostic laparotomy (microlaparotomy) is quite justified even in case when peritonitis symptoms are not well marked, but a progressive general intoxication cannot be explained through any other causes.

RESUMO

An experience with the construction of 138 anastomoses between the external bile ducts and the duodenum is analysed. The anastomoses were constructed in 47 patients with two-raw and in 91--with one-raw sutures. A partial nonhermeticity of the suture line was observed as seldom both in cases with one-raw and two-raw sutures. In constructing anastomoses one-raw suture is felt to be more preferable, since the formation of stoma with a one-raw suture is felt to be more preferable, since the formation of stoma with a one-raw suture simplifies the procedure.
