RESUMO

We conducted a radiographic assessment of 180 flanged modular titanium alloy Capital femoral hip replacements. Six significant factors associated with failure are identified. The mean follow-up period was 7.9 years (2.1-16 years) and there were 26 (14%) radiographic failures irrespective of whether these were symptomatic or led to revision. Failure, if it is going to occur, is initiated early, within an average of 1.4 years, and becomes clinically evident very quickly; the remainder appear to sustain more likelihood of stable survival. This is the longest published follow-up and it suggests that the survival is better than indicated in some other series. If the identified prejudicial aspects of surgical technique had not been present, it follows that the results would have been better. Comparison with other published series is attempted.

RESUMO

OBJECTIVE: To investigate the clinical findings and to identify the microorganisms in the pathogenesis of otitis externa. METHODS: The symptoms and signs, as well as the bacterial flora, from the auditory canals of 70 Jordanian patients (94 ears) suffering from otitis externa were studied during the period from February 1999 to February 2000. RESULTS: Pain was the most common symptom (82%). The most common sign was erythema (65%), while oedema was the least common (42%). Pseudomonas aeruginosa was found in 39%, Aspergillus in 27%, Candida albicans in 18%, Staphylococcus aureus in 18% and no growth in 8.5%. Ninety three percent of healthy ears revealed normal skin flora. CONCLUSION: Gram-negative microorganisms (Pseudomonas aeruginosa) were the most frequently isolated pathogenic microorganisms.

RESUMO

A case of patellar stress fracture after total knee arthroplasty in a man with gout and previous osteonecrosis of the tali is reported. The combination of fat pad excision and lateral release causing disruption to the patellar blood supply during primary total knee arthroplasty resulted in the development of a patellar fracture. Avascular necrosis, caused by gout, may form part of the pathogenesis.
