Orthopedic surgeons face a significant challenge in treating critical-size femoral defects (CSFD) caused by osteoporosis (OP), trauma, infection, or bone tumor resections. In this study for the first time, the application of photobiomodulation (PBM) and bone marrow mesenchymal stem cell-conditioned medium (BM-MSC-CM) to improve the osteogenic characteristics of mineralized bone scaffold (MBS) in ovariectomy-induced osteoporotic (OVX) rats with a CSFD was tested. Five groups of OVX rats with CSFD were created: (1) Control (C); (2) MBS; (3) MBS + CM; (4) MBS + PBM; (5) MBS + CM + PBM. Computed tomography scans (CT scans), compression indentation tests, and histological and stereological analyses were carried out after euthanasia at 12 weeks following implantation surgery. The CT scan results showed that CSFD in the MBS + CM, MBS + PBM, and MBS + CM + PBM groups was significantly smaller compared to the control group (p = 0.01, p = 0.04, and p = 0.000, respectively). Moreover, the CSFD size was substantially smaller in the MBS + CM + PBM treatment group than in the MBS, MBS + CM, and MBS + PBM treatment groups (p = 0.004, p = 0.04, and p = 0.01, respectively). The MBS + PBM and MBS + CM + PBM treatments had significantly increased maximum force relative to the control group (p = 0.01 and p = 0.03, respectively). Bending stiffness significantly increased in MBS (p = 0.006), MBS + CM, MBS + PBM, and MBS + CM + PBM treatments (all p = 0.004) relative to the control group. All treatment groups had considerably higher new trabecular bone volume (NTBV) than the control group (all, p = 0.004). Combined therapies with MBS + PBM and MBS + CM + PBM substantially increased the NTBV relative to the MBS group (all, p = 0.004). The MBS + CM + PBM treatment had a markedly higher NTBV than the MBS + PBM (p = 0.006) and MBS + CM (p = 0.004) treatments. MBS + CM + PBM, MBS + PBM, and MBS + CM treatments significantly accelerated bone regeneration of CSFD in OVX rats. PBM + CM enhanced the osteogenesis of the MBS compared to other treatment groups.

In chest computed tomography (CT), the breasts located within the scan range receive a substantial radiation dose. Due to the risk of breast-related carcinogenesis, analyzing the breast dose for justification of CT examinations seems necessary. The main goal of this study is to overcome the limitations of conventional dosimetry methods, such as thermoluminescent dosimeters (TLDs) by introducing the adaptive neuro-fuzzy inference system (ANFIS) approach. In this study, the breast dose of 50 adult female patients who underwent chest CT examinations was measured directly by TLDs. Then, the ANFIS model was developed with four inputs including dose length product (DLP), volumetric CT dose index (CTDIvol), total mAs, and size-specific dose estimate (SSDE), and one output (TLD dose). Additionally, multiple linear regression (MLR) as a traditional prediction model was used for linear modeling and its results were compared with the ANFIS. The TLD reader results showed that the breast dose value was 12.37 ± 2.46 mGy. Performance indices of the ANFIS model, including root mean square error (RMSE) and correlation coefficient (R), were calculated at 0.172 and 0.93 for the testing dataset, respectively. Also, the ANFIS model had superior performance in predicting the breast dose than the MLR model (R = 0.805). This study demonstrates that the proposed ANFIS model is efficient for patient dose prediction in CT scans. Therefore, intelligence models such as ANFIS are suggested to estimate and optimize patient dose in CT examinations.

Background: Multiple sclerosis (MS) disease causes structural and functional damage to brain. Structural imaging of the MS-induced damage cannot adequately describe the functional impairment of the brain in MS patients. Therefore, it seems that advanced functional imaging analysis such as functional magnetic resonance imaging (fMRI) data is needed for better management of this disease. Objective: The aim of present study was to evaluate the effective connectivity within the Papez circuit in MS patients using resting-state fMRI. Material and Methods: In this cross-sectional analytical study, 22 healthy individuals and 24 patients with MS underwent resting-state fMRI. After pre-processing of the obtained data, the time series of Cingulate gyrus (CG), Para hippocampus gyrus (PHG), anterior thalamic nuclei (ATN), Mammillary body (MB), and Hippocampus (HPC) were extracted as the main Papez circuit components. The obtained time series were statistically analyzed as an input of the dynamic causal model in order to evaluate the effective connectivity in the Papez circuit. Results: The power of effective connectivity between each pair of tested nodes in Papez circuit was significantly lower in MS patients than healthy subjects. Also, the effective connectivity level of MS patients in direction of HPC→ATN was higher in men than women. In addition, effective self-connection in CG→CG and MB→MB regions in healthy subjects were higher in women than them in men. Conclusion: The present study reveals significant difference in effective connectivity of the Papez nodes in MS patients than control group, which can be exploited to diagnosis and predict or evaluate the treatment response of these patients.

This study was designed to evaluate the effect of the radiological technologists' training on optimising the eye lens dose in brain computed tomography (CT) examinations. The lens dose of 50 adult patients was measured using thermoluminescent dosimeters before and after technologists' training. Dose values of lenses, dose length product (DLP), volumetric CT dose index (CTDIvol) as well as image quality in terms of quantitative (contrast to noise ratio and signal to noise ratio) and subjective (artefact) parameters were compared before and after training. Lens dose values were 31.57 ± 9.84 mGy and 5.36 ± 1.53 mGy before and after training, respectively, which was reduced by ~83% (p < 0.05). The values of DLP, CTDIvol and image quality parameters were not significantly different (p > 0.05) and all images were diagnostically acceptable. Excluding the orbits from the scanning range is an efficient approach to optimize the lens dose; the training of the technologists has also a pivotal role in dose reducing.

The inertial cavitation activity depends on the sonication parameters. The purpose of this work is development of dual frequency inertial cavitation meter for therapeutic applications of ultrasound waves. In this study, the chemical effects of sonication parameters in dual frequency sonication (40 kHz and 1 MHz) were investigated in the progressive wave mode using iodide dosimetry. For this purpose, efficacy of different exposure parameters such as intensity, sonication duration, sonication mode, duty factor and net ultrasound energy on the inertial cavitation activity have been studied. To quantify cavitational effects, the KI dosimeter solution was sonicated and its absorbance at a wavelength of 350 nm was measured. The absorbance values in continuous sonication mode was significantly higher than the absorbance corresponding to the pulsed mode having duty factors of 20-80% (p<0.05). Among different combination modes (1 MHz 100%+40 kHz 100%, 1 MHz 100%+40 kHz 80%, 1 MHz 80%+40 kHz 100%, 1 MHz 80%+40 kHz 80%), the continuous mode for dual frequency sonication is more effective than other combinations (p<0.05). The absorbance for this combined dual frequency mode was about 1.8 times higher than that obtained from the algebraic summation of single frequency sonications. It is believed that the optimization of dual frequency sonication parameters at low-level intensity (<3 W/cm(2)) by optically assisted cavitation event sensor can be useful for ultrasonic treatments.

Acoustic cavitation plays an important role in sonochemical processes and the rate of sonochemical reaction is influenced by sonication parameters. There are several methods to evaluate cavitation activity such as chemical dosimetry. In this study, to comparison between iodide dosimetry and terephthalic acid dosimetry, efficacy of sonication parameters in reactive radical production has been considered by iodide and terephthalic acid dosimetries. For this purpose, efficacy of different exposure parameters on cavitations production by 1 MHz ultrasound has been studied. The absorbance of KI dosimeter was measured by spectrophotometer and the fluorescence of terephthalic acid dosimeter was measured using spectrofluorometer after sonication. The result of experiments related to sonication time and intensity showed that with increasing time of sonication or intensity, the absorbance is increased. It has been shown that the absorbance for continuous mode is remarkably higher than for pulsing mode (p-value < 0.05). Also results show that with increasing the duty cycles of pulsed field, the inertial cavitation activity is increased. With compensation of sonication time or intensity in different duty cycles, no significant absorbance difference were observed unless 20% duty cycle. A significant correlation between the absorbance and fluorescence intensities (count) at different intensity (R = 0.971), different sonication time (R = 0.999) and different duty cycle (R = 0.967) were observed (p-value < 0.05). It is concluded that the sonication parameters having important influences on reactive radical production. These results suggest that there is a correlation between iodide dosimetry and terephthalic acid dosimetry to examine the acoustic cavitation activity in ultrasound field.
