RESUMO

This 32-centre multicentre study addresses the lack of knowledge about the prevalence and significance of microbial keratitis (MK) associated with contact lens (CL) wear in Spain. A total of 304 cases recruited from 32 hospitals were studied and showed that infectious keratitis associated with contact lens wear mainly affects young women during the summer months. In this study, soft lenses with monthly replacement and single solution cleaning were most commonly used, purchased and fitted in opticians' shops. Common risk factors were identified among users, such as topping off solutions, prolonging the life of lenses, and frequently sleeping, swimming and showering with lenses. Overnight lens wear was significantly associated with a higher incidence of corneal opacities, and the presence of Pseudomonas aeruginosa in bacterial cultures was associated with more severe sequelae and a greater need for corneal transplantation. Although most cases were benign, the time taken to heal was long, which poses a problem for working patients. This study provides valuable epidemiological, microbiological and risk factor information and estimates the incidence of CL related MK in Spain to be approximately 1 case per 30,000 inhabitants per year.

RESUMO

Congenital aniridia is a multisystemic genetic disease due to a mutation in PAX6 gene which severely affects the development and functionality of the human eyes. In patients affected by the mutation, aside from the absence or defects of iris tissue formation, abnormalities in position or opacities of the crystalline lens, macular hypoplasia, ocular surface disease is the main cause of visual loss and the deterioration of the quality of life of most patients. Limbal stem cell deficiency combined with tear film instability and secondary dry eye cause aniridic keratopathy which, in advanced stages, ends up in corneal opacification. In this paper, the actual knowledge about congenital aniridia keratopathy physiopathology and medical and surgical treatment options and their efficacy are discussed. Indications and results of topical treatments with artificial tears and blood-derivatives in its initial stages, and different surgical techniques as limbal stem cell transplantation, keratoplasty and keratoprostheses are reviewed. Finally, recent advances and results in regenerative medicine techniques with ex vivo stem cell cultivation or other types of cultivated cells are presented.

PURPOSE: Comparison of two immunosuppressive strategies after limbal allograft transplantation in an experimental model of ocular surface chemical burn. METHODS: Twenty albino rabbits underwent corneal and limbal chemical burn with NaOH. One week later limbal transplantation was performed. Clinical assessments included measurements of epithelial defect, stromal opacity and neovascularization areas, using a computerized planimetry system 0, 7, 15, 30, 60 and 90 days after the chemical burn. Histologic examination was performed at 3 months, after sacrifice of the animals. Two immunosuppressive strategies were evaluated: topical cyclosporin A on constant dosage (group 1), and a combination of topical cyclosporin A and dexamethasone followed by a tapered dosage of topical cyclosporin A (group 2). RESULTS: No statistical differences were observed between both groups regarding all of the variables 3 months after the chemical burn. Differences in the stromal opacity area were detected on day 7 (43.33 D.E. 4.84 vs 36.67 D.E. 3.88; p<0.01), day 15 (41.68 3 D.E.95 vs 33.6 D.E. 1.99; p<0.01), and day 30 (40.1 D.E. 5.45 vs 32.52 D.E. 7.33; p<0.05). As for the neovascularization area, differences were detected on day 7 (1.92 D.E. 0.48 vs 3.73 D.E. 1.18; p<0.01), day 15 (7.08 D.E. 1.54 vs 4.42 D.E. 1.68; p<0.01), and day 60 (4.9 D.E. 2.58 vs 9.65 D.E. 5.55; p<0.05). CONCLUSIONS: There are no long-term differences between both immunosuppressive strategies regarding all the studied variables (Arch Soc Esp Oftalmol 2003; 78: 29-34).

RESUMO

Results of an electro-oculographic study in 123 cases of esotropia emphasized the important role of the angle of deviation in the appearance of dissociated dyssynergia, as this phenomenon was present in all cases with a greater than 30 degree angle convergent strabismus. Small angle esotropia alone produces dyssynergic tracings interpretable as arising from a blocking syndrome. Furthermore, a new method for measuring dyssynergia was found to possess certain advantages, the position of the electrodes in relation to the globe, particularly when it is deviated, being of primary importance to obtain satisfactory tracings. This problem can be avoided by a new method proposed, employing fully compensated prisms for electro-oculography.
