Hemosorption was used in 35 patients with chronically disordered arterial circulation in the lower limbs. Atherosclerotic vascular lesions were noted in all the patients. The study group was composed of patients with diffuse lesions in the lower limbs' arterial bed, where surgical treatment was not indicated because of poor general condition, as well as those after reconstructive vascular surgery performed to prevent thrombosis and prolong operation of the shunts. Clinical parameters, blood lipid composition, rheovasographic, thermometric, thermographic, capillaroscopic, Doppler-ultrasonometric findings, and muscular flow (using local 133Xe clearance technique) were examined in all the patients before, and on days 1, 3, 7, 14 after the hemosorption. A stable short- as well as long-term improvement of regional circulation in lower limbs is noted after hemosorption. It appears to be a new promising method for the treatment of patients with atherosclerotic vascular lesion of lower limbs, where surgical treatment is not indicated, and survivors of vascular surgery for shunt thrombosis prevention.

The authors have made 390 operations on ductus choledochus. Indications for choledochotomy were choledocholithiasis, jaundice, cholangitis and others. The ideal choledochotomy was fulfilled in 7,1% of cases as a completion of diagnostic manipulations on the duct. The external drainage of ductus choledochus was performed in peritonitis, severe cholangitis and inflammatory alterations of the hepatoduodenal ligament and in weak senile patients (mortality--11,7%). Multiple stones and great dilatation of the duct (mortality--4,5%) were considered to serve indications for supraduodenal choledochoduodenostomy (38,7%). Papillotomy was made in fixed ampula stones, stenosis of Vater's papilla (18,2%). Lethality in such cases was twice as high as after choledochoduodenostomy (8,5%). The authors believe peritonitis and duodenostasis to be contraindications against the internal drainage of biliary ducts.

The work analyzes the first experience of clinical using the xenograft from the neonatal umbilical cord vein for different kinds of arterial plasty in chronic occlusion diseases. For a year 19 patients were operated. In most cases the femoropopliteal shunting or prosthetics above or under the knee joint was made. The observations continued from 3 till 13 months.

It is stated that latent spread of thrombosis in v. saphena magna in proximal direction is observed in 37--40%. Radioisotope technic was employed to indicate the level of thrombus. The operative technic is described.

107 patients were explored using a radioisotope method. An acute thrombosis of the lower extremities veins postoperatively developed in 71 of them (66.3%). The radioisotope method is considered to be a simple and safe method for early diagnosis of the postoperative venous thrombosis.
