Background: Chronic inflammatory skin diseases (CISDs) are among the most common diseases in the Western world. Current estimates of medical care for CISDs are primarily based on surveys among patients in medical care facilities and on health insurance data. Aim: Survey-based examination to what extent CISD patients in health-aware environment consider their skin disease to be controlled. Methods: The survey of CISD patients was carried out in 2022 among the employees of a pharmaceutical company located in Germany and Switzerland. Software-based, anonymous, self-reported questionnaires were used. Results: The number of employees, who answered the questionnaire, was 905. Of these, 222 participants (24.5%) reported having at least one CISD. 28.7% of participants with CISD described their disease as being hardly or not controlled. Regarding the nature of disease, more than one third of participants suffering from hidradenitis suppurativa (HS) or psoriasis fell into the hardly/not controlled category. In contrast, the largest proportion of participants with chronic spontaneous urticaria (43%) or atopic dermatitis (42%) considered their CISD to be completely or well controlled. Only 35.5% of CISD sufferers stated that they were currently under medical care for their skin condition. Being under medical care, however, had no influence on the extent CISD sufferers considered their skin disease to be controlled. The number of active CISD episodes but not the total number of symptomatic days per year was negatively associated with poor disease control (p = 0.042 and p = 0.856, respectively). Poor disease control had a negative effect on the personal and professional lives of those affected, as deduced from its positive association with the extent of daily activity impairment and presenteeism (p = 0.005 and p = 0.005, respectively). Moreover, 41.4 and 20.7% of participants with hardly/not controlled disease stated that their CISD had a moderate and severe or very severe impact on their overall lives (p < 0.001), respectively. A severe or very severe impact of their CISD on their overall life was most commonly reported by participants with HS. Conclusion: Medical care for CISDs, even in an environment with high socio-economic standard and high health-awareness, still appears to be limited and has a negative impact on individuals and society.

RESUMO

Workplace health promotion (WHP) is often faced with low-participation rates despite high relevance. This limits the potential for creating positive effects for the organization and its staff. Therefore, we investigated the barriers perceived by employees themselves using a representative sample (regarding age, gender and education) in Germany. Data were collected using a quantitative online questionnaire and then analyzed regarding underlying patterns. We found that there are different barrier types, and their importance differs depending on demographic criteria of the participants and the organizations they work for. These results can help organizations to reduce participation barriers to WHP for their staff.

BACKGROUND: Workplace health promotion (WHP) as a part of workplace health management (WHM) was strengthened in German legislature with the Prevention Act of 2015. However, smaller enterprises often do not offer WHM or WHP. Accordingly, a model-project for improving the uptake and implementation, particularly in micro-, small, and medium-sized enterprises (MSMEs) was carried out. The aim of the study was to determine reasons for non-participation in WHP offers and analyze communication issues, both from the employee's and employer's perspective. METHODS: In total, 21 managers or persons responsible for WHP participated in the first online survey between March and April 2020, and 156 employees responded to the second online survey between June and October 2021. The importance of barriers and communication issues was investigated. Based on a principal component analysis on non-participation, differences regarding sociodemographic variables were analyzed. RESULTS: Most employees knew about the offered measures and that the measures were cost free. There was no significant association between having communicated the offers to the employees and considering them suitable for their needs. Most of the managers or persons responsible for WHP rated the measures as sufficiently varied and allowed staff to take part during working-time. Reasons for non-participation from the managers' point of view were travel time to the location of the offers, lack of time, and a missing fit between offers and employees' needs. From the employees' point of view, workload (including working time) was the main barrier to participation. CONCLUSIONS: For the practical implementation of model projects in MSMEs, special attention should be paid to ensuring opportunity to participate, which may be easier with in-house offers.

PURPOSE: The purpose of the study was to analyze if 2 regional networks of small and medium enterprises (SME) for workplace health promotion are sustainable, and to find out the motivation of the enterprises to join the network. It was also examined if there is a stable culture of cooperation 6 -10 years after the founding of the network. Additionally, the study checked the current work and suggestions for improvement to the network structure, so that in the future, promotion of workplace health can be further improved. METHOD: 2 regional networks, founded in 2005 and 2009, were studied. Standardized telephone interviews carried out between September 2013 and January 2014 enabled data collection for this cross-sectional study. 42 interviews with 6 open questions were organized with the managers of the companies or the person responsible for workplace health promotion. RESULTS: The results of the study show that 88.1% (n=37) of the network company members profited from the exchange of experiences. 50.0% (n=21) benefited from shared activities and 28.6% (n=12) from making new contacts. 9.5% (n=4) of the respondents expressed concerns about excessive bureaucracy resulting in too much effort for too little benefit and 7.1% (n=3) were also missing comprehensive structural measures. Suggestions for improvement were enhancement of practical work (26.2%, n=11) and the wish for stronger commitment (11.9%, n=5). 90.5% (n=38) considered their expectations as fulfilled and 66.7% (n=28) evaluated the current work as being quite positive. CONCLUSION: The networks have turned out to be sustainable, proven by the fact that the companies still are members of the networks for 6 and 10 years, respectively and are still satisfied with the network. The study shows that the majority of the members profits from the membership of these regional networks. Networks can help them to implement permanent workplace health promotion. To further improve the work of the network, a systematic and scientific workplace health promotion scheme is recommended.
