RESUMO

Extranodal marginal zone lymphoma (EMZL) is a low-grade subtype of B-cell non-Hodgkin lymphoma that can affect any mucosal tissue, most commonly the GI tract. Primary involvement of the breast, known as primary breast lymphoma (PBL), is a very rare manifestation. We report an unusual case of a 65-year-old woman with primary breast EMZL presenting as calcifications discovered during screening mammography. This represents an exceedingly atypical appearance of primary breast lymphoma that is rarely described in the literature.

RESUMO

A 67-year-old female underwent a computed tomography angiogram (CTA) of the head in the setting of acute, short-term memory loss. Two lobulated hyperattenuating lesions were incidentally discovered at the base of the tongue and the hyoid bone. Upon further investigation in the outpatient setting including further imaging and ultrasound-guided biopsy, the lesions were confirmed to be ectopic thyroid tissue with dual ectopy. Heterotopic tissue, especially when arising at separate sites, can be concerning for a broad differential diagnosis including malignancy, and further evaluation is certainly recommended. When evaluating possible heterotopic tissue, one must always keep in mind the expected embryologic development of the organ in question. Further, in cases where biopsy is less favorable, consideration of the heterotopic tissue's expected physiology is equally important. With these 2 facts in mind, midline hyperattenuating, enhancing lesions in the neck must always be considered to be a possible developmental anomaly of the thyroid, even when there are multiple lesions.

RESUMO

Müllerian duct anomalies include a wide variety of developmental abnormalities involving the female reproductive system, many of which are not adequately represented by the current classification system used in the United States. Diagnosis can be made with imaging, but initial evaluation first requires a thorough physical exam. A 19-year-old female received a pelvic MRI for evaluation of a Müllerian duct anomaly following an abnormal pelvic exam. Imaging demonstrated a single uterine cavity which divides into 2 distinct cervices and vaginas. The patient received a hysteroscopic resection of her vaginal septum. This type of anomaly is extremely rare and associated clinical outcomes of potential infertility or complications with vaginal delivery are uncertain. Use of a more comprehensive classification system for Müllerian duct anomalies may assist with identification and research of such rare subtypes.

RESUMO

Teaching Point: High-pressure injection injuries are clinically significant injuries which may be underappreciated on initial physical exam and which require a high index of suspicion and early clinical intervention to avoid negative outcomes.

RESUMO

Fractures of the lesser tuberosity of the humerus are typically traumatic in nature, most commonly occurring in association with multi-part fractures of the humeral head, often in the setting of a posterior shoulder dislocation. Isolated fractures of the lesser tuberosity are considerably more rare and are difficult to diagnose on standard shoulder radiographs without an axillary view. These fractures have been associated with 3 main types of injury: acute abduction/external rotation injury, acute injury or repetitive stress injury in adolescent overhead or throwing athletes, and rarely as a seizure associated injury. The mechanism of injury in these cases has been posited to relate to the subscapularis tendon, either resisting forced abduction/external rotation in the setting of trauma, exerting chronic or acute avulsive traction in the setting of adolescent overhead or throwing athletes, or violently contracting and avulsing the lesser tuberosity in the setting of seizures. We present an unusual case of a 27-year-old male with a history of alcohol use disorder with bilateral shoulder pain after minor trauma and observed seizure-like activity. Clinical work-up revealed bilateral isolated avulsion fractures of the lesser tuberosities, which was thought to be on the basis of seizure-related violent contraction of the subscapularis muscles. The clinical relevance of this case is that a high clinical index of suspicion is needed in order to detect lesser tuberosity avulsion fractures and avoid the consequences of untreated injury such as instability or impingement.

RESUMO

Marginal zone lymphoma of mucosa-associated lymphoid tissue is a B-cell lymphoma which arises in extranodal sites, most commonly the stomach. Involvement of the breast is very rare. We present a case of an asymptomatic 63-year-old woman with synchronous mucosa-associated lymphoid tissue lymphoma involving both breasts and subcutaneous tissues. This represents a form of secondary breast lymphoma (given the concurrent involvement of the breast and non-mammary organ systems) and is an exceedingly rare diagnosis which, to our knowledge, has only been discussed in a few case series.

RESUMO

Endosalpingiosis is defined as the ectopic location of benign ciliated tubal epithelium outside of the fallopian tubes. It is a rare entity that was previously regarded as an incidental finding on pathology, and is becoming more prevalent within the medical literature. Diagnosis is made based on histologic sampling. There are no specific radiological features but commonly reported findings include numerous cystic and solid masses scattered throughout the pelvis. Common ectopic locations seen on imaging include the serosa of the uterus, fallopian tubes, ovaries and the pelvic cul-de-sac. Less common locations include the bladder wall, omentum, bowel serosa, and skin. We present the clinical presentation of a patient with histologically proven endosalpingiosis. Atypical imaging findings and correlative histology are also reviewed.

RESUMO

Babesiosis incidence in the United States has been increasing with an 11% rise between 2018 and 2019 based on the latest CDC annual summary, reaching its highest ever reported incidence. This primarily tick-borne disease is particularly prevalent in New England. Despite predominantly nonspecific and at times subtle symptoms, life-threatening complications do occur. One such complication is splenic rupture which has been suggested to be more common in younger and otherwise healthy individuals. This is a report on a successful splenic artery embolization in a 65-year-old male from upstate New York who, unlike most prior studies, showed splenic rupture after he was discharged with negative parasitemia and general improvement following several days of targeted antibiotic therapy. Increased incidence and various presentations of Babesiosis call for an attempt to promote clinical awareness for radiologists among other specialties.

RESUMO

An iterative Fourier-transform-based deconvolution method for resolution enhancement is presented. This method makes use of the a priori information that the data are real and positive. The method is robust in the presence of noise and is efficient especially for large data sets, since the fast Fourier transform can be employed.
