RESUMO

Autoantibodies against myelin oligodendrocyte glycoprotein (MOG) have recently been established to define a new disease entity, MOG-antibody-associated disease (MOGAD), which is clinically overlapping with multiple sclerosis. MOG-specific antibodies (Abs) from patients are pathogenic, but the precise effector mechanisms are currently still unknown and no therapy is approved for MOGAD. Here, we determined the contributions of complement and Fc-receptor (FcR)-mediated effects in the pathogenicity of MOG-Abs. Starting from a recombinant anti-MOG (mAb) with human IgG1 Fc, we established MOG-specific mutant mAbs with differential FcR and C1q binding. We then applied selected mutants of this MOG-mAb in two animal models of experimental autoimmune encephalomyelitis. First, we found MOG-mAb-induced demyelination was mediated by both complement and FcRs about equally. Second, we found that MOG-Abs enhanced activation of cognate MOG-specific T cells in the central nervous system (CNS), which was dependent on FcR-, but not C1q-binding. The identification of complement-dependent and -independent pathomechanisms of MOG-Abs has implications for therapeutic strategies in MOGAD.

Background: The COVID-19 pandemic caused discontinuities in cancer care (CC) in most countries. Here, the authors describe the real-world impacts of implementing a contingency plan employing telemedicine for CC. Methods: A retrospective study of patients who received CC through telemedicine at the Instituto Nacional de Enfermedades Neoplasicas, Peru, from March 2020 to February 2021 was conducted. Impacts were measured by comparing the amount of CC administered during the pandemic versus the prior year. Results: A total of 16,456 telemedicine visits were carried out. An annual comparative analysis showed a gap of 23% and telemedicine accounted for 27.6% of the total CC administered during the pandemic. A high (4.50/5) level of patient satisfaction with telemedicine was reported. Conclusion: Telemedicine is an important tool to facilitate the continuity of CC.

Autoantibodies to myelin oligodendrocytes glycoprotein (MOG) are found in a fraction of patients with inflammatory demyelination and are detected with MOG-transfected cells. While the prototype anti-MOG mAb 8-18C5 and polyclonal anti-MOG responses from different mouse strains largely recognize the FG loop of MOG, the human anti-MOG response is more heterogeneous and human MOG-Abs recognizing different epitopes were found to be pathogenic. The aim of this study was to get further insight into details of antigen-recognition by human MOG-Abs focusing on the impact of glycosylation. MOG has one known N-glycosylation site at N31 located in the BC loop linking two beta-sheets. We compared the reactivity to wild type MOG with that toward two different mutants in which the neutral asparagine of N31 was mutated to negatively charged aspartate or to the neutral alanine. We found that around 60% of all patients (16/27) showed an altered reactivity to one or both of the mutations. We noted seven different patterns of recognition of the two glycosylation-deficient mutants by different patients. The introduced negative charge at N31 enhanced recognition in some, but reduced recognition in other patients. In 7/27 patients the neutral glycosylation-deficient mutant was recognized stronger. The folding of the extracellular domain of MOG with the formation of beta-sheets did not depend on its glycosylation as seen by circular dichroism. We determined the glycan structure of MOG produced in HEK cells by mass spectrometry. The most abundant glycoforms of MOG expressed in HEK cells are diantennary, contain a core fucose, an antennary fucose, and are decorated with α2,6 linked Neu5Ac, while details of the glycoforms of MOG in myelin remain to be identified. Together, we (1) increase the knowledge about heterogeneity of human autoantibodies to MOG, (2) show that the BC loop affects recognition in about 60% of the patients, (3) report that all patients recognized the unglycosylated protein backbone, while (4) in about 20% of the patients the attached sugar reduces autoantibody binding presumably via steric hindrance. Thus, a neutral glycosylation-deficient mutant of MOG might enhance the sensitivity to identify MOG-Abs.

El artículo que presentamos tiene como objetivo describir un sistema de acciones y operaciones para desarrollar la habilidad diagnosticar en el proceso de atención de enfermería y ser aplicado en la docencia de manera que contribuya en la calidad del egresado. A partir del análisis del perfil profesional y del grado de desarrollo científico-técnico alcanzado y su relación con la identificación de los problemas que constituyen la base del plan de cuidados en el proceso de atención de enfermería se estructuran las acciones y operaciones con una secuencia lógica para llegar al diagnóstico.


The process of nursing primary health care is the application of the scientific method in the assistencial practice of this discipline in such a way that it can offer systematic, logical and rational care. The present article has the objective to describe a system of actions and procedures to develop the ability of diagnosing in the process of nursery primary health care and be applied in teaching to favour the increasement of the egressed quality. Actions and procedures have been structured with a logical secuence to reach the diagnosing, starting with the analysis of the professional profile, the level of scientific and technical development reached and its relation with the identification of problems that constitute the base of the patient plan of care in the process of nursery primary health care.

A serological study was undertaken in 1998 to evaluate levels of Taenia solium cysticercosis in 3 rural Venezuelan communities. Infection with viable metacestodes was diagnosed with a trapping enzyme-linked immunosorbent assay (ELISA) that detects a secreted product of viable parasites. Anti-metacestode antibodies were assayed by ELISA using T. solium vesicular fluid as antigen. A total of 1254 sera was collected from 3 communities (Canoabo, Sanare, and Rio Tocuyo) where previous studies had suggested the presence of T. solium. Our results demonstrate an unusually high seroprevalence of cysticercosis, indicating an attendant risk of transmitting the disease to other areas. The seroprevalence of infection with viable cysts, as indicated by detection of circulating parasite antigen, was 9.1% in Canoabo, 6.1% in Sanare, and 5.7% in Rio Tocuyo. The corresponding frequency of antibodies to T. solium cyst antigens was 36.5% in Canoabo, 36.5% in Sanare, and 4% in Rio Tocuyo. As these communities are probably representative of many others in Venezuela, T. solium cysticercosis may be a significant public health problem and more work is certainly indicated. An important finding was that local knowledge of the disease and its transmission do not necessarily guarantee diminished disease prevalence, indicating a lack of appropriate vigilance towards disease control.

El agua es un recurso fundamental para la vida, no obstante, de ser esencial, es considerada como vehículo de enfermedades, debe reunir condiciones básicas de calidad. Determinadas en esta investigaciones mediante análisis bacteriológico: número más probable de coliformes totales, fecales y escherichia coli (métodos de tubos múltiples), y análisis parasitológico solución salina, lugol, faust y coloración de kin youn a 31 muestra de agua provenientes de diferentes fuentes de captación: grifos, filtros, pipotes, tanques y botellón. Encontrándose un elevado nivel de contaminación de 70,97 por ciento, con un recuento de aerobios mesófilos de 83,9 por ciento, coliformes totales 74,23 por ciento, fecales 67,8 por ciento, escherichia coli 67,8 por ciento y 80,64 por ciento de positividad para protozoarios como: blastocystis hominis, endolimax nana, giardia lamblia, criptosporidium sp, su presencia posiblemente se deba a fallas en los procesos de filtración y desinfección. Los indicadores bacteriológicos representan un criterio de calidad; pero no demuestran la presencia o ausencia de protozoarios en el agua, por ello se deben realizar. Además se realizó análisis coprológico de 27 escolares, observándose presencia de: endolimax nana, blastocystis hominis, giardia lamblia, criptosporidium sp, probablemente transmitidos por el consumo de agua contaminada. La calidad del agua no cumple con los criterios establecidos por la OMS/OPS (1998) haciéndolo no apta para el consumo humano
