RESUMO

The aim of the present study was to evaluate the use of collagen, elastin, or chitosan biomaterial for bone reconstruction in rats submitted or not to experimental alcoholism. Wistar male rats were divided into eight groups, submitted to chronic alcohol ingestion (G5 to G8) or not (G1 to G4). Nasal bone defects were filled with clot in animals of G1 and G5 and with collagen, elastin, and chitosan grafts in G2/G6, G3/G7, and G4/G8, respectively. Six weeks after, all specimens underwent radiographic, tomographic, and microscopic evaluations. Bone mineral density was lower in the defect area in alcoholic animals compared to the abstainer animals. Bone neoformation was greater in the abstainer groups receiving the elastin membrane and in abstainer and alcoholic rats receiving the chitosan membrane (15.78 ± 1.19, 27.81 ± 0.91, 47.29 ± 0.97, 42.69 ± 1.52, 13.81 ± 1.60, 18.59 ± 1.37, 16.54 ± 0.89, and 37.06 ± 1.17 in G1 to G8, respectively). In conclusion, osteogenesis and bone density were more expressive after the application of the elastin matrix in abstainer animals and of the chitosan matrix in both abstainer and alcoholic animals. Chronic alcohol ingestion resulted in lower bone formation and greater formation of fibrous connective tissue.

INTRODUCTION: Mastopexy with an implant is undoubtedly one of the most challenging surgeries in cosmetic surgery since it involves two overlapping procedures. The aim of this study was to demonstrate that by using horizontal glandular flaps in the sulcus in mastopexy with implants, it is possible to reduce the number of postoperative complications. METHODS: Sixty-three female breast surgery patients with some degree of flaccidity who underwent surgery from July 2018 to January 2020 were selected. All surgeries were performed in one procedure with an initial resection of the excess skin associated with a horizontal flap of the upper pedicle that promoted the protection of the implants and decreased the surgical wound tension. RESULTS: Forty-seven patients were included in the sample, for a total of 94 breasts. The motivation of the surgery was esthetic in all cases. Related to complications, we had 1 (1.06%) case of seroma, 1 (1.06%) case suffering from areola necrosis, 6 (6.3%) cases with scar revisions, 10 (10.6%) cases with complications in the transition from the "T" and no case of hematoma, infection or extrusion of the implants. The follow-up time was 12 months. CONCLUSION: This technique allows the surgeon to perform a resection of excess skin and place the implants in one procedure, thereby decreasing the tension in the suture line, protecting the implants and forming a protective stabilizing brace in the mammary groove. The use of low-level laser therapy (LLLT) with biomodulatory effects on biological tissues, currently called photobiomodulation therapy (PBMT), assists in healing and reduces inflammation. The application of biomaterials has emerged in bone reconstructive surgery, especially the use of bovine bone due to its biocompatibility. Due to the many benefits related to the use of PBMT and bovine bones, the aim of this research was to review the literature to verify the relationship between PBMT and the application of bovine bone in bone reconstruction surgeries. We chose the PubMed/MEDLINE, Web of Science, and Scopus databases for the search by matching the keywords: "Bovine bone AND low-level laser therapy", "Bovine bone AND photobiomodulation therapy", "Xenograft AND low-level laser therapy", and "Xenograft AND photobiomodulation therapy". The initial search of the three databases retrieved 240 articles, 18 of which met all inclusion criteria. In the studies concerning animals (17 in total), there was evidence of PBMT assisting in biomaterial-related conduction, formation of new bone, bone healing, immunomarker expression, increasing collagen fibers, and local inflammation reduction. However, the results disagreed with regard to the resorption of biomaterial particles. The only human study showed that PBMT with bovine bone was effective for periodontal regeneration. It was concluded that PBMT assists the process in bone reconstruction when associated with bovine bone, despite divergences between applied protocols.

Introdução: A mama está diretamente relacionada com a beleza, autoestima e sexualidade feminina. A simastia é uma alteração inestética mamária, caracterizada pela união das duas mamas na região pré-esternal. O autor demonstra uma técnica cirúrgica para correção de simastia congênita. Métodos: O trabalho relata o caso de uma paciente jovem com história prévia de hipertrofia mamária e simastia, que realizara previamente duas mamoplastias e uma lipoaspiração no seio mamário, sem correção da simastia. O autor realizou a ressecção do excesso de tecido no seio mamário e um lifting reverso dessa região, associado com um retalho em avanço do abdome superior. Resultados: Houve completa resolução da simastia com o tratamento proposto. Conclusão: O tratamento realizado pelo autor demonstrou ser mais uma alternativa cirúrgica segura no tratamento da simastia.


Introduction: Breasts are associated with a feeling of beauty, self-esteem, and female sexuality . Symmastia is a condition, characterized by the confluence of both breasts in the presternal region. The author demonstrates a surgical technique for correction of congenital symmastia. Methods: This is the case report of a young patient with previous history of breast hypertrophy and symmastia. She had previously undergone two mammoplasties and one liposuction of the breast, without correction of the symmastia . The author performed resection of the excess tissue in the breast and a reverse lifting of this region, with an upper abdominal advancement flap. Results: There was complete resolution of the symmastia. Conclusion: The treatment performed by the author is a safe surgical alternative for the treatment of symmastia.

INTRODUÇÃO: A ritidoplastia tem demonstrado excelentes resultados no tratamento da flacidez facial ao longo dos tempos, porém em alguns casos selecionados, nos quais o paciente apresenta flacidez facial acentuada associada com implantação capilar pré-auricular alta, observamos, após a tração dos retalhos, uma ascensão da implantação capilar acima da raiz da helix, causando um aspecto inestético facial e configurando um estigma cirúrgico para o procedimento. O objetivo desse estudo é demonstrar uma técnica cirúrgica com intuito de prevenir a ascensão capilar e manter o seu correto alinhamento, no lifting facial. MÉTODOS: Foram operados 25 pacientes, submetidos à ritidoplastia, dos quais 12 realizamos a península capilar segmentada. Foi realizada uma análise descritiva desse grupo, quanto à idade, altura da implantação capilar, tempo cirúrgico, elevação do "pé do cabelo", necrose, hematoma pós-operatório, cicatrizes inestéticas e neuropraxia. RESULTADOS: Em relação à população analisada, realizamos a península capilar segmentada em 12 pacientes, ou seja, 48%, a idade variou entre 41 e 74 anos, sendo a média de 60 anos. O retorno às atividades habituais variou de 10 a 21 dias. Não ocorreram alterações inestéticas da implantação da linha capilar na "costeleta" nos 12 pacientes submetidos à península capilar segmentada, houve um (4%) caso de hematoma, nenhum caso de necrose ou neuropraxia. CONCLUSÃO: A península capilar segmentada demonstrou ser uma técnica de fácil execução, baixo índice de complicações e com resultados estéticos satisfatórios.


INTRODUCTION: Rhytidoplasty has shown excellent results in the treatment of facial laxity over the years, but in a few cases in which the patient has increased facial laxity associated with high preauricular scalp implantation, we observed elevation of implantation above the root of the helix after flap traction, resulting in an unesthetic facial appearance and marring the surgical procedure. The objective of this study was to demonstrate a surgical technique designed to prevent hairline recession and maintain correct alignment for the facelift. METHODS: Of 25 patients who underwent rhytidoplasty, 12 received a segmented scalp peninsula. Descriptive analysis in this group included age, level of scalp implantation, surgical time, elevation of the hairline, necrosis, postoperative hematoma, unesthetic scars, and neuropraxia. RESULTS: We analyzed the results of a segmented scalp peninsula in 12 patients, ranging in age between 41 and 74 years, with a mean of 60 years. The return to usual activities ranged from 10 to 21 days. No unesthetic changes resulted from placement of the hairline on the sideburn in these 12 patients; there was one (4%) case of hematoma, but no case of necrosis or neuropraxia. CONCLUSION: The segmented scalp peninsula is an easily performed technique, with a low risk of complications and with satisfactory esthetic results.
