BACKGROUND: Neck pain is common among individuals with migraine, but there is a lack of information of how this comorbidity can be associated with cervical muscle function. This controlled cross-sectional study aimed to compare cervical muscle function, activity, and sensitization in women with migraine, neck pain, both, and neither. METHODS: This study included women, between 18 and 55 years old, with either episodic migraine with or without aura, without any concomitant headache diagnosis; chronic neck pain, with at least moderate intensity and mild disability; or neither headache nor neck pain. Pain pressure threshold, allodynia, muscle strength, and endurance and cervical muscles activity were evaluated. RESULTS: One hundred subjects, with mean age of 30.4 years old, were stratified by diagnosis (n = 25 per group) and by self-reported pain during tests. Lower endurance during flexion was observed for migraine and neck pain (34s) relative to neck pain alone (45s), migraine (40s), and controls (58s) (p = 0.04). For extensor endurance, means were 142s, 166s, 215s, and 270s, respectively (p < 0.001). Endurance times were impacted by the presence of test-induced pain decreasing about 40%-53% of the performance. Diagnostic groups did not differ significantly in strength (p > 0.05), but all pain groups presented significantly higher proportion of test-induced pain, lower muscle activity during the maximal isometric voluntary contractions, and lower pressure pain thresholds. CONCLUSION: Patients with migraine, chronic neck pain, and the association of both present altered cervical muscle function and activity. Also, test-induced pain impacts significantly on neck muscles endurance. SIGNIFICANCE: The diagnosis of migraine and chronic neck pain is associated with altered function and activity of the cervical muscles. However, the test-induced pain had an important contribution to worse cervical muscle endurance. This suggests that the therapeutic approach should focus on de-sensitization of the trigeminal-cervical complex when dealing with the comorbidity of migraine and cervical pain.

Sequential anaerobic and aerobic processes have been recommended to treat textile wastewater reliably. In this work, the focus was on finding an energetically more competitive system to remove tetra-azo dye Direct Black 22 (DB22). We operated two upflow anaerobic sludge blanket (UASB) reactors (R1 and R2) in three phases (PI, PII, and PIII). R1 was operated as a conventional UASB, while R2 was microaerated in the upper part (0.18 ±â€¯0.05 mg O2. L-1), aiming to remove DB22 simultaneously with the aromatic amine byproducts. PI consisted of feeding reactors with synthetic textile wastewater (STW), PII had higher salinity in the STW, and PIII was the same as PII, plus sulfate. The results showed that color and COD removal efficiencies were similar for both reactors (67-72% for R1 and 59-78% for R2) without a substantial influence of oxygen in R2. However, microaeration played a crucial role in R2 by removing the anaerobically formed aromatic amines; during PIII, the effluent was 16 times less toxic than that of R1. The microbial community that developed in the sludge bed of both reactors was quite similar, with the core microbiome represented by Trichococcus, Syntrophus and Methanosaeta genera. The increase in salinity in PII and PIII promoted a shift in the microbial community, excluding salty-sensitive genera from the core microbiome. The putative genera Brevundimonas and Ornatilinea were associated to aromatic amine microaerobic removal. Therefore, there is a potential application of a compact microaerated anaerobic system for textile wastewater treatment.

The 1,3-propanediol (1,3-PDO) yield and productivity from glycerol were studied over a 155-day period. A UASB reactor that also contained silicone support for biomass attachment was used to evaluate the optimal operational conditions and microbiota development. The highest average 1,3-PDO yield was 0.54 and 0.48 mol.mol-gly-1 when reactor pH was 5.0-5.5 and the applied loading rate was 18 and 20 g-gly.L-1.d-1 using the pure and crude substrate, respectively. The productivity was close to 7.5 g.L-1.d-1 for both substrates; therefore, the direct use of crude glycerol can be valorized in practice. Clostridium was the predominant genus for 1,3-PDO production and C. pasteurianum was dominant in the biofilm. Using crude glycerol, C. beijerinckii dropped strongly; some Clostridium population was then replaced by Klebsiella pneumoniae and Lactobacillus spp. The good process performance and the advances in the microbiota knowledge are steps forward to obtain a more cost-effective system in practice.

OBJECTIVE: To assess the prevalence of falls and fear of falling in patients with migraine compared with controls. DESIGN: Cross-sectional. SETTING: Tertiary headache clinic. PARTICIPANTS: This study consisted of 105 controls and 105 consecutive patients diagnosed with migraine with aura (MA, n=35), migraine without aura (MO, n=35) and chronic migraine (CM, n=35). MAIN OUTCOME MEASURES: Patients were interviewed using a questionnaire containing questions about the history of falls and impairment in balance, and completed the International Falls Efficacy Scale (FES-I). Groups were contrasted using Student's t-test and analysis of variance, and prevalence ratios were estimated. RESULTS: Falls and self-reported impairment in balance are more prevalent in patients with migraine (54% and 69%, respectively) than in controls (2% and 2%, respectively). In particular, patients with CM and MA reported a greater mean number of falls during the previous year {CM 1.4 [standard deviation (SD) 2.2]; MA 2.2 (SD 2.3)} compared with patients with MO [0.5 (SD 1.0); P <0.04] and controls [0.05 (SD 0.2); P <0.002]. The prevalence ratio of falls was greater in patients with MA (7.2; P <0.002) and CM (4.5; P <0.002) compared with controls. Patients with migraine experienced a high level of concern about falls during their daily activities compared with controls (29.8 vs 20.1 points in the FES-I questionnaire; P<0.0001). CONCLUSIONS: The balance impairment of patients with migraine may have a functional impact. Migraine is associated with risk of falling, and patients exhibit a higher prevalence of impairment in balance, falls and fear of falling.

In the present study, we evaluate the behavior of real textile wastewater treatment using a system composed of two sequential pilot-scale reactors (anaerobic followed by aerobic) during 622 days. The work focused on the competition between color and sulfate removal processes, when the hydraulic retention time (HRT) was increased in the anaerobic/aerobic reactors from 16/12 hours in phase I (PI) to 4/3 days in phase II (PII). The organic matter was successfully removed in both phases through the system, and the highest efficiency (75%) was achieved in PII. The increase in the HRT did not improve azo dye degradation under anaerobic conditions. Instead, it favored sulfate reduction, which removal efficiency increased from 26% in PI to 75% in PII. Aromatic amines were detected in the anaerobic reactor effluent and removed in the aerobic reactor.

Introducción: En las 2 últimas décadas la ventilación mecánica no invasiva (VNI) se ha consolidado como estrategia inicial para el abordaje de la insuficiencia respiratoria en el paciente crítico adulto y pediátrico. Objetivos: Identificar los factores de riesgo y estrategias preventivas para disminuir la incidencia de lesiones cutáneas asociadas a dispositivos clínicos (LESADIC) relacionadas con la VNI, así como el tratamiento más eficaz para las lesiones que no se han podido evitar. Metodología: Revisión en las bases de datos MEDLINE, CINAHL y Cochrane de estudios publicados en los últimos 10años para llegar al consenso mediante panel de expertos. Resultados: Los conocimientos acerca de cómo medir la talla correcta de máscara y la protección de la piel con apósitos de espuma o hidrocoloides son factores relacionados con la incidencia de LESADIC, pues condiciona el grado de presión-fricción y cizalla que la interfaz ejerce sobre la piel. La interfaz que menos LESADIC causa y es mejor tolerada es la máscara facial. Cuando hay lesiones, lo primero es retirar la interfaz que provoca presión sobre la piel dañada, recomendando como alternativa el casco Helmet(R), tratar la infección, gestionar el exudado y estimular piel perilesional. Conclusiones: La máscara de elección es la facial, siempre con uso de apósitos de espuma o hidrocoloides en el puente nasal. Evaluar el estado de la piel debajo la interfaz y arnés entre las 4 h (recomendable) y 11 h (máximo). Valorar la estrategia de rotación de la interfase a las 24 h si la VNI sigue siendo necesaria de forma continuada

When there are injuries, the first thing is to remove the interface that causes pressure on damaged skin, recommending a Helmet(R) hood as an alternative, treating the infection, managing the exudate and stimulating perilesional skin. Conclusions: The mask of choice is the facial, always using foam or hydrocolloid dressings on the nasal bridge. Evaluate the condition of the skin under the interface and harness every 4 hours (recommended) and 11 hours (maximum). Evaluate the rotation strategy of the interface at 24 hours if the NIV is still needed on an ongoing basis

BACKGROUND: Many studies have demonstrated the presence of somatosensory modulation changes at different sites in patients with temporomandibular disorders (TMDs) using different modalities. However, the neck area, a well-know condition related to TMD, remains unexplored. OBJECTIVE: To assess the thermal pain threshold in patients with TMD and controls at cephalic and extra-cephalic areas, including the neck. METHODS: Twenty female patients with TMDs diagnosed by the Research Diagnostic Criteria for TMD (RDC/TMD) and twenty age-matched controls underwent a first interview about neck pain and disability (NDI questionnaire). A blinded evaluator assessed the thermal pain threshold for cold (CPT) and heat (HPT) stimuli in accordance with an ascending method of limits of the Quantitative Sensory Testing at the following sites: periorbital, masseter, cervical posterior and ventral forearm. The groups were compared using a t-test with α = 5%. RESULTS: Patients with TMDs reported pain at higher temperature for cold stimuli in all sites (P < 0·05) and at lower temperature for heat stimuli in the right periorbital site (P < 0·05) than controls. Pain and disability due tothis symptom were reported more often in the TMD group (P < 0·05). CONCLUSION: Patients with TMD have pain modulation changes in the neck area as well, especially for cold stimuli, associated with higher disability and a higher report of neck pain than controls. These findings reinforce the evidence regarding the relationship between TMDs and neck pain.

In northeastern Brazil, stabilization ponds are very suitable for wastewater treatment because of the relative great land availability and environmental conditions (e.g., high temperature) favorable for microorganism optimal development. However, blooms of potentially toxic cyanobacteria may affect the use of these treatment ponds due to resulting effluent poor quality. The objective of this study was to evaluate the dynamics of phytoplankton communities and the occurrence of cyanobacteria in a maturation pond located immediately after a series of two ponds. Temperature, dissolved oxygen, pH, BOD, N, and P were measured during a period of four months when samples were collected from the surface and the bottom of 7 sampling points distributed inside the pond. The phytoplankton of collected samples was also identified and classified using a conventional optical microscopy. Analysis of variance and Tukey test were used to evaluate the results. The three phytoplankton divisions found (Cyanophyta, Chlorophyta, and Euglenophyta) did not change considerably through surface and bottom. However, they changed greatly over the sampled months; great dominance of Cyanophyta was found at April and October, while Chlorophyta dominated the lagoon in September. Low superficial organic loads (between 78 and 109 kg BOD.ha-1.d-1) and N:P ≤ 10 were the determinant factors that favored the predominance of Cyanophyta. The presence of two potentially toxic species of Cyanophyta, Oscillatoria sp. and Microcystis aeruginosa, indicates that caution is required when considering the final destination of treated effluent and suggests a need to assess the risks and benefits associated with the use of the treatment technology.

Resumo No nordeste do Brasil, as lagoas de estabilização são muito adequadas para o tratamento de águas residuárias por causa da disponibilidade relativamente grande de terra e das condições ambientais (por exemplo, altas temperaturas) favoráveis ao melhor desenvolvimento dos microorganismos. Entretanto, florações de cianobactérias potencialmente tóxicas podem afetar o uso dessas lagoas de tratamento, devido à consequente qualidade inferior do efluente. O objetivo deste estudo foi avaliar a dinâmica das comunidades de fitoplâncton e a ocorrência de cianobactérias em uma lagoa de maturação situada após duas lagoas em série. Temperatura, oxigênio dissolvido, pH, DBO, N e P foram medidos durante um período de quatro meses, durante o qual amostras foram coletadas na superfície e fundo em sete pontos de amostragem da lagoa. As comunidades de fitoplâncton das amostras coletadas foram também identificadas e classificadas utilizando-se um microscópio óptico convencional. Para avaliar os resultados utilizou-se a análise de variância e o teste de Tukey. Para as três divisões de fitoplâncton encontradas (Cyanophyta, Chlorophyta e Euglenophyta), não houve diferença significativa para as amostras de superfície e de fundo de um mesmo mês. Entretanto, ocorreu grande variação para as amostras dos diferentes meses; nos meses de abril e outubro houve uma predominância de Cyanophyta, ao passo que em setembro o predomínio na lagoa foi de Chlorophyta. Os fatores determinantes que favoreceram o predomínio de Cyanophyta foram a baixa carga orgânica superficial aplicada (entre 78 e 109 kg DBO.ha–1.d–1) e N:P ≤ 10. A presença de duas das espécies de Cyanophyta, Oscillatoria sp. e Microcystisaeruginosa, consideradas potencialmente tóxicas, indica que é necessária precaução quando se considera o destino final do efluente tratado e sugere a necessidade de avaliar os riscos e benefícios associados ao uso da tecnologia de tratamento.

In this study, we evaluated the use of an up-flow anaerobic sludge blanket (UASB) reactor to treat crude glycerol obtained from cottonseed biodiesel production. The laboratory-scale UASB reactor (7.0 L) was operated at ambient temperature of 26.5°C with chemical oxygen demand (COD) concentrations between 0.5 and 8.0 g/L. The volatile fatty acid contents, pH, inorganic salt contents and biogas production were monitored during a 280-day experimental period. Molecular biology techniques were used to assess the microbial diversity in the bioreactor. The reactor achieved COD removal efficiencies of up to 92% except during one phase when the efficiency decreased to 81%. Biogas production remained stable throughout the experimental period, when the fraction converted to methane reached values as high as 68%. The profile of the denaturing gradient gel electrophoresis (DGGE) bands suggested slight changes in the microbial community during reactor operation. The overall results indicated that the crude glycerol from biodiesel production can serve as a suitable substrate for anaerobic degradation with a stable reactor performance and biogas production as long as the applied organic loads are up to 8.06 kg COD/m3·d.

An upflow anaerobic sludge blanket (UASB)-submerged aerated biofilter (SAB) system was evaluated to remove color and chemical oxygen demand (COD) from real textile effluent. The system was operated for 335 days in three phases (P-1, P-2, P-3) with total hydraulic retention time varying from 21 h to 14 h. The results showed that high sulfate levels (>300 mg SO4(2-)/L) impaired the dye reduction. The best color removal efficiencies of 30% and 96% for the UASB and the reactor system, respectively, were obtained in P-1; the SAB higher efficiency was associated with adsorption. The best COD removal efficiency of 71% for the reactor system was obtained in P-2. Precipitation of some material composed mostly of sulfur (98%) and some metals occurred in the UASB. However, the precipitated sulfur was again oxidized in the SAB. The system also showed an effective toxicity reduction in tests (Daphnia magna) with the treated effluent.

An upflow anaerobic sludge bed (UASB)-submerged aerated biofilter (SAB) system that treats effluents from a jeans factory was evaluated. The 210-day operational period was divided into three phases (PI, PII and PIII), each with a different hydraulic retention time (HRT in h) and organic loading rate (OLR in kg COD/m3.d). In PI, the best performance was achieved using the UASB (HRT 24, OLR 1.3) with COD and color removal efficiencies of 59 and 64%, respectively; the corresponding values were 77 and 86% for the final effluent. In PII, the efficiencies were 50 and 55% using the UASB (HRT 16, OLR 1.2), respectively, and 69 and 81% for the final system effluent, respectively. In PIII, the UASB (HRT 12 and ORL 3.2) showed the poorest performance; the efficiencies decreased to 48 and 50%, respectively. The same phenomenon occurred in the system with corresponding efficiencies decreasing to 69 and 61%. Throughout the experiment, the system removal efficiencies were between 57 and 88% for nitrogen and between 14 and 63% for sulfate. The final effluent showed relatively non-toxicity or moderate toxicity using Daphnia magna as an indicator. Therefore, the overall results showed that the use of a sequential anaerobic-aerobic system is promising for treatment of textile industrial wastewater.

The performance of an EGSB (expanded granular sludge bed) reactor type using flocculent sludge was evaluated for the post-treatment of effluent from UASB reactor treating domestic sewage. A pilot-scale 157.5-L EGSB reactor was monitored during a 331-day period. The original concept of the EGSB reactor was based on granular sludge use and by applying of high upflow liquid velocities (Vup). However, even using flocculent sludge from a UASB full-scale (Mangueira Plant, Recife, Brazil), good mixing conditions and high retention of biomass were achieved. By applying a 4-h hydraulic retention time and Vup values up to 3.75 m/h, effluent chemical oxygen demand concentrations in the EGSB were below 87 mg/L and 55 mg/L, for total and filtered samples, respectively. Total suspended solid (TSS) concentrations in the effluent were below 32 mg/L. Good performance and stable reactor operation during the whole experimental period were observed. Therefore, the EGSB reactor type using flocculent sludge can also be used for post-treatment of very dilute anaerobic effluent from reactors treating domestic sewage.

The anaerobic technology application for domestic sewage treatment in Pernambuco State (Brazil) is relatively recent. Some UASB reactors of less than 250 m3 were built in the Recife Metropolitan Region (RMR) in the 1990s. Mangueira (18,000 inhabitants) was the first neighborhood where the municipality built a plant with a bigger UASB reactor of 810 m3. It was intended to evaluate the performance and verify if such technology would be feasible. The objective would be the possible application of UASB reactors to the RMR, according to the new sewage master-plan under elaboration that would benefit about 3 million inhabitants. The monitoring of the Mangueira UASB reactor over 30 months showed that satisfactory results were obtained. Three distinct operational phases occurred, in which efficiency varied from 60% up to 90% based on COD removal. The results were very dependent on the operation and maintenance, either in the plant or in the sewage collection. Significant amount of inert solids was measured inside the reactor. Despite the operational problems, the UASB was shown to be very robust and stable. Under high fluctuation of influent concentrations (150-750 mg COD/L) during the period, resulting in applied organic loading rate of 0.5 to 2.5 kg COD/m3.d, the average values of COD removal efficiency did not change significantly. An active biomass with specific methanogenic activity varying from 0.18 to 0.25 g COD/g VSS.d was measured at the end of the period.

RESUMO

The Lagoon Olho d'Agua in Pernambuco State, Northeast Brazil has received increasing environmental concern due to significant stress from pollution in the catchment. The existing environmental problems are the result of great pressure from a broad range of human activities, especially in the last 10 years. Serious pollution exists mainly from some industrial and urban activities, which increased intensively after the eighties. There is a strong social and economical pressure for housing and construction near the lagoon, due to the available land nearby beaches and estuarine zone, and recently by growing tourism activities. Uncontrolled land use by low-income communities and the pressure for construction by developers have led to landfilling and to deterioration of water quality in the lagoon catchment. Improvement of the environmental conditions in the catchment needs integrated measures. Guidelines and some specific actions involving several institutions have been established and refer to sanitation and urban infrastructure as the main priorities. A main target is the construction of low-cost sewage system with smaller and decentralised treatment plants.

Methanol is an important anaerobic substrate in industrial wastewater treatment and the natural environment. Previous studies indicate that cobalt greatly stimulates methane formation during anaerobic treatment of methanolic wastewaters. To evaluate the effect of cobalt in a mixed culture, a sludge with low background levels of cobalt was cultivated in an upflow anaerobic sludge blanket reactor. Specific inhibitors in batch assays were then utilized to study the effect of cobalt on the growth rate and activity of different microorganisms involved in the anaerobic degradation of methanol. Only methylotrophic methanogens and acetogens were stimulated by cobalt additions, while the other trophic groups utilizing downstream intermediates, H2-CO2 or acetate, were largely unaffected. The optimal concentration of cobalt for the growth and activity of methanol-utilizing methanogens and acetogens was 0.05 mg liter-1. The higher requirement of cobalt is presumably due to the previously reported production of unique corrinoid-containing enzymes (or coenzymes) by direct utilizers of methanol. This distinctly high requirement of cobalt by methylotrophs should be considered during methanolic wastewater treatment. Methylotroph methanogens presented a 60-fold-higher affinity for methanol than acetogens. This result in combination with the fact that acetogens grow slightly faster than methanogens under optimal cobalt conditions indicates that acetogens can outcompete methanogens only when reactor methanol and cobalt concentrations are high, provided enough inorganic carbon is available.
