RESUMO

Rapid diagnosis of obligate anaerobes was conducted by means of gas liquid chromatography, bacterioscopy, and monitoring. It was found that in 80% of patients the results of rapid diagnosis coincided with those of complete cultural study. Monitoring of obligate anaerobes yields generalized information on their species composition, drug sensitivity, and etiological importance in mixed anaerobo-aerobic infections. The data of rapid diagnosis makes it possible to use directed antibiotic therapy before bacteriological examination is completed.

RESUMO

The composition of the causative agents of suppurations, isolated from suppurating wounds of patients hospitalized at different departments of the Sklifosovskii Emergency Aid Institute was studied. The proportion of representatives of different microbial families among more than 18,000 strains of the causative agents of wound infection was estimated. Fluctuations in the amount and structure of the agents were analyzed in different clinics over the period of 1967-1987. Under the conditions of emergency aid hospital Staphylococcus aureus proved to be the most frequent causative agent of suppurative inflammatory processes at different departments. At the same time no tendency towards an increase of the etiological role of any microbial family in the development of wound infections was noted. The incidence of different causative agents of suppurations was found to vary at different departments, depending on their specialization and season. The results of observations on the composition of the causative agents of wound infection may be used for the planning and evaluation of antiepidemic measures.

Consideration of an inflammation focus as an "open system" provided analogy between microbiological processes in inflamed wounds and in systems of continuous cultivation of microorganisms. Mathematical modeling of such systems is widely used. Some of the methods for the mathematical modeling were applied to chemoprophylaxis and chemotherapy of postoperative wounds. In modeling continuous cultivation of microorganisms it is usually necessary to determine optimal conditions for the maximum yield of their biomass. In modeling of wound treatment the aim was to determine the process parameters providing the minimum biomass. The described simple models showed that there could be certain optimal flow rate of the washing fluid in the aspiration-washing procedure for wound treatment at which the drug was not completely washed out while the growth rate of the microbial population was minimal. Such mathematical models were shown valuable in optimizing the use of bactericidal and bacteriostatic antibiotics.

A direct express-method is developed for estimation of hydrocortisone, based on the use of individual human blood plasma endogenous transcortine as a binding system. The method is designed for non-serial estimation of hydrocortisone in human blood plasma and may be used for an express-analysis, since extraction of the hormone from blood plasma by organic solvents is omitted. The dissociation constant of the transcortine-hydrocortisone complex was calculated using Scatchard plot. On the basis of this procedure and considering the rule of acting masses the initial concentration of receptor sites was measured for transcortine at a given blood plasma dilution and theoretical curves were constructed for the reaction of competitive substitution of labelled hydrocortisone by the unlabelled hormone. Estimation of hydrocortisone, carried out by means of these theoretical curves of substitution, closely correlated with the data of experimental standard curves of substitution (CEA-IRE-Sorin). The express-method of hydrocortisone estimation was used in clinical research.
