RESUMO

PURPOSE: We evaluated contrast enhanced spiral computerized tomography (CT) as a single session for the anatomical and functional assessment of patients with chronic obstructive uropathy and normal serum creatinine. MATERIALS AND METHODS: The study included 65 patients with unilateral or bilateral chronic renal obstruction and normal serum creatinine. Five patients had bilateral obstruction and the remaining 60 had unilateral obstruction and a normal contralateral kidney. Therefore, the total number of renal units was 130, that is 70 obstructed and 60 normal. All patients underwent contrast enhanced spiral CT together with excretory urography (IVP) and Tc-mercaptoacetyltriglycine renal scan. CT was used to identify the cause of obstruction and selectively determine the glomerular filtration rate (GFR) of the 2 kidneys. The diagnostic accuracy of CT for identifying the cause of hydronephrosis was compared with that of IVP. Moreover, a correlation was made between CT GFR and isotope GFR. RESULTS: Obstruction was caused by ureteropelvic junction narrowing in 25 cases, ureteral stones in 21, ureteral stricture in 20 and extrinsic ureteral obstruction in 4. Contrast enhanced spiral CT identified the cause of hydronephrosis in all obstructed kidneys (100% sensitivity), while IVP identified the cause in 52 (74% sensitivity), which was a significant difference (p <0.05). CT and IVP excluded obstruction in all normal kidneys (100% specificity). A comparison between the isotope GFR of obstructed kidneys with the corresponding CT GFR showed a perfect correlation (r = 0.78, p <0.0001). Moreover, a similar comparison between isotope GFR and CT GFR of normal kidneys showed an excellent correlation (r = 0.73, p <0.0001). In obstructed and normal kidneys mean isotope clearance was not significantly different from that of mean CT clearance. CONCLUSIONS: Contrast enhanced spiral CT is more sensitive than IVP for identifying the cause of chronic obstructive uropathy. Moreover, it is as accurate as radioisotope renal scan for calculating the total and separate kidney function. We recommend spiral CT with contrast medium as a single radiological diagnostic modality for the assessment of patients with chronic renal obstruction and normal serum creatinine.

The prevalence of Kaposi's sarcoma (KS) after renal transplantation varies greatly depending on geographic and ethnic backgrounds. Out of 1400 live-donor kidney transplantation, 50 patients developed malignancy in our center. Kaposi's sarcoma was diagnosed in 24 patients (48%). Twenty patients were males and the main age of the patients was 29.8 +/- 11.1 years. They developed KS at a mean interval of 33.9 +/- 27.2 months, and the main duration of follow up was 62.2 +/- 50.7 months. Nineteen patients were cyclosporinee treated while four patients were azathioprine-based. Cutaneous KS was diagnosed in 20 patients, visceral in one, and three patients had both lesions. We found that the frequency of HLA-A1, -A2 and -DR5 were significantly common in KS patients. Titrated reduction of immunosuppression was initially tried in all patients. Bleomycin injection and superficial irradiation was tried in some cases. Response to therapeutic modalities was good in cutaneous form of KS. Twelve patients died (50%), two of them with functioning graft. We conclude that KS is the most common malignancy after kidney transplantation in our locality. Patients on cyclosporine are more prone to develop KS, and certain HLA-antigens may predispose to this. Early diagnosis and interference carry a favorable outcome.

UNLABELLED: We evaluated the incidence and management of vascular complications after live donor renal transplantation. Possible risk factors and their effects on patient and graft survival were also assessed. MATERIALS AND METHODS: A total of 1,200 consecutive live donor renal transplants were performed in 1,152 patients at a single institution. The incidence of different types of vascular complications were determined and correlated with relevant risk factors. The impact on patient and graft survival was also studied. RESULTS: There were 34 vascular complications (2.8%). Stenotic or thrombotic complications were recorded in 11 cases (0.9%), including renal artery stenosis in 5 (0.4%), renal artery thrombosis in 5 (0.4%) and renal vein thrombosis in 1 (0.1%). Hemorrhagic complications were observed in 23 patients (1.9%). Although no risk factors could be identified that were related to stenotic or thrombotic complications, grafts with multiple renal arteries were significantly associated with hemorrhagic complications (p = 0.04). Stenotic and thrombotic complications as well as hemorrhagic complications were significantly associated with subsequent biopsy proved acute tubular necrosis (p <0.001). The mean 5-year patient and graft survival rates +/- SD for those with vascular complications were 71.9% +/- 1.9% and 41.6% +/- 8.9% compared with 86.3% +/- 1.1% and 76.8% +/- 1.4% for the remainder of our transplant population, respectively (p <0.001). The deleterious impact on survival was not only observed in recipients with thrombotic or stenotic crises, but also in those with hemorrhagic sequelae. CONCLUSIONS: Hemorrhagic crises are as serious as the stenotic and thrombotic complications affecting patient and graft survival. Because they are a significant factor in the development of hemorrhagic complications, grafts with multiple renal arteries should be managed critically.
