RESUMO

The authors describe the case of a 60-year-old woman patient with a crural ulcer with a traumatic, arterial, venous and lymphatic aetiology. After three-year unsuccessful treatment by conventional methods and surgery a combination of hyperbaric oxygen therapy and plastic surgery was used. This rational therapeutic approach was very effective and the authors recommend its introduction into routine practice.

RESUMO

In eight healthy volunteers--four professional and four amateur divers--in a hyperbaric chamber simulated diving was implemented, corresponding to the most frequently encountered conditions of diving activities in Czechoslovakia. Throughout the trial indicators of haemostasis were investigated. Under the selected model conditions changes of the investigated parameters developed which can be considered indicators of a latent process of disseminated coagulation and the so-called syndrome of silent bubbles.

RESUMO

The authors present eleven year's experience with comprehensive treatment of patients who suffered an ischemic cerebrovascular attack. This comprises so-called conventional treatment of cerebrovascular attacks and also hyperbaroxia, i.e. administration of oxygen under high partial pressure. The authors used a therapeutic pressure of 0.2 MPa for 90 minutes in individual exposures. Of 31 treated patients 23 improved, i.e. 76%; eight improved slightly, eight significantly and seven substantially. The condition of the remainder did not change. Hyperbaroxia should therefore be used everywhere where this method is available.

RESUMO

The authors report their three-year experience with application of thrombolytic treatment in 31 patients with vascular occlusions of the lower extremities. They were successful in two thirds of the patients. The authors analyze the advantages and shortcomings of systemic or selective administration of antithrombotic therapy. Concurrent administration of hyperbaric oxygen is a rational therapy.

RESUMO

Rational antithrombotic treatment in vascular occlusions is a marked advance in the therapeutic results, incl. reduced mortality. The prerequisite of success is early adequate thrombolytic or anticoagulant treatment taking into account absolute and relative contraindications. Despite this it is necessary to foresee the risk of haemorrhagic complications and orientate diagnosis with the aspect in mind. As an example three case-histories are presented, their common sign being haemorrhage into the lower extremities during antithrombotic treatment. Attention is drawn to the difficulty of differential diagnosis of venous thrombosis and haemorrhage into muscles or intermuscular spaces where at a given site computer tomography is a great asset. In the treatment in addition to surgical intervention in particular hyperbaric oxygen therapy proves useful.

RESUMO

The authors submit their initial experience with fibrinolytic treatment of acute arterial occlusions. In a group of 14 patients streptokinase and urokinase was administered 12 times as general treatment and twice locally. Treatment was successful in seven patients, no change in the local finding was recorded in two patients, in another two patients despite treatment progression of the ischaemia occurred. Two patients died in causal relation with treatment, one patient died for other reasons. Based on their own experience and that of authors abroad the authors emphasize the necessity of strict indication for fibrinolytic treatment, they prefer local to general administration, with regard to the number of complications. In the authors' opinion fibrinolysis holds a justified position in the treatment of acute arterial occlusions as a supplementary method of surgical treatment.
