RESUMO

We report a case of a fatal retrograde Type A aortic dissection following thoracic endovascular aortic repair (TEVAR). The patient was diagnosed with vascular Ehlers-Danlos syndrome (vEDS) only postoperatively, which is a relative contraindication for TEVAR. The patient had no major or minor criteria for vEDS. This case report emphasizes pitfalls of TEVAR in patients with a connective tissue disorder.

Pathology and radiology are complimentary tools, and their joint application is often crucial in obtaining an accurate diagnosis in non-neoplastic pulmonary diseases. However, both come with significant limitations of their own: Computed Tomography (CT) can only visualize larger structures due to its inherent-relatively-poor resolution, while (histo) pathology is often limited due to small sample size and sampling error and only allows for a 2D investigation. An innovative approach of inflating whole lung specimens and subjecting these subsequently to CT and whole lung microCT allows for an accurate matching of CT-imaging and histopathology data of exactly the same areas. Systematic application of this approach allows for a more targeted assessment of localized disease extent and more specifically can be used to investigate early mechanisms of lung diseases on a morphological and molecular level. Therefore, this technique is suitable to selectively investigate changes in the large and small airways, as well as the pulmonary arteries, veins and capillaries in relation to the disease extent in the same lung specimen. In this perspective we provide an overview of the different strategies that are currently being used, as well as how this growing field could further evolve.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) has gained worldwide acceptance and implementation as an alternative therapeutic option in patients with severe aortic valve stenosis unable to safely undergo surgical aortic valve replacement. This transformative technique places the radiologist in a key position in the pre-procedural assessment of potential candidates for this technique, delivering key anatomical information necessary for patient eligibility and procedural safety. Recent trials also provide encouraging results to potentially extend the indication to patients with safer risk profiles. METHOD: The review is based on a PubMed literature search using the search terms "transcatheter heart valve", "TAVI", "TAVR", "CT", "imaging", "MR" over a period from 2010-2020, combined with personal comments based on the author's experience. RESULTS AND CONCLUSION: CT plays a prominent role in the pre-procedural workup, delivering as a true 3D imaging modality optimal visualization of the complex anatomy of the aortic root with simultaneous evaluation of the patency of the different access routes. As such, the contribution of CT is key for the determination of patient eligibility and procedural safety. This input is supplementary to the contributions of other imaging modalities and forms an important element in the discussions of the Heart Valve Team. Knowledge of the procedure and its characteristics is necessary in order to provide a comprehensive and complete report. While the role of CT in the pre-procedural evaluation is well established, the contribution of CT and MR and the clinical significance of their findings in the routine follow-up after the intervention are less clear and currently the subject of intense investigation. Important issues remain, including the occurrence and significance of subclinical leaflet thrombosis, prosthetic heart valve endocarditis, and long-term structural valve degeneration. INTRODUCTION: Low and moderate endurance exercise is associated with better control of cardiovascular risk factors, a decreased risk of coronary artery disease and atrial fibrillation (AF). There is, however, a growing proportion of individuals regularly performing strenuous and prolonged endurance exercise in which the health benefits have been challenged. Higher doses of endurance exercise have been associated with a greater coronary atherosclerotic plaque burden, risk of AF and myocardial fibrosis (MF). METHODS AND ANALYSIS: Master@Heart is a multicentre prospective cohort study aiming to assess the incidence of coronary atherosclerosis, AF and MF in lifelong endurance athletes compared to late-onset endurance athletes (initiation of regular endurance exercise after the age of 30 years) and healthy non-athletes.The primary endpoint is the incidence of mixed coronary plaques. Secondary endpoints include coronary calcium scores, coronary stenosis >50%, the prevalence of calcified and soft plaques and AF and MF presence. Tertiary endpoints include ventricular arrhythmias, left and right ventricular function at rest and during exercise, arterial stiffness and carotid artery intima media thickness.Two hundred male lifelong athletes, 200 late-onset athletes and 200 healthy non-athletes aged 45-70 will undergo comprehensive cardiovascular phenotyping using CT, coronary angiography, echocardiography, cardiac MRI, 12-lead ECG, exercise ECG and 24-hour Holter monitoring at baseline. Follow-up will include online tracking of sports activities, telephone calls to assess clinical events and a 7-day ECG recording after 1 year. ETHICS AND DISSEMINATION: Local ethics committees approved the Master@Heart study. The trial was launched on 18 October 2018, recruitment is complete and inclusions are ongoing. Myxomas are common benign cardiac tumors that can cause life-threatening events. Urgent surgical excision is needed. Therefore, little is known about their growth rate. We present a peculiar case of an incidental left atrial myxoma offering calculation of a growth curve over more than 6 years. The myxoma was incidentally seen retrospectively on computed tomography scans in follow-up for bladder carcinoma in a 71-year-old patient. The patient underwent surgical resection of the myxoma. The authors emphasize that myxoma can be unnoticed on non-electrocardiogram-gated computed tomography scans. In the presented case, cardiac myxoma appears to grow linearly over time.

RATIONALE: Bronchopulmonary dysplasia (BPD) is a common complication of preterm birth. Lung function and imaging are classically used to assess BPD. Functional respiratory imaging (FRI) combines a structural and functional assessment of the airways and their vasculature. We aimed to assess BPD using FRI and to correlate these findings with the clinical presentation. METHODS: We included 37 adolescents with a history of preterm birth (22 BPD cases and 15 preterm controls). The study protocol included a detailed history, lung function testing and computed tomography (CT) (at total lung capacity (TLC) and functional residual capacity (FRC)) with FRI. CT images were also assessed using the Aukland scoring system. RESULTS: BPD patients had lower forced expiratory volume in 1 s to forced vital capacity ratio (p=0.02) and impaired diffusion capacity (p=0.02). Aukland CT scores were not different between the two groups. FRI analysis showed higher lobar volumes in BPD patients at FRC (p<0.01), but not at TLC. Airway resistance was significantly higher in the BPD group, especially in the distal airways. Additionally, FRI showed more air trapping in BPD patients, in contrast to findings on conventional CT images. CONCLUSION: This study is the first to use FRI in research for BPD. FRI analysis showed higher lobar volumes in BPD patients, indicating air trapping and reduced inspiratory capacity. In contrast to Aukland CT scores, FRI showed more air trapping in the BPD group, suggesting that FRI might be a more sensitive detection method. Importantly, we also showed increased distal airway resistance in BPD patients. By combining structural and functional assessment, FRI may help to better understand the long-term sequelae of BPD.

BACKGROUND: The correlation of diffusion-weighted MRI and tumor aggressiveness has been established for different tumor types, which leads to the question if it could also apply for neuroendocrine tumors (NET). PURPOSE: To investigate the possible correlation between apparent diffusion coefficient (ADC) value on magnetic resonance imaging (MRI) and histopathologic WHO-grades of NET. MATERIAL AND METHODS: Electronic patient records from patients presented at the multidisciplinary neuro-endocrine tumor board between November 2017 and April 2019 were retrospectively reviewed. Patients with both available MR imaging (primary tumor or metastasis) and known WHO tumor grade were included (n = 47). Average and minimum ADC values (avgADC; minADC) were measured by drawing a freehand ROI excluding only the outermost border of the lesion. The largest axial size (primary tumor) or most clearly delineated lesion (metastasis) was used. RESULTS: Forty seven patients met the inclusion criteria (mean age 59 ± 12 SD; 24F/23M). Twenty one patients (45%) were diagnosed with WHO G1 tumor, 17 seventeen with G2 (36%) and nine with G3 (19%) tumor. Twenty eight primary tumors and 19 metastases were measured. A significant difference was found between low-grade (G1+G2) and high-grade (G3) tumors (Mann-Whitney; avgADC: p < 0,001; minADC: p = 0,001). There was a moderate negative correlation between WHO-grade and avgADC/minADC (Spearman; avgADC: -0,606; 95% CI [-0,773; -0,384]; minADC: -0,581; 95% CI [-0.759; -0.353]). CONCLUSION: Our data show a significant difference in both average and minimum ADC values on MRI between low and high grade NET. A moderate negative correlation was found between histopathologic WHO grade and ADC value.

"Lung cancer associated with cystic airspaces" is an uncommon manifestation, in which lung cancer presents on imaging studies with a cystic area with associated consolidation and/or ground glass. With the widespread use of computed tomography (CT), both in clinical practice and for lung cancer screening, these tumors are being more frequently recognized. An association of this entity with smoking has been established with the majority of cases reported being in former and current smokers. The true pathogenesis of the cystic airspace is not yet fully understood. Different causes of this cystic airspace have been described, including a check-valve mechanism obstructing the small airways, lepidic growth of adenocarcinoma on emphysematous lung parenchyma, cyst formation of tumor and tumor growth along the wall of a pre-existing bulla. Adenocarcinoma is the commonest histological type, followed by squamous cell carcinoma. Two classification systems have been described, based on morphological features of the lesion, taking into account both the cystic airspace as well as the morphology of the surrounding consolidation or ground glass. The cystic component may mislead radiologists to a benign etiology and the many different faces on imaging can make early diagnosis challenging. Special attention should be made to focal or diffuse wall thickening and consolidation or ground glass abutting or interspersed with cystic airspaces. Despite their atypical morphology, staging and management currently are still similar to that of other lung cancer types. Although the rarity of this entity will hamper larger studies, numerous aspects regarding this particular lung cancer type still need to be unraveled. This manuscript reviews the CT-imaging findings and gives an overview of available data in the English literature on pathogenesis, histopathology and clinical findings. Differential diagnosis and pitfalls are discussed as well as future directions regarding staging and management.

PURPOSE: To evaluate the presentation of inflammatory hepatocellular adenomas (HCAs) on hepatocyte phase MRI. MATERIALS AND METHODS: We retrospectively reviewed the MRI features of histologically proven HCAs on hepatocyte phase imaging. Twenty-one lesions (17 with inflammatory subtype) were scanned with gadobenate dimeglumine. Signal intensities of the lesions were assessed in the hepatocyte phase and on the T1-weighted sequences before contrast. RESULTS: After gadobenate dimeglumine injection, 71% (12/17) of the inflammatory HCAs showed areas of iso- or hyperintensity to the surrounding liver in the hepatocyte phase. In 82% (10/12) of the iso- or hyperintense lesions, this was found over more than 75% of the lesion surface. None of the noninflammatory HCAs showed areas of iso- or hyperintensity to the surrounding liver in the hepatocyte phase. From these 12, 7 were hyperintense on T1-weighting before contrast due to liver steatosis, 2 due to intrinsic hyperintensity (on the in-phase sequence), and 3 were isointense. CONCLUSION: In contrast to noninflammatory HCAs, inflammatory HCAs can show areas of iso- to hyperintensity to the surrounding liver in the hepatocyte phase; therefore, other typical imaging features should also be used to distinguish between HCAs and FNHs.
