RESUMO

Many techniques and pulse sequences have been devised for the assessment of the anterior cruclate ligament. The present study compares fast spin echo (FSE) imaging to conventional spin echo imaging at a field strength of 1.5 T in an effort to determine if these sequences are diagnostically equivalent. Where available, arthroscopy was also done. A total of 52 patients were imaged using both FSE and conventional spin echo sequences. Eight volunteers were used as controls. Arthroscopy was performed on 10 patients. The anterior cruciate ligament was assessed in a blinded fashion by three radiologists. The Kappa statistic was then used to determine the percentage agreement between FSE and conventional spin echo imaging. Fast spin echo sequencing demonstrated a sensitivity of 100%, a specificity of 94.8% and an accuracy of 96.3% when compared to arthroscopy. Conventional spin echo imaging and arthroscopy had a sensitivity of 100%, specificity of 84.6% and an accuracy of 88.9%. The remaining 34 patients who did not undergo arthroscopy were followed clinically because clinical and imaging findings were not suggestive of ACL tears. These demonstrated 72% agreement between FSE and conventional spin echo imaging using the Kappa statistic, with regards to calling ACL normal or having only a low-grade partial tear. Fast spin echo imaging produces images of the anterior cruciate ligament that have similar diagnostic accuracy to conventional spin echo images (P < 0.05) within a much shorter scan time. These results however, require further validation in a larger group, preferably with arthroscopic correlation.

PURPOSE: To describe the radiographic features and potential causes of urethral disruption in pancreas transplant recipients. MATERIALS AND METHODS: Eight episodes of urinary extravasation were depicted with retrograde urethrography in five male patients who had undergone pancreatic transplantation. The patients' medical records were reviewed to determine a cause of the extravasations. RESULTS: Four extravasations occurred at the proximal (deep) bulbar urethra, three at the bulbomembranous junction of the urethra, and one at the distal bulbar urethra. Four of eight cases of extravasation were preceded by recent cystoscopy or placement of a Foley catheter; one case was preceded by possible urethral injury due to a fall. CONCLUSION: Urethral disruption occurs as a complication of pancreatic transplantation. It has so far been seen only in male patients and occurs at the bulbar urethra or bulbomembranous junction. If the treating physician is unaware of this condition, diagnosis and institution of appropriate therapy may be delayed. Recent prior urinary tract instrumentation or trauma may be predisposing factors in urinary extravasation.

RESUMO

The aim of this study was to assess the value of the measurement of os-placenta distance by translabial ultrasonography in the evaluation of placenta previa. This method was used in 40 women with suspected placenta previa to measure the distance between the placenta and internal cervical os. Sonographic diagnoses were compared to placental location determined at delivery. Translabial ultrasonography proved superior to the transabdominal route in both diagnosis and exclusion of placenta previa. Measurement of the os-placenta distance can be used as an adjunct to clinical assessment to predict the likelihood of safe vaginal delivery in cases of suspected placenta previa.

RESUMO

The authors describe a previously unreported complication of insertion of an automatic implantable cardioverter-defibrillator via the left subcostal surgical approach. Splenic hematoma, intraperitoneal hemorrhage and hypotension developed in a 66-year-old man within 5 days of implantation, and the patient underwent splenectomy.

OBJECTIVE: To determine whether the rate of detecting a tumour, arteriovenous malformation (AVM) or aneurysm with the use of enhanced or unenhanced computed tomography (CT) is significant in patients with chronic headache and to calculate the cost. DESIGN: Case series. SETTING: Chronic headache clinic at a tertiary care referral centre. PATIENTS: All 373 consecutive patients with chronic headache (284 women, 89 men) referred for CT scanning from May 1987 to October 1992 who met one or more of the following criteria: increased severity of symptoms or resistance to appropriate drug therapy (287 patients [76.9%]), change in characteristics or pattern of headache (78 [20.9%]) or family history of intracranial structural lesion (8 [2.1%]). INTERVENTIONS: CT scans of the head were enhanced with nonionic contrast medium (292 scans), were unenhanced (70) or involved both methods (40). OUTCOME MEASURES: Number and nature of minor and major findings, and total price per scan. RESULTS: Of the 402 CT scans 14 (95% confidence interval [CI] 7 to 21) revealed minor findings that did not alter patient management: infarct (9 scans), cerebral atrophy (2), cavum vergae (1), hyperostosis frontalis interna (1) and communicating hydrocephalus (1). Four scans (95% CI 0 to 8) showed significant lesions: osteoma (2), low-grade glioma (1) and aneurysm (1); only the aneurysm was treated. There were no cases of AVM. An unenhanced scan cost $82.63 and an enhanced scan $204.05. The cost per significant finding was over $18,000. In all, it cost $74,243 to find one treatable vascular lesion. CONCLUSIONS: The detection rate of CT scanning in patients with chronic headache is similar to that expected in the general population, provided the neurologic findings are normal. The cost of detecting intracranial lesions in this patient population is high [corrected].

RESUMO

Chondromyxoid fibroma is an uncommon bone tumour, found predominantly in the metaphyses of the long bones of the lower limbs. The authors describe a patient in whom the tumour arose from the occipital bone. To the authors' knowledge, this is only the second report of chondromyxoid fibroma occurring in this location.
