RESUMO

UNLABELLED: The purpose is to evaluate the effectiveness of itopride (IP) and domperidone (DP) in the treatment of patients with gastroesophageal reflux disease (GERD), in combination with diabetes mellitus (DM) type 1 and 2. MATERIALS AND METHODS: 40 patients were examined with GERD and type 1 diabetes and 50 patients with GERD and type 2 diabetes. Each group of patients with GERD, DM 1 and 2 has been divided into: the basic subgroup receiving IG 50 mg 3 tid and control--DP 10 mg tid. Patients were also administered omeprazole. Both subgroups were strictly randomized to key indicators, except for therapy. Baseline and after 2 and 4 weeks, all patients were examined to identify complaints, endoscopy and pH-metric changes, gastric motility was studied by electrogastroenterographic method (PEGEG). RESULTS: In the subgroups of patients with GERD + DM 1 and GERD + DM 2, received treatment with IG complaints on heartburn, regurgitation, odynophagia relieved significantly earlier then in the subgroups treated with DP. After 4 weeks of therapy, decreasing in the number of gastroesophageal refluxes, number of patients with erosive esophagitis B level, and normalization of the motility of the stomach were significantly higher in the groups of GERD + DM 1 and GERD + DM 2 received treatment with IG when compared with the subgroup of PD. There were no side effects of prokinetics. CONCLUSION: IG was more effective then DP in the treatment of GERD in patients with diabetes, and may be recommended for inclusion in the scheme of treatment of this comorbidity.

RESUMO

1. The daily gastric pH monitoring in patients with diabetes of the first type failed to reveal any differences in the state of intragastric acidity, as compared with the control group of healthy people. A tendency towards increased stomach acidity is observed only during the first five years of type I diabetes, and it is normalized later on. 2. 68% patients with type II diabetes had normal gastric acidity, and only 23% had hypoacidity. A tendency towards moderate hypoacidity in the stomach body was observed on average when the disease lasted for more than 5 years. 3. In case of type I diabetes in combination with chronic gastroduodenal ulcers at the acute stage, the intragastric pH in the whole group was reduced to the degree of pronounced hyperacidity with the decompensation of alkalization in the antral portion of the stomach. 4. In case of type II diabetes in combination with chronic gastroduodenal ulcers at the acute stage, there also was a tendency towards hyperacidity in the stomach for the whole group. Moderate intragastric hyperacidity was recorded in case of stomach ulcers, and pronounced intragastric hyperacidity was recorded in case of duodenal ulcers.

RESUMO

105 patients with coronary heart disease and functionally varying angina pectoris received in sanatoria or inpatiently segmental nitroglycerin electrophoresis. The microcirculatory bed was studied with bulbar biomicroscopy in comparison with clinical effects. There appeared a relationship between improvements in microcirculation and clinical symptoms. The course of 10-15 procedures at maximum strength of current 0.03 mA/cm2 proved effective in 78% and 95% of inpatients and sanatorium patients, respectively.

Content of total free serotonin, histamine, kallikrein and prekallikrein was studied in blood of 45 patients with hemorrhagic fever accompanied by kidney syndrome; vessel permeability was also evaluated by means of radiological procedure. Analysis of correlation between vessel permeability and content of these biologically active substances in blood was carried out. Content of kallikrein was distinctly increased in blood plasma of the patients; content of serotonin and histamine was less markedly altered. Vessel permeability was elevated during all the disease periods; it was especially high in the period of oligoanuria. Analysis of the data obtained showed that an increase in vessel permeability correlated most distinctly with augmented content of kallikrein in the blood, while correlation between vessel permeability and content of histamine and free serotonin was less distinct.

RESUMO

The study determined the content of free, total and thrombocytic serotonin in 26 patients suffering from hemorrhagic fever with renal syndrome (HFRS) varying in stage. Oligoanuria and polyuria periods of the disease were associated with a 5.5-fold increase in the level of free serotonin whereas the level of blood platelet serotonin reduced two-fold and more. The convalescence period was characterized by rapid recovery of free serotonin values, thrombocytic serotonin being still insufficient. Peak values of total serotonin were noted in the polyuric period.

As many as 56 patients with associated hemorrhagic fever and the renal syndrome were examined for vascular permeability, microcirculation and blood plasma histamine, serotonin, kallikrein and prekallikrein. In all the disease periods, the majority of the patients (n-52) demonstrated a rise of vascular permeability for protein and liquid part of the plasma, with this being marked in particular in the oligoanuric period. Besides, significant perivascular and intravascular changes in microcirculation were noted in the oligoanuric and polyuric disease periods. It was established during examination of biologically active substances that the greatest shifts were experienced by the kinin system. Sometimes the content of kallikrein surpassed the control values more than 7-fold. Less pronounced changes were seen in blood plasma serotonin and histamine. A direct correlation was established between vascular permeability, microcirculation and the content of all biologically active substances explored by the authors. The strongest correlation was found to exist between the above parameters and the kinin system of blood plasma.

Vascular permeability and microcirculation (MC) were studied in 25 patients with hemorrhagic fever associated with the renal syndrome (HFRS). A considerable increase in vascular permeability for liquid and protein was recorded during oligoanuria and polyuria. These parameters slowly returned to normal with recovery. Investigation of MC identified the presence of changes mainly occurring in the intra- and extravascular parts of MC: turbidity of the angioscopic background, hemorrhages, spread sludge, atonia of the venules, winding of all groups of the vessels, aneurysms of the venules, and so forth. The increased vascular permeability and MC impairment are important components of the pathogenesis of HFRS which should be taken into consideration during institution of the pathogenetic treatment.
