RESUMO

The authors report a typical case of osteopoikilosis striata Voorhoeve in a 51-years-old female. In connection with case they make some consideration concerning the diagnostic of this very infrequent sclerotic bone dysplasia.

RESUMO

Starting from the rareness of vertical fractures of the patella, in connection with a clinical case, the authors present this entity and the therapeutical problems in correction with this type of fracture. Interfragmentary diastasis is more than 3 mm, so the authors choose for treatment the osteosynthesis with a spongious screw placed horizontally, perpendicular on the fracture, keeping the integrity of the prepatellar fibrous planes. The authors appreciate that orthopedic treatment is reserved to the incomplete fractures or fractures without displacement.

RESUMO

In connection with 23 different patellar instabilities, the author makes some diagnostical and treatment consideration. The patients were between 18 and 40 years old, 13 women and 10 men. From the clinical point of view there were found 2 simple instabilities, 3 external dislocations, 6 permanent dislocations, 9 relapsed dislocations and 3 of them were habitual. The results in first time were good and excellent in 90 percent. It was a single relapse and by 4 of the patients which have been in watch for five years it was found arthrosis without major functional signs.

RESUMO

The location of the mega-cell tumours in the inferior extremity of radius is relatively frequent and it represents 10 percent from all mega-cell tumours. The aggressive tumours and the relapsed one must be cut-away. In 3 of the cases which were operated, after a large cutting of the tumour, the author used for reconstruction a grafting from the fibula-bone without vascularisation. The fixing of the focus metallic plates is highly superior to other osteosynthesis. After 3 years, now relapse was remarked. The functional results can be considered good-enough, even some of patients can not perform hard-working.

RESUMO

Based on their own experience and literature, the authors review the etiopathogenic, anatomo-clinical, diagnostic and management aspects of torso-metatarsal dislocations. The surgical treatment by open reduction or reposition-arthrodesis, as a first or secondary step, using Kirschner wire and staples, are more and more used. The early and late complications, which may aggravate the long-term functional prognosis are detailed.

RESUMO

By analysing the various surgical procedures used in the management of scapula alata, the author describes a personal technique where scapula is fixed to the 7th rib with the shot palmar tendon. For the stabilisation of the scapula, its inferior angle introduced under the great dorsal is sutured with strong catgut, the needle entering from under the depth of muscle and emerging on its posterior aspect where catgut ends are knotted.

RESUMO

The authors present the substantiation of the use of conservative treatment in a series of 163 patients with coxarthrosis operated upon between 1983 and 1990. In 135 of these patients various types of osteotomies were performed: Pauwels valgisation or varus femoral osteotomy (42 cases), McMurray displacement osteotomy (41 cases), varus and displacement osteotomy (19 cases) and Chiari pelvic osteotomy (33 cases). In the remainder of 28 patients tenotomies and hip denervations were performed. The postoperative results, estimated by a strict clinico-radiological survey revealed a marked clinical improvement and radiographically a restoration of joint congruence in about 80% of the cases.

By analysing 32 patients with nontraumatic aseptic necrosis of femoral head, operated upon between 1 January, 1980 and 31 December, 1988, the author underlines the importance and at the same time the difficulties of an early diagnosis and the concordance which should exist between the therapeutical indications and the evolutive stage of the lesions and patients' age.

RESUMO

In the interval 1962-1987 the author had treated 41 cases of giant cell bone tumors (25 females and 16 males). The 20-40 age group was prevalently affected (32 cases). The upper epiphysis of tibia and lower epiphysis of femur were the most common sites (13 and 9 cases, respectively). The clinical, x-ray and especially the histological data decided the type of intervention.

RESUMO

Based on the experience gained in the treatment of chronic anterior knee dislocations, the author presents a new procedure of extra-articular syndesmoplasty. In the 34 cases operated by this technique of whom 30 reexamined after a 6 months-2 years interval, 23 very good, 6 good and 1 satisfactory results were obtained.
