Introduction: Chronic Kidney Disease (CKD) is a persistent condition affecting renal function and structure. The prevalence of CKD varies worldwide, with a higher incidence among individuals with diabetes and hypertension. Moreover, the mortality rate due to CKD has decreased in recent years in Chile. General objective: To describe CKD-related deaths in Chile according to sex, age group, and region between 2021 and 2022. Materials and methods: A descriptive cross-sectional observational study was conducted using data obtained from the Department of Health Statistics and Information (DEIS). Descriptive statistics were used and the mortality rate was calculated. Microsoft Excel was used for the statistical analysis. This study did not require ethical committee review, and there are no conflicts of interest. Results: A total of 2478 deaths due to CKD were studied during 2021-2022, with a 15.85% increase in the overall number of deaths. The general mortality rate was 12.54 per 100,000 inhabitants. Both sexes had similar mortality rates, and the 81-90 age group had the highest number of deaths. The Metropolitan Region registered the majority of the deaths. Discussion: The mortality rate in Chile is comparable to developed countries, with an increasing trend in both sexes. The higher number of deaths among individuals over 61 years old and the variation in mortality rates by region may be attributed to cardiovascular risk factors. Addressing CKD requires a focus on prevention and early diagnosis strategies.

Colorectal cancer (CRC) is a prevalent and significant health problem worldwide, especially in developed countries. The aim of this study was to evaluate the CRC mortality rate in Chile in the last 25 years. An ecological descriptive study was carried out using public mortality data from 1997 to 2022, which included a total of 50,944 deaths. The results revealed that colon cancer represented the majority of cases (72.15%) where the sigmoid colon was the most affected site. The average mortality rate was 11.19 per 100,000 individuals, showing a significant increase over time. The analysis by age showed a greater burden of mortality in individuals from 75 to 90 years. Gender disparities were observed, with a predominance of female deaths until 2019. The analysis of the rate configured by region revealed non-significant differences in mortality rates, with Valparaíso and Magallanes and Antártica Chilena being the regions with the highest rates for colon and rectal cancer, respectively. These findings contribute to our understanding of the epidemiology of CRC in Chile and emphasize the need for specific interventions in primary prevention and screening to reduce mortality from this disease.

RESUMO

Sex change occurs as a usual part of the life cycle for many teleost fish and the modifications involved (behavioural, gonadal, morphological) are well studied. However, the mechanism that transduces environmental cues into the molecular cascade that underlies this transformation remains unknown. Cortisol, the main stress hormone in fish, is hypothesised to be a key factor linking environmental stimuli with sex change by initiating gene expression changes that shift steroidogenesis from oestrogens to androgens but this notion remains to be rigorously tested. Therefore, this study aimed to experimentally test the role of cortisol as an initiator of sex change in a protogynous (female-to-male) hermaphrodite, the New Zealand spotty wrasse (Notolabrus celidotus). We also sought to identify potential key regulatory factors within the head kidney that may contribute to the initiation and progression of gonadal sex change. Cortisol pellets were implanted into female spotty wrasses under inhibitory conditions (presence of a male), and outside of the optimal season for natural sex change. Histological analysis of the gonads and sex hormone analyses found no evidence of sex change after 71 days of cortisol treatment. However, expression analyses of sex and stress-associated genes in gonad and head kidney suggested that cortisol administration did have a physiological effect. In the gonad, this included upregulation of amh, a potent masculinising factor, and nr3c1, a glucocorticoid receptor. In the head kidney, hsd11b2, which converts cortisol to inactive cortisone to maintain cortisol balance, was upregulated. Overall, our results suggest cortisol administration outside of the optimal sex change window is unable to initiate gonadal restructuring. However, our expression data imply key sex and stress genes are sensitive to cortisol. This includes genes expressed in both gonad and head kidney that have been previously implicated in early sex change in several sex-changing species.

RESUMO

The stunning sexual transformation commonly triggered by age, size or social context in some fishes is one of the best examples of phenotypic plasticity thus far described. To date our understanding of this process is dominated by studies on a handful of subtropical and tropical teleosts, often in wild settings. Here we have established the protogynous New Zealand spotty wrasse, Notolabrus celidotus, as a temperate model for the experimental investigation of sex change. Captive fish were induced to change sex using aromatase inhibition or manipulation of social groups. Complete female-to-male transition occurred over 60 days in both cases and time-series sampling was used to quantify changes in hormone production, gene expression and gonadal cellular anatomy. Early-stage decreases in plasma 17ß-estradiol (E2) concentrations or gonadal aromatase (cyp19a1a) expression were not detected in spotty wrasse, despite these being commonly associated with the onset of sex change in subtropical and tropical protogynous (female-to-male) hermaphrodites. In contrast, expression of the masculinising factor amh (anti-Müllerian hormone) increased during early sex change, implying a potential role as a proximate trigger for masculinisation. Collectively, these data provide a foundation for the spotty wrasse as a temperate teleost model to study sex change and cell fate in vertebrates.

RESUMO

BACKGROUND: 1,3-beta-D-glucan (BG) is a ubiquitous cell wall component of gut micro-organisms. We hypothesized that the serum levels of BG could reflect active intestinal inflammation in patients with inflammatory bowel disease. AIM: To determine whether the serum BG concentrations correlate with intestinal inflammation. METHODS: A prospective observational study was performed in a tertiary referral center, from 2016 to 2019, in which serum BG was determined in 115 patients with Crohn's disease (CD), 51 with ulcerative colitis (UC), and 82 controls using a photometric detection kit. Inflammatory activity was determined by ileocolonoscopy, histopathology, magnetic resonance enterography, and biomarkers, including fecal calprotectin (FC), C-reactive protein, and a panel of cytokines. The ability of BG to detect active vs inactive disease was assessed using the area under the receiver operating characteristic curve. In subgroup analysis, serial BG was used to assess the response to therapeutic interventions. RESULTS: The serum BG levels were higher in CD patients than in controls (P = 0.0001). The BG levels paralleled the endoscopic activity in CD patients and histologic activity and combined endoscopic and histologic activity in both CD and UC patients. The area under the curve (AUC) in receiver operating characteristic analysis to predict endoscopic activity was 0.694 [95% confidence interval (CI): 0.60-0.79; P = 0.001] in CD, and 0.662 (95%CI: 0.51-0.81; P = 0.066) in UC patients. The AUC in receiver operating characteristic analysis to predict histologic activity was 0.860 (95%CI: 0.77-0.95; P < 0.001) in CD, and 0.786 (95%CI: 0.57-0.99; P = 0.015) in UC patients. The cut-off values of BG for both endoscopic and histologic activity were 60 µg/mL in CD, and 40 µg/mL in UC patients. Performance analysis showed that the results based on BG of 40 and 60 µg/mL were more specific for predicting endoscopic activity (71.8% and 87.2% for CD; and 87.5% and 87.5% for UC, respectively) than FC (53.3% and 66.7% for CD; and 20% and 80% for UC, respectively); and also histologic activity (60.5% and 76.3% for CD; and 90.0% and 95.0% for UC, respectively) than FC (41.7% and 50.0% for CD; and 25% and 50% for UC, respectively). Regarding the clinical, endoscopic, and histologic activities, the BG levels were reduced following therapeutic intervention in patients with CD (P < 0.0001) and UC (P = 0.003). Compared with endoscopic (AUC: 0.693; P = 0.002) and histologic (AUC: 0.868; P < 0.001) activity, no significant correlation was found between serum BG and transmural healing based on magnetic resonance enterography (AUC: 0.576; P = 0.192). Positive correlations were detected between BG and IL-17 in the CD (r: 0.737; P = 0.001) and the UC group (r: 0.574; P = 0.005), and between BG and interferon-gamma in the CD group (r: 0.597; P = 0.015). CONCLUSION: Serum BG may represent an important novel noninvasive approach for detecting mucosal inflammation and therapeutically monitoring inflammatory bowel diseases, particularly in CD.

RESUMO

ABSTRACT: Mucosal healing (MH) has become a major target in the management of ulcerative colitis (UC). Because repeat endoscopy is expensive and invasive, we aimed to evaluate fecal calprotectin (FC) as an alternative marker to predict MH in UC.Eighty patients with UC in clinical remission were consecutively included in a prospective observational study. FC was measured using a quantitative enzyme-linked immunosorbent assay. The colonic mucosa was assessed for endoscopic and histological measures of inflammatory status. Endoscopic and histological remission were defined according to the Mayo endoscopic subscore (MES) and Geboes score (GS), respectively. Deep remission was defined as a combination of the MES and GS. FC performance and cutoff values for identifying MH and deep remission were determined using contingency tables and receiver operator characteristic (ROC) and area under the curve (AUC) analysis. The median FC concentration in patients who met the criteria for deep remission (MES ≤1 and GS < 3.1) was 65.5 µg/g, while that in patients with disease activity was 389.6 µg/g (P = .025). A FC cutoff value of 100 µg/g, determined by the ROC analysis, resulted in sensitivity and specificity of 91.7% and 57.1%, respectively, for histological remission, and 82.4% and 60.9%, respectively, for deep mucosal remission. Positive correlations were detected between FC concentrations with the histologic (CC: 0.435; P < .001) and the combined endoscopic and histologic (CC: 0.413; P < .001) scores.FC can be used confidently as a noninvasive biomarker to predict deep remission in patients with UC in clinical remission when concentrations are below 100 µg/g.

RESUMO

BACKGROUND AND OBJECTIVE: The phytophagous insects select their host plants according to plant tissue nutritional quality. Thus, the objective of this study was to correlate the direct and indirect effects of phosphate fertilization on the nutritional status of the soybean crop and its relationship with the occurrence of insect pests. MATERIALS AND METHODS: The effect of phosphate fertilization on soybean was evaluated using two phosphate sources, Single Superphosphate (SSP) and Monoammonium Phosphate (MAP), applied at five rates of P2O5. A sampling of insects initiated from stage V5 and was done weekly. Leaves were collected for nutrient analysis stage R1, in stage R9 was harvest was carried out. SSP or MAP phosphate fertilization in soybean affected the incidence of Chrysodeixis includens, Helicoverpa armigera, Elasmopalpus lignosellus and Euschistus heros. RESULTS: The plants treated with MAP had the infestation reduced compared with plants treated with SSP. Higher contents of Cu and Fe in the leaf reduces the incidence of insect-pests, whereas the opposite occurred with Mn. The occurrence of E. lignosellus reduced soybean yield. CONCLUSION: Therefore, the source and rates of phosphorus in soybean fertilization change the concentration of macro and micronutrients in the leaves and affect the behavior and incidence of pest species.

RESUMO

OBJECTIVE: To investigate the advantages of using modified signal intensity measurements on chemical shift imaging alone or in conjunction with proton magnetic resonance spectroscopy in the differential diagnosis of adrenal adenomas. MATERIALS AND METHODS: This was a prospective study involving 97 patients with adrenal nodules or masses. The signal intensity index (SII) was calculated as [(signal intensity on the in-phase image - signal intensity on the out-of-phase image) ∕ (signal intensity on the in-phase image)] × 100%. We determined the averages of the minimum, mean, and maximum signal intensity values measured on three consecutive images. When that was not possible (for smaller lesions), we used one or two images. We employed a region of interest that covered one half to two thirds of the mass. All indices were compared with metabolite ratios derived from spectroscopy: lactate/creatine; glutamine-glutamate/creatine; choline/creatine; choline/lipid; 4.0-4.3 ppm/Cr; and lipid/creatine. RESULTS: Of the 97 patients evaluated, 69 were diagnosed with adenomas and 28 were diagnosed with nonadenomas. All SII measurements and spectroscopy-derived metabolite ratios were significant to the differentiation between adenomas and nonadenomas, except for the lipid/creatine and choline/lipid ratios. In 37.8% of the cases, it was not possible to perform spectroscopy. When it was possible, the lactate/creatine ratio was found to have higher accuracy than did the SII. CONCLUSION: Determining the SII and metabolite ratios increased the accuracy of the differential diagnosis of adrenal adenomas.

Meniscal lesions are one of the main causes of pain and lameness in horses. The objective of this study was to evaluate the morphometry of the meniscus of the femorotibial joint of horses. 48 meniscus were used from 12 animals, aged between five and 15 years and weighing between 400kg and 500kg. The measurement of the peripheral extension went from the most cranial part to the most caudal, denominated external circumference (CE). The internal margin, with the same treatment, was called internal circumference (IC). The meniscus was divided into cranial, middle and caudal thirds. The thickness was obtained at the midpoints of each third. The area of the meniscus in contact with the femoral condyles was also calculated. The medial meniscus presented a higher CE with a mean of 126.38mm, while the lateral meniscus presented a mean of 115.32mm. The lateral meniscus showed greater thickness in the middle and caudal thirds, with mean values of 16.00mm and 19.85mm respectively, against 13.75mm and 14.99mm of the medial meniscus. The results of this study showed an important relationship between the morphometric data and the clinical findings in an attempt to explain the higher incidence of lesions involving the medial meniscus.(AU)

RESUMO

ABSTRACT Objectives: To identify the group of patients who could safely avoid prostate biopsy based on the findings of multiparametric prostate resonance imaging (MRmp), parameterized with PI-RADS v2, using prostate biopsy as reference test and to assess the sensitivity and specificity of mpMR in identifying clinically significant prostate cancer using prostate biopsy as a reference test. Patients and Methods: Three hundred and forty two patients with suspected prostate cancer were evaluated with mpMR and prostate biopsy. Agreement between imaging findings and histopathological findings was assessed using the Kappa index. The accuracy of mpMR in relation to biopsy was assessed by calculations of sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV). Results: A total of 342 biopsies were performed. In 201 (61.4%), mpMR had a negative result for cancer, which was confirmed on biopsy in 182 (53%) of the cases, 17 (4.9%) presented non-clinically significant cancer and only 2 (0.5%) clinically significant cancer. 131 (38.3%) patients had a positive biopsy. Clinically significant cancer corresponded to 83 (34.2%), of which 81 (97.5%) had a positive result in mpMR. Considering only the clinically significant cancers the mpMR had a sensitivity of 97.6%, specificity of 76.8%, PPV 57.4% and VPN of 99%. Conclusions: mpMR is a useful tool to safely identify which patients at risk for prostate cancer need to undergo biopsy and has high sensitivity and specificity in identifying clinically significant prostate cancer.

RESUMO

OBJECTIVES: To identify the group of patients who could safely avoid prostate biopsy based on the findings of multiparametric prostate resonance imaging (MRmp), parameterized with PI-RADS v2, using prostate biopsy as reference test and to assess the sensitivity and specificity of mpMR in identifying clinically significant prostate cancer using prostate biopsy as a reference test.

RESUMO

Lipaphis erysimi (L.) Kaltenbach (Hemiptera: Aphididae) is one of the most important pests of brassica crops, mainly causing losses due to sap sucking, toxin injection and viral transmission. Knowledge about the main natural factors that regulate populations of this pest, as well as its critical mortality stage, is crucial for the development of integrated pest management of L. erysimi. Here, we determined the critical stage and key mortality factors for L. erysimi in cabbage using an ecological life table. Causes of mortality at each stage of L. erysimi development were monitored daily in the field for seven seasons. From the experimental data, we determined the key factor and critical stage of mortality through correlation and regression analyses. The nymphal stage, especially first instar nymphs, was critical for L. erysimi mortality. The key mortality factors were, in descending order of importance, physiological disturbances and predation by Syrphidae, Coccinellidae and Solenopsis ants. Therefore, control measures should target early stages of L. erysimi and the use of cabbage cultivars that have negative effects against L. erysimi may be a promising strategy for its management. Our results may be useful for plant geneticists who could develop new cabbage cultivars based on these findings. In addition, conservation measures of the main predators of L. erysimi may contribute to the natural control of this pest.

Breast cancer is the most frequent cancer among women, and its diagnosis can be made by ultrasound, mammography or MRI, along with biopsy. In patients with dense breasts, MRI is considered the best diagnostic method for its high sensitivity when compared to other techniques. Plus, there's the application of gadolinium contrast medium, which is impregnated in the affected tissue for easier viewing the tumor and the possibility of selectively saturate the fat signal, helping thus the diagnosis of breast cancer.

RESUMO

There is very little information available about Culicoides species (Diptera: Ceratopogonidae) in the western Brazilian Amazon. However, studies of the fauna of this region are essential to knowledge of the species and potential vectors within it. Thus, the present study aims to evaluate the abundance, richness and composition of Culicoides species in rural areas in the state of Rondônia, Brazil. Culicoides specimens were collected in forest and pasture environments in the municipality of Porto Velho, using light traps. A total of 1708 individuals (1136 females and 572 males) belonging to 33 species were collected; 28 of these samples represent new records for the state of Rondônia and include the first record of Culicoides contubernalis in Brazil. Culicoides insignis was the most abundant species (86.1%). Species richness was greater in forest areas (32 species, 96.96%), whereas pastures presented the greatest number of Culicoides captured (n = 1540, 90.1%). This study shows that Culicoides populations differ between forest and pasture environments and indicates that the abundance of C. insignis is an important factor in epidemiological vigilance studies in the region.

RESUMO

ABSTRACT Diabetes mellitus is a chronic and severe metabolic dysfunction, it's slow and progressive evolution interferes directly in the metabolism of carbohydrates, fats and proteins, causing hyperglycemia, glycosuria, polydipsia, hyperlipidaemia, among others. The aim of this study was to evaluate the antidiabetic effect of hydroethanolic extract and granulated of Calophyllum brasiliense Cambess., Clusiaceae, species in diabetic rats as well as it's biochemical parameters. The results demonstrated that both the pharmaceutical forms, hydroethanolic extract and granulated, were able to reduce significantly (p < 0.001) hyperglycemia and glycosuria, in addition to improve polydipsia, polyuria, and weight loss. Treatments using hydroethanolic extract and granulated were also able to reduce significantly levels of triacylglycerides, cholesterol and low-density lipoprotein, as well as the transaminases, urea and creatinine levels. Therefore, it is concluded that these pharmaceutical forms have anti-diabetic effect and act improving the biochemical parameters, this effect is probably due to the high content of polyphenolic compounds found in the formulations.

RESUMO

Revolutionizing treatment strategies is an urgent clinical need in the fight against cancer. Recently the scientific community has recognized chromatin-associated proteins as promising therapeutic candidates. However, there is a need to develop more targeted epigenetic inhibitors with less toxicity. Sin3 family is one such target which consists of evolutionary conserved proteins with two paralogues Sin3A and Sin3B. Sin3A/B are global transcription regulators that provide a versatile platform for diverse chromatin-modifying activities. Sin3 proteins regulate key cellular functions that include cell cycle, proliferation, and differentiation, and have recently been implicated in cancer pathogenesis. In this chapter, we summarize the key concepts of Sin3 biology and elaborate the recent advancements in the role of Sin3 proteins in cancer with specific examples in multiple endocrine neoplasia type 2, pancreatic ductal adenocarcinoma, and triple negative breast cancer. Finally, a program to create an integrative approach for screening antitumor agents that target chromatin-associated factors like Sin3 is presented.
