RESUMO

Morphological diagnosis of cervical intraepithelial neoplastic (CIN) lesions sometimes requires an additional study of molecular markers. Cyclin D1 is a key regulatory protein, which regulates cell cycle progression from G1 to S phase. Disruption of G1/S regulatory mechanisms is basic mechanism of HPV mediated malignant transformation of cervical epithelium. The aim of the research was to study the peculiarities of cyclin D1 protein expression in reproductive and menopausal women with cervical hyperkeratosis. We examined cyclin D1 protein expression in 381 reproductive and 233 menopausal women with cyto-colposcopically detected and histologically proved hyperkeratosis, using immunohistochemical method. Monoclonal ready to use (RTU) antibody against cyclin D1 antigen (Dako) was used. Cyclin D1 positivity in ≤50% of cells considered as low and in ≥50% - as high expression. High expression of cyclin D1 was present in CIN1 of 93,3% reproductive and 66,7% menopausal women, whilst in CINII the high expression was revealed in 53,3% and 43,8% respectively. The weak expression of cyclin D1 was present in only one cases of CINIII, other CINIII cases were all negative. The expression of cyclin D1 protein in cervical intraepithelial neoplastic lesions is not regular, however the overexpression of cyclin D1 is almost always present in CIN1 of reproductive women, in which it might be considered as an additional diagnostic marker.

RESUMO

We have determined the effectiveness of cryodestruction and radio wave surgery during the treatment of background diseases of cervical lesions. To 52 patients with background diseases of cervical lesions the radio wave surgery has been performed and for 81 patients cryodestruction has been carried out, 65 patients got laser, anti-virus and immunomodulating therapy after cryosurgical treatment. The usage of radio wave surgery and cryodestruction helps to avoid unpleasant postoperative consequences such as pain, edema, infection and postoperative shock from loss of blood, which quite often arise after application of traditional methods of surgical operation. The advantage of cryodestruction and radio wave surgery at treatment of background diseases of cervical lesions in comparison with other invasive methods of treatment of given pathology is the absence of scars and preservation of cervical lesions' reproductive function. During the radio wave surgery operation the depth of destruction of tissues are being controlled by the adjustment of the device's capacity, which in our opinion is an advantage of radio wave method of treatment of cervical lesions' pathology in comparison with the method of cryodestruction.

The aim of our investigation was the detection of endocrine-metabolic disorders in patients with hyperplastic processes of endomyometrium, uterine cervix and mammary glands. 88 patients of reproductive age with several gynaecological complaints have been investigated. 72 patients with hyperplastic processes in endomyometrium, uterine cervix (hyperplasia, polyposis, myoma) and mammary glands (fibroadenomatosis, adenomatosis) were selected in main group. Control group consisted of 16 patients without any hyperplastic processes of reproductive organs. Metabolic syndrome in main group was revealed in 28% of cases, in control - 18,8% (chi(2)=3,95, p=0,047); insulin resistance - 37,5% and 18,7% (chi(2)=4,59, p=0,033), respectively; obesity - 52,8% and 25,0% (chi(2)=4,05, p=0,045), respectively; dyslipidemia - 52,8% and 0,0%; hypertension - 26,4% and 12,5% (chi(2)=1,88, p=NS), respectively. Blood leptin level in main group was - 13,7+/-10,9 ng/ml, and in control - 5,0+/-2,9 ng/ml (p=0,005). Our results suggest that metabolic syndrome and its components significantly influences the formation of hyperplastic processes of endomyometrium, uterine cervix and mammary glands. Blood leptin level is significantly increased in patients with hyperplastic pathologies.

Possibilities of colposcopy in the diagnostics of cervical chlamydia-mycoplastic infections are less investigated. The aim of given work was the study of this problem. Colposcopic examination was performed in 952 women (age - from 16 to 67) asked to clinics for examination and more precise definition of diagnosis because of different complaints. It was revealed specific colposcopic features of chlamydia-mycoplastic genital infections, which were compared with the results obtained from "gold standard" methods - direct immunofluorescence method and bacteriological analysis of cervical smears. High sensitivity (Se) - 0,96, specificity (Sp) - 0,79 and diagnostic effectiveness (De) - 0,93 of colposcopy were established. Authors conclude that the application of colposcopy for diagnostics of chlamydia-mycoplastic genital infection is particularly important: during urgent cases; during the case, when it is impossible to carry out the deep diagnostics of the sexually transmitted infections; for optimization of the diagnostics of the sexually transmitted infections.
