RESUMO

Application of Margaret Newman's theory of Health as Expanding Consciousness is presented as beneficial for both the school nurse and the adolescent with Insulin Dependent Diabetes Mellitus (IDDM). The tenets of Newman's theory are reviewed and application drawn to adolescent development, the management of IDDM, and the development of an Individualized Healthcare Plan (IHP). Implications of the theory for school nurse case management and the process of identifying an adolescent's meaning of health are explored.

RESUMO

Newman's model of health as expanding consciousness provided the framework for investigation of the phenomenon of powerlessness among aging women. The study sought to identify a pattern of relationships among the impact of chronic illness, frame of temporal reference, death anxiety, hopelessness, and powerlessness. However, subjects manifested high levels of perceived situational control or powerfulness, with restricted range and variance of powerlessness scores. Thus the research questions were not answered. A profile of health patterns among active, aging women did emerge from the findings that it is congruent with Newman's model of health as expanding consciousness.

RESUMO

A study was conducted to investigate the relationship of human rhythms, as expressed by heart rate and blood pressure, in response to environmental rhythms, including a companion dog and an unknown dog, within the context of Rogers' Principle of Integrality. A convenience sample of 30 was used in replicating a study by Baun et al. Results did not support the hypothesis but did support and clarify the relationship between human and environmental fields as postulated by Rogers.

In a study of turkeys over the period 0 to 3 wk of age, body weights of birds fed 5 or 15% dietary peat or carrier-type feed byproducts (rice hulls, sunflower hulls, peanut hulls, soybean mill feed. wood flour, and corn cobs) were compared to those of turkeys fed a diet composed primarily of corn and soybean meal. Turkeys fed 15% dietary peat or carriers showed a greater growth depression than those fed the diluents at the 5% dietary level. At the 5% peat or carrier level, growth was not significantly (P greater than .05) depressed when birds were fed rice hulls (body weight was 98.5% of control), peat (97.1%), sunflower hulls (96.9%), and peanut hulls (95.8%); growth was significantly depressed (P less than .05) when birds were fed corn cobs (93.1%), soybean mill feed (92.9%), and wood flour (92.8%). Peat, when used as a drying agent for liquid agricultural byproducts, would account for 2 to 4% of the diet. Results of this study suggest that peat as a carrier fed at the 5% level would minimally affect growth. In a market study male turkeys were raised on floor pens containing peat or wood shavings and fed 0, 5, or 10% reed-sedge peat as a diluent of a typical corn-soybean meal mash diet. Body weights of turkeys fed 5 or 10% dietary peat showed a slight growth depression on both litter materials, compared with control. Higher body weights (nonsignificant) were observed for turkeys on wood shavings compared with those on peat litter after 12 wk of age.(ABSTRACT TRUNCATED AT 250 WORDS)

A large number of cases (303) of primary granulomatous inflammation, present in surgically removed specimens from a broad range of locations, were reviewed and examined in detail. The granulomas were characterized as to morphology, location, and etiology. Epithelioid granulomas without necrosis were most frequently due to sarcoidosis and mycobacterial infection, while epithelioid granulomas with necrosis were most commonly due to mycobacterial infection, fungal infection, rheumatoid arthritis and sarcoidosis. Mature granulomas, most of which did not contain necrosis, were generally due to foreign bodies. The responsible etiologic agents were generally identified in granulomas due to fungi, bacteria, or foreign bodies (92%). However, mycobacteria were infrequently identified in granulomas, even when the lesions were examined by the auramine-O technique (31%). Overall, the majority of the granulomas (76%), were due to five causes: sarcoidosis, mycobacterial infection, particulate inclusions, fungal infection, and rheumatoid arthritis. The frequency of a given etiology, however, varied widely depending on the location of the lesion. By classifying granulomas morphologically and knowing their location, useful predictive information concerning the etiology of a given granuloma, beyond that obtained by histochemical stains, could be derived.

RESUMO

Generalized histiocytic proliferation with a component of phagocytosis and thrombocytopenia accompanies a number of infectious diseases and malignant neoplasms. Its extent and occurrence were studied in the hematopoietic and lymphoreticular tissues of fatal childhood cases of Rocky Mountain spotted fever. Its frequent occurrence and morphologic similarities to other associated causes were documented.

In 42 patients with metastatic adenocarcinoma of the prostate, histopathologic specimens of the original prostatic biopsy and lymphatic metastasis were classified using the grading system of tumor differentiation described by Gleason et al. The observed similarity between prostatic biopsy morphologic and the morphology in the corresponding lymphatic metastasis supports the concept that the histologic growth pattern of the primary tumor is reduplicated in the metastatic lesions. The observation that a more anaplastic pattern did not appear in the metastatic deposits implies that factors other than cellular loss of differentiation are responsible for the cells' ability to metastasize.

Of 44 patients with clinically localized prostatic adenocarcinoma and regional lymphatic metastases proved by staging pelvic lymphadenectomy 11 were treated by radical prostatectomy, 20 received extended field radiation and 13 were assigned to delayed hormonal therapy. The median survival for the entire group was 39.5 months. None of the 3 treatments appeared superior in prolonging survival. Furthermore, no enhancement of disease control could be demonstrated in either treatment group. Patients with prostatic adenocarcinoma and positive nodes appear to have equivalent adverse biological potential and should be candidates for treatments designed to produce systemic effect.

RESUMO

The clinical course of 75 males subjected to radical prostatectomy after pelvic lymphadenectomy were compared with that of 29 males receiving radical without lymphadenectomy. The adverse survival impact of positive pelvic nodes and the impact of pelvic lymphadenectomy are discussed.
