RESUMO

The paper shows that intravenous administration of trental, curantil, cavinton and sermion to cerebrovascular deficient patients causes a decline in blood circulation rates in the extracranial part of the carotid arteries. Sermion exhibited a biphase reaction: an increase in the blood flow followed by a decrease 15-30 minutes after introduction in the carotid arteries. Trental and curantial act in the intracranial arteries as moderate vasodilators reducing blood flow speed and blood vessel resistance. Cavinton activates intracranial blood flow, especially in the cortical segments of the middle cerebral arteries. Sermion initially increases blood flow rate in the middle cerebral arteries, than decreases it, especially in segments M2 and M3. Cavinton and sermion intravenous infusions are beneficial in cases of arterial hypertension.

Piracetam was applied to the treatment of 60 patients with the initial manifestations of brain blood supply failure and stage I encephalopathy associated with circulatory disorders. Echopulsography of the intracranial vessels and dopplerography of the extracranial vessels of the brain were employed, changes in lipid metabolism, rheological and coagulation properties of the blood were defined. Continuous use of piracetam per os was found to produce a number of beneficial therapeutic effects. Some patients suffering from encephalopathy associated with circulatory disorders showed headache enhancement after intake of 1.6 g of the drug, accompanied by a considerable increase of the amplitude of pulse fluctuations, prompting the reduction of the drug dose. Piracetam decreased the high vascular resistance and raised the lowered volume of pulse fluctuations. In all the patients examined, aggregation of formed elements of the blood returned to normal whereas the content of atherogenic lipids in blood serum significantly dropped.

Altogether 94 patients with initial manifestations of brain blood supply failure (IMBBSF) consequent on vegetative vascular dystonia and atherosclerosis of the cerebral vessels were examined. Clinical, echopulsographic and dopplerographic examinations demonstrated that 54 patients had cerebral venous discirculation. The main diagnostic criteria for cerebral venous discirculation in patients with IMBBSF lie in an increase of the configuration of the diastolic part of the echoPG of the cerebral arteries, possibility of recording echoPG of the cerebral veins, a rise of the linear velocity of the blood flow in the jugular veins under head-down test during dopplerography.

Piracetam was applied to the treatment of 60 patients with initial manifestations of brain circulation failure and stage I circulatory encephalopathy. The drug exerted a beneficial therapeutic effect by reducing high brain vascular resistance (both extra- and intracranial) and by increasing the lowered volume of pulse fluctuations. It made fibrinolytic blood activity and aggregation of formed elements of the blood return to normal. An appreciable antiatherogenic effect was discovered as well. It should be taken into consideration that in patients with a dramatic lowering of pulse fluctuations, the use of piracetam in a dose of 1.6 g/day may enhance headache. In such cases the dose should be reduced.

Phonoangiography (PAG) was applied to study the extracranial parts of cerebral blood vessels in 16 symptom-free healthy individuals and 92 patients with cerebrovascular disorders. The conductance of cardiac tones to the cerebral arteries was assessed in comparison between PAG and phonocardiography. Correlations were found between the arterial wall elasticity, luminal changes and heart sounds conduction. As the atherosclerotic changes increased in arteries under study, the tones conductance velocity increased and their amplitude decreased. With the vascular tone reduced, the heart sounds conduction velocity decreased and their amplitude increased. The 1st and 2nd tones displayed differential increase in conduction velocity in distorted and dilated arteries. PAG vascular bruits were characteristic hemodynamic disorders.

The rheological properties of the blood and aggregation of erythrocytes and platelets were studied in 30 apparently healthy subjects and in 220 patients with impaired blood supply to the brain. The results have confirmed a high informative value of the complex of parameters of rotational viscosimetry: the limit of blood fluidity, apparent blood viscosity, caisson viscosity of the blood, and the coefficient of erythrocyte cohesion (A) and of the parameters of aggregation of the formed elements of the blood, this complex allowing an objective differentiation between microcirculatory peculiarities in patients with initial manifestations of cerebral blood supply insufficiency (IMCBSI) versus patients with ischemic stroke (IS). It has been elucidated for the first time that patients with IMCBSI secondary to atherosclerosis have a low A parameter confirming optimization of the microcirculation following elevation of the hematocrit. The alterations in the rheological properties of the blood were the greatest in patients with IS within the first three days after the onset of the disease. Elevated cohesion of erythrocytes persisted for one month after the onset of IS. It has been proved that repeated examinations are necessary for the individual and adequate correction of the blood rheology.

RESUMO

The action of trental was studied in 125 patients with initial manifestations of cerebral circulation insufficiency (CCI) secondary to vasomotor dystonia or atherosclerosis. Trental was used in the form of tablets (100 mg 3 times daily) for 60 days. To assess the therapeutic effect of trental, the authors analyzed the time-course of subjective manifestations, parameters of echopulsography of cerebral vessels and biomicroscopy of the conjunctiva, and the data of the total blood count and the rheological properties of the blood. In patients with initial manifestations of CCI trental decreased the cerebral vascular resistance and improved the venous outflow, reduced platelet adhesive and aggregation properties and diminished blood viscosity. It also elevated the spontaneous fibrinolytic activity of the blood. The advisability of selective administration of the drug for treating initial manifestations of CCI was confirmed.

RESUMO

Results of determining the chemical composition and the electrophoretic spectrum of chlorine-soluble proteins and lipoproteins in the cerebrospinal fluid of patients with cerebral infarctions and brain tumours are presented. The determinations showed non-identity of those proteins in the patients with the above diseases. The information obtained can be of value for solving a number of questions concerning the differential diagnosis, determination of the tumour character, as well as some other questions.

RESUMO

A method of recording echopulsograms of the basilar artery has been developed and tested in experimental and clinical conditions. The examinations were carried out in 40 healthy individuals and 18 patients with basilar artery patholody. Standards of the echopulsograms were elaborated, and the latters' changes in patients with atherosclerotic stenosis, thrombosis, and basilar artery aneurism were determined.

The study deals with the influence of Droperidol introduced intravenously in doses 2.5 mg on the REG indices of cerebral hemodynamics in patients with disorders of cerebral circulation. The achieved data indicate its mild hypotensive effect, an increase of the module of elasticity of the walls in large brain vessels along with a distinct decrease in the cerebral vascular resistance, as well as an improvement of the venous outflow of blood due to the influence of droperidol. The effect of this preparation appeared to be more expressed in patients with hemmorrhages into the brain and its membranes. A drop in the amplitude of brain fluctuations along with a drop of the arterial pressure made it possible to adequately correct the prescription of droperidol to patients with disturbed brain circulation.
