RESUMO

Programmed irrigation and drainage were used in the process of treatment of 17 patients with purulent arthritis of the knee joint and talocrural articulation and of 13 patients with intramedullary phlegmons of the femoral, tibial and humeral bones. The programmed irrigation and drainage were proved to decrease the microbial dissemination of the wash liquid to 10 colony-forming units per 1 ml during 1-3 days. The method allowed to avoid synovectomy and resection of the joint in patients with purulent gonitis, to decrease the risk of appearance of secondary infections. The programmed irrigation and drainage for intramedullary phlegmons provide complete sanitation of the purulent focus during 7-10 days, create favorable conditions for consolidation of fractures and prevent the development of chronic osteomyelitis.

RESUMO

Treatment of 92 patients aged from 18 to 69 years with abscesses and phlegmons of soft tissues included programmed irrigation and drainage. Apparatuses with two-level programs were used. It was shown that programmed irrigation and drainage allowed microbial dissemination by wash liquid to be decreased to 10 colony-forming units per 1 ml during 3 days. This term can be shortened to 1 day when using the program of intensive irrigation and drainage (more than 200 cycles during the first days after operation) and when intraoperative treatment with a beating jet of liquid is used in combination with curettage and vacuuming.

RESUMO

The article describes the reasons and the frequency of purulent complications in gunshot wounds in Afghanistan. The authors stress the role of primary surgical management of wounds in the prophylaxis of wound infection, show the trends towards the improvement of the first aid and primary medical care. The article studies the efficiency of physical methods in the management of wounds, such as treatment by the pulsed stream of liquid, active drainage, treatment in regulated air environment. The results of studies make it possible to recommend these methods for complex treatment at the phase of the secondary medical care.

RESUMO

Purulent wounds in 88 victims of the Armenian earthquake with the syndrome of long-term squeezing were examined microbiologically and the experience with using antimicrobial drugs for their treatment was analyzed. In all the cases microbial associations were detected, coliform and nonfermenting gram-negative bacilli, enterococci, anaerobes and yeast-like fungi being isolated from 80, 20, 15 and 25 per cent of the patients respectively. 130 bacterial strains were studied in detail. 80 to 100 per cent of the bacterial isolates were resistant to the antibiotics routinely used in surgical practice, i.e. benzylpenicillin, tetracycline, levomycetin, kanamycin, cephalothin and cefazolin and highly sensitive to new antimicrobial drugs belonging to ureidopenicillins (mezlocillin), quinolones (ciprofloxacin) and cephalosporins of the 3rd generation (cefoperazone and ceftazidime). Retrospective estimation of the antibiotic therapy revealed its inadequacy in 55.7 per cent of the patients for the most part because of the isolates resistance. The 19 variants of the antibiotic use prescribed by the physicians mainly included penicillins, aminoglycosides and their combinations (in 67.2 per cent of the cases). The use of the highly efficient drugs of the groups of ureidopenicillins and quinolones was extremely rare which was likely due to lacking of information on the drugs.

RESUMO

The work presents data on dynamics of microbial dissemination of purulent wounds in the process of surgical treatment with an additional action of the beating jet of liquid and repeated treatment by the beating jet of antibiotics. An analysis of results of healing the wounds has shown efficiency of the method when finishing the surgical treatment of purulent wounds by putting sutures as well as by plastic closure of the wounds.

RESUMO

Experimental investigations of the authors have shown active drainage to be optimal at negative pressure in the wound not higher than 80 mm Hg. The application of active drainage in the treatment of 1369 patients has demonstrated the effectiveness of the method for prevention of local complications of the wound process.

RESUMO

On the basis of studying the concentration and volume indices of the erythron, iron plasmic pool, proliferation activity of the erythron and iron kinetics in 137 patients operated for pulmonary diseases the authors note a depression of the bone marrow red stem as a characteristic feature of postoperative anemia. A scheme of the main steps of postoperative anemia is presented.
