El trabajo de parto lleva con este la experiencia que causa uno de los dolores más intensos que se han descrito. En el Instituto de Seguridad Social de El Salvador (ISSS) se registran aproximadamente 600 a 700 partos vaginales por mes y de estos aproximadamente 10-20% solicitan analgesia para trabajo de parto, la cual el servicio de anestesia provee a través del abordaje epidural. Es conocido que se considera que el medicamento ideal para la analgesia de trabajo de parto es la bupivacaína o lidocaína 3, sin embargo, es de especial importancia describir el puntaje de APGAR de los recién nacidos de madres que recibieron cualquiera de estos dos fármacos por la vía ya mencionada. Objetivo: Determinar cómo influye el uso de lidocaína y bupivacaína como anestésico epidural sobre el puntaje APGAR posterior al nacimiento en mujeres embarazadas en fase activa del trabajo de parto con necesidad de analgesia epidural. Métodos: El presente es un estudio observacional descriptivo retrospectivo de fuentes primarias. El servicio de anestesiología del Hospital Materno Infantil Primero de Mayo cuenta con un registro sobre cuántos y a cuáles pacientes se les administra analgesia epidural por lo que el equipo investigador realizó una revisión de la técnica anestésica y se describe el Apgar al final del parto por vía vaginal en aquellas pacientes que recibieron analgesia epidural con lidocaína o bupivacaína. Se realizó el filtrado con la base de datos de Excel utilizando criterios de inclusión y exclusión, logrando incluir finalmente 81 pacientes en el análisis de los datos. Se encontró durante el escrutinio de estos que a 25 de estas se les realizó cesárea de emergencia y en 6 de estos mismos expedientes no se consignó de manera completa todos los datos necesarios para el análisis de ello, por lo que se utilizaron finalmente 51 expedientes clínicos para la realización de la presente investigación. Resultados: El APGAR promedio de los pacientes que recibieron analgesia de trabajo de parto con bupivacaína es de 8.75 y 8.9 al minuto y cinco minutos respectivamente y el promedio de APGAR alcanzado por los recién nacidos de pacientes que recibieron analgesia de trabajo de parto con lidocaína es de 9 puntos al minuto y cinco minutos. Conclusión: El promedio de APGAR en pacientes que recibieron bupivacaína es de 8.75 y el de lidocaína es de 9. (provisto por Infomedic International)


Labor brings with it the experience that causes one of the most intense pains ever described. At the Social Security Institute of El Salvador (ISSS) approximately 600 to 700 vaginal deliveries are registered per month and of these approximately 10-20% request labor analgesia, which the anesthesia service provides through the epidural approach. It is known that the ideal drug for labor analgesia is considered to be bupivacaine or lidocaine 3, however, it is of special importance to describe the APGAR score of newborns born to mothers who received either of these two drugs by the aforementioned route. Objective: To determine how the use of lidocaine and bupivacaine as epidural anesthetic influences the post-birth APGAR score in pregnant women in the active phase of labor requiring epidural analgesia. Methods: The present is a retrospective descriptive observational observational study from primary sources. The anesthesiology service of the Hospital Materno Infantil Primero de Mayo has a record of how many and to which patients epidural analgesia is administered, so the research team conducted a review of the anesthetic technique and described the Apgar at the end of vaginal delivery in those patients who received epidural analgesia with lidocaine or bupivacaine. Filtering was performed with the Excel database using inclusion and exclusion criteria, finally including 81 patients in the data analysis. It was found during the scrutiny of these that 25 of them had undergone emergency cesarean section and in 6 of these same records not all the data necessary for the analysis were completely recorded, so that 51 clinical records were finally used for the present investigation. Results: The average APGAR of the patients who received labor analgesia with bupivacaine was 8.75 and 8.9 at one minute and five minutes respectively, and the average APGAR achieved by the newborns of patients who received labor analgesia with lidocaine was 9 points at one minute and five minutes. CONTEXT: The rate of lower extremity musculoskeletal injury (LE MSK) is elevated after concussion; however, the underlying mechanism has not been elucidated. Physical characteristics have been investigated despite poorer mental health being a common postconcussion complaint and linked to MSKs. OBJECTIVE: To evaluate the role of mental health as a predictor of postconcussion LE MSK. DESIGN: Case-control study. SETTING: Intercollegiate athletic training facility. PATIENTS OR OTHER PARTICIPANTS: A total of 67 National Collegiate Athletic Association Division I student-athletes (n = 39 females) who had been diagnosed with a sport-related concussion. MAIN OUTCOME MEASURE(S): The Brief Symptom Inventory-18, Hospital Anxiety and Depression Scale, and Satisfaction With Life Scale (SWLS) measures were completed at baseline (preseason) and on the day participants were cleared for unrestricted return to play (RTP) after a concussion. Two binary logistic regressions were used to predict postconcussion LE MSK within a year, one for the baseline time point and the second for the RTP time point. A 2 (group: LE MSK, no LE MSK)-by-2 (time: baseline, RTP) repeated-measures analysis of variance compared performance between baseline and RTP. RESULTS: Subsequent LE MSKs were sustained by 44 participants (65.7%). The only significant predictor of postconcussion LE MSK was the SWLS score at RTP, with Exp(B) = 0.64, indicating that an increased (improved) SWLS score was associated with a lower LE MSK rate. No significant interactions were present between mental health measures and subsequent MSK (P values = .105-.885). CONCLUSIONS: Limited associations were evident between postconcussion LE MSK and scores on commonly used measures of anxiety, depression, and satisfaction with life. Reported increased satisfaction with life was associated with a decreased injury risk, which warrants further attention. Our results suggest that these measures of anxiety, depression, and satisfaction with life have limited value in assisting sports medicine clinicians with determining which student-athletes are at elevated risk of postconcussion LE MSK.

BACKGROUND: Symptom resolution is a key marker in determining fitness for return to activity following concussion, but in some cases, distinguishing persistent symptoms due to concussion versus symptoms related to other factors can be challenging. OBJECTIVE: To determine base rates of postconcussional syndrome (PCS) diagnostic categorization in healthy cadets and student athletes with no recent concussion. METHODS: 13,009 cadets and 21,006 student athletes completed baseline preseason testing. After inclusion/exclusion criteria were applied, the final sample included 12,039 cadets [9123 men (75.8%); 2916 women (24.2%)] and 18,548 student athletes [10,192 men (54.9%); 8356 women (45.1%)]. Participants completed the Sport Concussion Assessment Tool-3rd Edition (SCAT3) symptom evaluation as part of baseline preseason testing. The PCS diagnostic categorization was classified by the International Classification of Diseases, 10th Revision (ICD-10) symptom criteria for PCS. RESULTS: In the absence of recent concussion, subgroups of cadets (17.8% of men; 27.6% of women) and student athletes (11.4% of men; 20.0% of women) reported a cluster of symptoms that would meet the ICD-10 symptom criteria for PCS. Participants with insufficient sleep and/or preexisting conditions (e.g., mental health problems), freshmen cadets, and cadets at the U.S. Coast Guard Academy and at the U.S. Air Force Academy (freshmen were tested during basic cadet training) were more likely to report a cluster of symptoms that would meet the ICD-10 symptom criteria for PCS. CONCLUSION: The ICD-10 symptom criteria for PCS can be mimicked by preexisting conditions, insufficient sleep, and/or stress. Findings support person-specific assessment and management of symptoms following concussion.

Abstract Objective Identify risk factors that determine pneumonia development in patients who have undergone cardiac surgery. Methods Prospective study of a single cohort in a postoperative intensive care unit at a tertiary care center, encompassing all patients undergoing cardiac surgery from January to July 2014. Results 31 postoperative pneumonia cases were enrolled out of 211 patients (14.6%). The following independent risk factors were identified: hypertension (OR: 3.94, p = 0.01), chronic renal failure (OR: 13.67, p = 0.02), reintubation (OR: 22.29, p = 0.001) and extubation after 6 h (OR: 15.81, p = 0.005). Conclusions Main determinants for pneumonia after surgery were hypertension, chronic renal failure, extubation after 6 h and reintubation.


Resumen Objetivo Identificar los factores de riesgo para desarrollar neumonía en el periodo postquirúrgico de pacientes sometidos a cirugía cardiaca. Método Estudio de cohorte prospectiva que incluye pacientes > 18 años sometidos a cirugía cardiaca de enero a julio de 2014, en el Instituto Nacional de Cardiología Ignacio Chávez, México D.F. Se considera un valor de p < 0.05 como significativo. Resultados De un total de 211 pacientes operados, se diagnosticaron 31 casos de neumonía postquirúrgica (14.6%). Se identificaron los siguientes factores de riesgo independientes para desarrollar neumonía: la hipertensión arterial sistémica (OR: 3.94, p = 0.01), el antecedente de insuficiencia renal crónica (OR: 13.67, p = 0.02). La reintubación (OR: 22.29, p = 0.001) y permanecer intubado por más de 6 horas (OR: 15.81, p = 0.005). Conclusiones Los principales factores de riesgo independientes para desarrollar neumonía en el periodo postquirúrgico son la preexistencia de hipertensión arterial sistémica o insuficiencia renal crónica, la reintubación y permanecer intubado durante más de 6 h.

Existe un problema en el mundo por la población que vive por encima de los 3000 m. que tienen cambios adaptativos que ponen a prueba el organismo humano, la Baja presión barométrica estimula respuestas fisiológicas y fisiopatológicas. Además de cambios en la dinámica de gases. La Anestesia General Balanceada usada en Laparoscopia en el Hospital II Pasco, fue evaluada para determinar sus Características. Conclusiones: Los pacientes Sometidos a Anestesia General Balanceada en el Hospital II Pasco, tienen características antropométricas de talla corta con un IMC de 27.47, con una Saturación de Oxigeno baja comparada con pacientes de bajas cotas. El manejo intraoperatorio tiene como observación un aumento del porcentaje analizado de Sevoflurano, recuperando la baja Sat de O2 con suplemento de oxígeno, mantienen una ETCO2 baja. El perfil hemodinámico es normal. El periodo Post operatorio; tiene las mismas características que los de bajas cotas, una vez recuperado el paciente vuelve a sus valores preoperatorios de Sat de O2 sin diferencia significativa, lo mismo sucede con los parámetros hemodinámicos.


There is a problem in the world by population living above 3000 m. with adaptive changes that test the human body, the Lower barometric pressure stimulates physiological and pathophysiological responses. In addition to changes in the gas dynamics. The Balanced General Anesthesia used in Laparoscopy Hospital II Pasco, was evaluated to determine its characteristics. Conclusions: Patients Undergoing General Anesthesia in the Hospital II Balanced Pasco have anthropometric characteristics of short stature with a BMI of 27.47, with a low oxygen saturation compared with patients in low levels. The intraoperative management is to observation increased percentage of Sevoflurane analyzed, recovering lowest O2 Sat with supplemental oxygen, maintained a low ETCO2. The hemodynamic profile is normal. Post-operative period; has the same characteristics as those of lower altitudes, the patient once again regained their preoperative O2 Sat values without significant difference, so does hemodynamic.
