RESUMO

The most common side effect of nonsteroidal anti-inflammatory drug (NSAID) therapy involves gastrointestinal damage that may involve hemorrhages and petechiae to erosions and ulcers. The NSAID usage increases the risk for ulcer complications such as bleeding and perforation. While prophylaxis against NSAID-induced ulcers is an appealing clinical strategy, the studies to date concerning its effectiveness have been flawed by uneven experimental and statistical design. Important questions concerning management of NSAID-induced ulceration have yet to be answered.

RESUMO

Achalasia is the most recognized esophageal motor disorder first described 300 years ago by Sir Thomas Willis. Although the cause of achalasia remains unknown, the disease is thought to occur as a result of degeneration of Auerbach's plexus in the intramural nervous network. A review of the literature reveals that there is reversibility of this disease in some patients after treatment, raising intriguing questions as to the pathogenesis of this disorder and underscoring the need for further study.

RESUMO

We describe a previously unreported association between somatostatinoma and congenital pseudoarthrosis. Because pseudoarthrosis is part of von Recklinghausen's disease, in turn with an association with somatostatinoma, we speculate on a link between pseudoarthrosis and somatostatinoma.

RESUMO

The 99mtechnetium pertechnetate nuclear red blood cell scan proved to be a valuable diagnostic tool in localizing the site of bleeding, from a Meckel's diverticulum, in a 41-year-old man. When peptic ulcer or colorectal disease has been excluded by panendoscopy, bleeding Meckel's diverticulum remains a diagnostic consideration, even in adults. We provide a 20-year list of bleeding Meckel's diverticula reported in adults, along with a brief discussion.

RESUMO

One thousand thirty-four cases of epidermoid carcinoma of the mouth and pharynx were seen by the Head and Neck Service of Memorial Hospital during a recent 5 year period, 1965 to 1969. The 656 determinate cases previously untreated offer a clear basis for evaluation of the 5 year results of treatment, which as surgical in 90 percent of the cases. Preoperative radiotherapy was used in 130 cases. Preoperative radiotherapy was used in 130 cases, matched with 126 untreated cases. Etiologic factors are discussed. The presence and degree of lymph node metastasis are of paramount significance. Indications for elective treatment of the clinically negative neck are discussed in terms of anatomic, logistic and statistical principles. An analysis of failure point toward a continuing effort at combined therapy in this regional disease.
