RESUMO

In present work we studied the morphological features of the esophageal mucosa in 63 children with gastroesophageal reflux disease (GERD). The biopsies were taken at level of 3 cm above a Z-line and at level of 0.5-1 cm above a Z-line. The results of our study showed that the mucosa of the esophago-gastric junction may contain areas covered with columnar epithelium of 44.4% of children in the biopsies from the level of 0.5-1.0 cm above the Z-line. Inflammatory changes in the mucosa of the esophago-gastric junction identified in 71.4% of cases. The inflammation in the majority of cases (82.1%) was observed in the presence of H. pylori infection (p < 0.001). In addition, H. pylori in our study, we noted the relationship detection carditis in overweight child. When compared with the height-weight parameters, the excess body weight was observed in 17 of 28 patients. We couldn't found increasing detection of the cardia in patients with erosive GERD compared with non-erosive variants.

RESUMO

Fatty acid composition of erythrocyte and leukocyte membranes has been studied in children (with normal body mass and obesity) with inflammatory diseases of the gastrointestinal tract and children with bronchial asthma in comparison with basically healthy children. Fifty seven children aged from 7 to 14 years were examined: 13 with inflammatory diseases of the gastrointestinal tract (IDGIT) (eosophagitis, gastroduodenitis, stomach ulcer), 25 with obesity stages (I-III) complicated by IDGIT, 9 with bronchial asthma and 10 basically healthy children. The study revealed that both IDGIT and bronchial asthma caused significant and similar changes in fatty acid composition of cell membranes. These included accumulation of omega3-eicosapentaenoic acid (EPA) and the decrease of docosahexaenoic acid (DHA); this phenomenon observed in both erythrocyte and leukocyte membranes suggests a common feature of the detected changes in fatty acid composition of cell membranes in inflammation. There was a significant decrease in the level of membrane omega6 polyunsaturated fatty acids (PUFA), first of all arachidonic acid and total omega6 PUFA. Consequently, EPA accumulation in membranes may be a compensatory response to low dietary arachidonic acid supply and/or its increased synthesis of pro-inflammatory eicosanoids (prostaglandins, leukotriens, thromboxans) during inflammatory process.

In present work we studied the morphological features of the esophageal mucosa in 63 children with endoscopic diagnosis of the distal esophagitis having overweight and normal weight of a body. The biopsies were taken at level of 3 cm above a Z-line and at level of 1 cm above a Z-line. Dystrophic and dysregenerative changes were revealed at the majority of children and half of children had inflammatory changes of the esophageal mucosa regardless of weight of a body. These changes are more pronounced at level of 1 cm above a Z-line, their occurrence decreases with a distance from low esophageal sphincter. We used the pathology score system for assess the esophageal biopsies. According our scale we obtained following results: at level of 1 cm above Z-lines at 95% of children had the normal, minimum or mild features of esophagitis regardless of weight of a body. Morphological evidence of a reflux esophagitis was diagnosed statistically more often at level of 1 cm above Z lines in comparison with level of 3 cm above Z-lines (p < 0.01) as among children with overweight of the body (78 and 43% accordingly), and among children with normal weight of the body (78 and 35% accordingly). The obtained data will be allowed to avoid hyperdiagnostics of esophageal lesions in children.

RESUMO

Histospectrophotometric study of DNA content shows that, at the lung periphery, oval structures with atypia, adenomatosis with atypia, dysplasia and basal cell hyperplasia with bronchial epithelium atypia should be regarded as precancerous lesions. Carcinoma in situ and bronchoalveolar carcinoma are distinguished from the foci of epithelial dysplasia by an increase of DNA content and appearance of hetero- and aneuploidy. Aneuploidy is observed in 97% peripheral lung carcinoma and is combined with a heterogeneity of cells by their DNA content. Poorly differentiated squamous cell carcinoma, adenocarcinoma and large cell carcinoma are characterized by a marked heterogeneity which correlates with a degree of tumour morphological atypia. Bronchoalveolar carcinoma is a heterogeneous group of tumours with a different histogenesis and pronounced anaplasia which is expressed in a various degree of polyaneuploidy and heteroploidy. Small cell lung carcinoma, a special group of tumours, which, as distinct from other carcinoma types, is characterized by a low, sometimes diploid DNA content which does not correlate with its malignancy.
