RESUMO

In a previous study, we reported the cytotoxic activity against various tumour cells of the peptidoglycan of Lactobacillus casei. To isolate the most active components, we performed column-chromatography separation of the peptidoglycan complex and tested the related fractions for their cytotoxic activity. The most active fractions were then lyophilized and the residue was analysed by gas chromatography for its amino acid content and composition. On the basis of the known chemical formula of the basic peptidic component of the peptidoglycan complex of L. casei, a peptide was then synthesized [Europ. (CH-DE-FR-GB) Patent number 1217005; IT number 01320177] and its cytotoxicity was tested against tumoural and normal cells. The synthetic peptide was found to impair the entire metabolism of cultured tumour cells and to restore the apoptotic process. By contrast, normal cells appeared to be stimulated rather than inhibited by the peptide, whereas primary mouse embryo fibroblasts behaved similarly to tumour cells. On the basis of these results, L. casei peptidoglycan fragments and their constituent basic peptide might be applicable as potent antitumour agents.

AIM: To test three hypotheses in an Italian sample of call center workers: higher levels of perceived work stress are associated with more frequent common mental disorders (GHQ-12) and a lower Work Ability Index; combining the Job Strain (JS) and Effort/Reward Imbalance (ERI) models increases explained variance in health over and above either model when applied separately; compared with outbound operators, inbound call handlers are expected to report a lower health status,which is due to a more intense exposure to task-related work stress factors in the latter. DESIGN: A multi-center cross-sectional study, conducted by means of interviews and self-administered questionnaires. SETTING AND PARTICIPANTS: Call handlers working in the Italian branch of a telecommunication multinational company. In all, 1,106 permanent workers were examined (35.9%of the total target population, 98.9% response rate). The majority were women (76.5%);mean age was 33.3 (SD: 3.9) and company seniority 8.0 (SD: 2.1). Nearly 60% worked as inbound call handlers, about one third as outbound operators. EXPOSURES: Work stress was measured with the well-known JS and ERI models. Three exposure levels (based on tertiles) were identified for each scale. OUTCOMES: Common mental disorders were measured with the GHQ-12 questionnaire. Subjects with a GHQ-12 score 4 were classified as "cases". The Work Ability Index (WAI) was used to evaluate work ability. Being in the "poor" or "moderate" categories of the WAI indicated a low work ability status. Cronbach's alphas were 0.70 for all scales. RESULTS: Multivariate Poisson regressions showed that both models were linked to more frequent common mental disorders and a lower WAI. Moreover, combined models demonstrated an advantage in terms of explained variance in health. Finally, performing inbound call handling was associated with a lower WAI in comparison with engaging in outbound activities. Mediation analyses showed that such association is explained by the higher levels of psychological job demands and Job Strain experienced by inbound operators. CONCLUSIONS: Our results highlight the relevance of work stress as a risk factor for lower psychological health, and especially for a poorer WAI among call center workers. The combined use of the two models increases completeness of work stress assessment in this sector.The higher levels of work stress and the lower WAI observed among inbound operators are due to objectively less favourable task-related characteristics.

RESUMO

The Italian Advisory Committee on work-related stress has outlined a step-by-step approach which considerably limits workers' participation in the first phases of the assessment procedure. Albeit cost-effective, such approach neglects the relevance of employees' perception in correctly identifying likely risk factors for job stress. Results of the present study, considering call center workers, points to the need of adopting subjective instruments for job stress assessment that are context-specific, whereas using generic job stress models only (such as Job Demand-Control and Effort Reward/Imbalance) may not permit the identfication of risk factors that are peculiar to the work settings under investigation.

BACKGROUND: Intragastric band migration is an unusual but major long-term complication of gastric banding: its frequency ranges from 0.5-3.8% and always requires removal of the band. Different laparoscopic, laparotomic or endoscopic methods are currently used for band removal. METHODS: 571 morbidly obese patients underwent adjustable gastric banding from February 1998 to July 2006. Band erosion occurred in 3 patients (0.52%). In addition, 6 such patients were referred to our Department from other hospitals. To remove the migrated band, in most patients we used an endoscopic approach with a device designed to cut the band: the Gastric Band Cutter (AMI, Agency for Medical Innovation). RESULTS: In 7 of the 9 patients, we used the gastric band cutter to remove the band endoscopically. It was able to cut the band successfully in all cases except one, where twisting of the cutting wire required conversion from endoscopy to laparoscopy. In another case, the band, after being cut, was locked in the gastric wall and required laparotomic removal. In 2 patients, we had to remove the band surgically - in one case for port-site infection with subphrenic abscess, and in the other case for complete band migration into the jejunum associated with acute pancreatitis, cholelithiasis and choledocholithiasis. CONCLUSION: The Gastric Band Cutter, when used, was successful in dividing the band in all cases except one, although we could not always complete the procedure endoscopically. Endoscopic removal seems to be the procedure of choice for band erosion, because it allows earlier patient discharge and avoids a surgical operation. It is advisable to perform the endoscopic removal in the operating theater, because of possible complications of the procedure.

INTRODUCTION: Since 1998, we adopted in our clinical practice for obesity treatment the new intragastric balloon (BioEnterics Intragastric Balloon, BIB), it has the characteristics of an "ideal gastric balloon" (Workshop Tarpon Springs, 1987). MATERIALS AND METHODS: Since March 1998 we have placed by endoscopic approach 518 BIB in 480 patients affected by first, second and severe morbid obesity; 124 male and 356 female, mean age 41.6 years (18-72), mean weight Kg 116.9 (67-229), mean BMI 42 Kg/m2 (27.34-81.14). All our patients underwent to diet of 1000 Kcal/day and treatment with antisecretory drugs. RESULTS: The mean weight lost was Kg 14.69 and the mean reduction in BMI was 5.11 Kg/m2. Weight loss was greater in male severe obese (BMI > 40). Weight was accompanied by an improvement of the disease associated with obesity, in particular diabetes, hypertension and sleep apnoea. DISCUSSION AND CONCLUSION: The best indication for BIB were: morbidly obese patients (BMI > 40) and super-obese patients (BMI > 50) in preparation for bariatric operation; obese patients with BMI 35-40 with co-morbilities in preparation for bariatric surgery; obese patients with BMI < 30 only in a multidisciplinary approach patients with BMI 30-35 with a chronic disease otherwise unresolved.

The gene SEL1L is involved both in human breast and pancreatic cancer progression. It is located on 14q24.3-31, a region known to be lost in invasive cancer of the esophagus. We aimed to assess whether SEL1L could become a useful biomarker for this cancer. We assessed SEL1L mRNA and protein expression in 35 patients and found it to be weak in low-grade and strong in high-grade dysplasia. Although the majority of cancer patients showed differential expression (mRNA and protein) of SEL1L, in five cases it was completely absent; these patients had the worst outcomes. SEL1L immunoreactivity was negative in normal tissue samples from five patients with mild esophagitis as well as in normal mucosa adjacent to the tumor. We hypothesize that SEL1L could influence those cellular changes that mediate the transition from a normal mucosa to a neoplastic lesion and may help in the identification of those patients at higher risk of developing this cancer. The specific impact of SEL1L in esophageal cancer needs further investigation.

Carotid artery atherosclerosis, age, family history, smoking, arterial hypertension, diabetes mellitus, and dyslipidemia represent risk factors of cerebrovascular events (stroke and transient ischemic attack). Hence, preventive treatment is vital for patients presenting these factors. Seven hundred and sixty-six patients (348 men and 418 women; age range 30-80 years) who were asymptomatic and attending a vascular unit for routine ultrasound (US) screening of the carotid arteries and with risk factors, intima-media thickness (IMT) of carotid artery > 1.3 mm, IMT associated with findings of atherosclerotic plaque creating stenosis > 20%, < or = 50%, and carotid plaque creating stenosis > 50% were considered. For patients with diabetes and hyperlipemia (35.8%)--or with hypertension, diabetes, and hyperlipemia (34.8%)--a higher prevalence of IMT > 1.3 mm was found. Higher prevalence of atherosclerotic plaque > 50% varied between 14.2% (in cases with diabetes mellitus and dyslipidemia) and 29.6% (in cases presenting 4 concomitant risk factors). Direct correlation was found between different risk factors and US findings. Our US findings revealed both a high incidence of carotid atherosclerosis, and that single and multiple correlation between US diagnosis of vascular damage and risk factors further enhanced risk. The study results confirmed the significance of vascular risk factors in patients with carotid artery atherosclerosis, and underlined the diagnostic validity of US.

Immunohistochemical detection of proliferating cell nuclear antigen (PCNA), densitometric analysis of nuclear DNA content and fluorescence in situ hybridization with centromere-specific DNA probes to chromosomes 11 and 17 were carried out on histological sections from 55 colorectal adenomas, in order to identify those adenomas that are more likely to progress to cancer. A considerable variability of PCNA positivity (range: 23-28.2%; mean value: 12.8%), a DNA indexfrom 1 to 2.3 and numerical alterations of chromosome 11 were observed. In particular, 14 out of 55 adenomas (25%), independent of histological type, degree of dysplasia, location and size, showed a DNA-aneuploid content, trisomies and tetrasomies of chromosome 11 and high PCNA index. It was concluded that the combination of some biomarkers (additional chromosomes 11, high PCNA and DNA indices) may allow identification of more aggressive colorectal adenomas with increased ability to undergo malignant transformation.
