RESUMO

Growth of Malassezia furfur in the intravascular catheter used for administration of lipid emulsion resulted in occlusion of deep intravascular Silastic catheters in 12 infants in 2 intensive care nurseries. At the time of occlusion visible growth was noted in the clear catheter which was connected to the Silastic intravascular line. Five infants showed clinical signs suggestive of sepsis. The yield of M. furfur from blood cultures and catheter tips was low even when oil enrichment was used. The highest yield of M. furfur was found in the connecting catheter (11 of 11). The source from and the route by which M. furfur entered the catheter remain unclear. The potential portals of entry include the proximal and distal ends of the connecting catheter as well as the colonized skin of the infants and caretakers.

RESUMO

A newborn female infant who experienced an intramural bladder hematoma with upper tract obstruction secondary to a suprapubic aspiration is described. The literature is reviewed and the conservative management of this child is discussed.

RESUMO

Two cases of lactobezoar formation were seen in premature low-birth-weight infants in a 24 cal/oz formula fortified with medium-chain triglycerides and glucose polymers given by continuous nasogastric infusion. Medical management included cessation of oral feedings and increased hydration and led to a rapid dissolution of the problem. The use of this preparation in small low-birth-weight infants is not recommended as an initial formula, and it should only be used when oral feedings have been well tolerated in a growing premature infant.

RESUMO

A child with a lateral trunk replacing one side of the external nose is presented with a detailed X-ray study of the deformed head. In the explanation of the etiology and teratogenesis of this facial anomaly, the modern embryologic development of the head is reviewed and correlated with the findings of this anomaly. It is felt that the lesion responsible for a proboscis lateralis involves the organizer tissue for the naso-face. As a secondary phenomenon, the development of the maxillary process is retarded and a spectrum of defects, common to many mid-face derangements, is identified and explained.

RESUMO

Fatal myocardial infarction occurring in a neonate is reported. The patient presented with a clinical picture of cardiogenic shock simulating a hypoplastic left heart syndrome. Etiology of the myocardial infarction is uncertain for the coronary arteries were patent, anatomically and histologically normal, and there was no significant associated cardiac defect. The possible etiologies in relationship to myocardial infarction in the neonatal period are presented.
