RESUMO

A clinical analysis of a patient with a rare form of secondary facial pain (Herzenberg's disease) is presented. The conducted research and pharmacological tests made it possible to exclude such diseases as trigeminal neuralgia, stylohyoid syndrome, TMJ pain dysfunction, dental plexalgia, myofascial facial pain syndrome. The patient is consulted by a dental surgeon to exclude pathology of the parotid salivary gland and sialolithiasis. The patient was treated with broad-spectrum antibiotics for 8 days. Also, supportive therapy was prescribed: desensitizing, vitamins, as well as an anxiolytic and an antidepressant in minimum therapeutic doses for a course of 14 days (with subsequent correction of the dosage and duration of administration) to correct the emotional state of the patient. We recommend a gentle diet (alkaline warm drink, soft food), and a diet that does not provoke salivation. Locally it was recommended to rinse with antimicrobial drugs, phonophoresis, electrophoresis, magnetotherapy. On the 6th day of therapy, the therapeutic effect was obtained. By the 14th day of therapy, the pain syndrome was leveled.

RESUMO

A case-report of burning mouth syndrome is presented. A 27-year-old man complained of burning pain in the tongue and oral mucosa, taste disorder, and sensory impairment. All symptoms appeared after suffering a cold and had a wave-like course during self-medication with antibiotics. The pain has continued for 8 months. Diagnoses of atypical facial pain, glossodynia or secondary facial pain (craniomandibular dysfunction) were made. The effect of treatment in the hospital (carbamazepine, amitriptyline, haloperidol, phenozepam) was not achieved. A microbial test showed a higher number of pathogenic microbes. The final diagnosis was secondary facial pain (burning mouth syndrome) with concomitant lesions of the oral mucosa (Staphylococcus aureus, Candida sp.). The patient received a combined therapy with the pronounced positive effect.

Authors present a case-report of a 36-year-old patient with pain syndrome in the face region (craniomandibular dysfunction with occlusive disorders) involving pericranial muscles and shoulder girdle muscles. Thioctic acid was prescribed as a basic therapy, and relaxation tire, mimic gymnastics, post-isometric relaxation, vitamin and mineral complex were additionally used. Thioctic acid was prescribed according to the following scheme: stage I - intravenous injections of thioctic acid (600 mg/daily, 5 procedures, every other day); stage II - oral administration of the drug in the dose of 600 mg/daily for 3 months. A significant reduction of pain syndrome and the palpable tension of muscles were noted; the symmetric even muscle tone was revealed. This case-report demonstrates the high efficacy of thioctic acid in treatment of patients with craniomandibular disorders using the 2-stage scheme. There was a significant improvement of quality of life of the patients due to the additional use of vitamin and mineral complex and medical tools (relaxation tire) for treatment at all stages of pain formation.

OBJECTIVE: To study the efficacy and safety of tenoten in the preventive treatment of frequent episodic tension-type headache (FETHA) compared to patients treated with pain relievers. MATERIAL AND METHODS: A study included 60 patients with FETHA. Patients of the main group (n=30) received tenoten in addition to standard treatment. The study comprised 3 visits: beginning of treatment, after one month and after three months. All patients underwent physical and clinical/neurological examinations. In each visit, treatment efficacy was assessed according the following parameters: VAS scores (0-10) for assessment of pain and tension in pericranial muscles in 6 standard points, mean frequency and duration of the headache episode, quality of life indices, Beck depression scores, Spilberger trait and state anxiety, autonomic symptom severity, parameters of sleep disorders, frequency of adverse effects, CGI scores (0-7). RESULTS AND CONCLUSION: Tenoten as a preventive medication reduced the frequency of headache episodes that allowed to diagnose patients with rare episodic tension-type headache in the end of treatment. At the same time, there was a significant reduction in headache intensity during the episode and decrease in amount of analgesics used by the patients.

The investigation had the purpose to measure energy substrates in different fibers of 3- and 14-d suspended postural m. soleus and its main antagonist - anterior tibial muscle in rats. Hindlimb suspension served as a model of gravitational unloading. After suspension, left m. soleus or anterior tibial muscle was extracted. Serial cross-sections were subject to histochemical staining by Oil Red O (Sigma) for determination of intracellular triglycerides or by Schiff's reagent and iododic acid (Sigma-ALDRICH) for determination of glycogen and simultaneous immunohistochemical fibers typing by antibodies to heavy types I and II myosin chains (NCL-MHCs and NCL-MHCf, Novocastra). Analysis of intracellular energy substrates showed that both types of fibers in the anterior tibial muscle had accumulated triglycerides by week-2 of suspension. At the same time, glycogen level in equally slow and fast fibers of m. soleus and fast fibers anterior tibial muscle decreased substantially after 3 days of suspension but returned to baseline value by day 14. These changes are likely to be linked with altered activity of the muscles and hormonal factor.

The study was carried out in 28 clinical centers of Russia. The data on 728 patients with psychoautonomic syndrome (91.6% of the total sample) were available for statistical analysis. All patients suffered from asthenic disorders. The duration of treatment with ladasten in daily dose from 50 to 100 mg was 28 days. Patient's state was assessed with psychometric scales at baseline, 3, 7, 14 and 28 days of therapy and one month after the end of treatment. The percentage of responders was 76.0% on the CGI-S and 90.8% on the CGI-I. The antiasthenic effect of ladasten was seen on day 3 and remained during one month after the withdrawal of therapy. We determined clinical efficacy of ladasten in regard to anxiety-depressive spectrum disorders, autonomic dystonia, sleep disorders. Ladasten therapy led to the significant increase of quality of life, which was seen not only after the end of therapy, but after the withdrawal of the drug. These results suggest the stability of the therapeutic effect achieved. Adverse effects were observed only in 3% of patients, the therapy was discontinued in 0.8%. No serious adverse effects were found. In conclusion, the efficacy of ladasten was shown in its antiasthenic, anxyolytic, autonomic nervous system stabilizing properties, the normalization of sleep-awake cycle and the increase of quality of life. Ladasten in daily dose from 50 to 100 mg is a highly effective, well-tolerated and safety drug with a wide spectrum of clinical effects. Therefore, this drug could be recommended for treatment of asthenic disorders in neurological practice.

The aim of this study was to elucidate the effects of 14- day hindlimb suspension (HS) and subsqquent reloading (3 or 7 days) on the m. soleus mass, muscle fiber cross-sectional area (CSA), soleus fiber properties and serum IGF-1 in rats. Rats were hindlimb suspended for 14 days or kept as controls (C, n = 7). Soleus muscles were isolated after HS (HS, n = 7) or after reambulation for either three (R3, n = 5) or seven days (R7, n = 6). Frozen serial sections of m. soleus were stained by primary monoclonal antibodies against MHCI. For measurement of concentration IGF-1 in the blood serum, test-system for IFA DSL-10-2800 Non-Extraction IGF-1 ELISA was used. Muscle mass was significantly reduced in HS (-35 %) but subsequently increased with reloading in R3 (-10 % to C) and was recovered to control values in R7 (+5 % to C). Fiber CSA was significantly reduced (-43 %) in HS and was greater in R7 than in HS and slightly greater than in R3. 14 days of HS resulted in a mean maximal tension reduced by 35 %. After 7 days of subsequent reloading the mean maximal specific tension was still low (-33 % to C) and didn't differ from HS level. The level in blood IGF-1 has obviously decreased during 14-day unloading by 48 %, remained at the same level in R3, and increased 10 fold in R7.

We supposed that the triglyceride content might be used as a cellular marker of muscle fiber activity and for the first time analyzed the triglyceride content in the fast- and slow-twitch muscle fibers of m. soleus and m. tibialis anterior under conditions of 7-day rat hindlimb suspension. Although the hindlimb suspension led to decrease of triglyceride content in the fast-twitch fibers of m. soleus and slow-twitch fibers of m. tibialis anterior, these differences were not significant. In spite of this the obtained data do not contradict our initial hypothesis.

RESUMO

The study presents a clinical analysis of a large population of migraine patients (n = 320). Computed database used in the study included over 100 clinical parameters and was based on analysis of primary medical records. As a result of statistic processing of the migraine archive, a general review of clinical picture and migraine course in patients from Moscow and Moscow region community and analysis of the peculiarities of migraine heredity, concomitant symptoms, provoking factors, co-morbid diseases, state in attack-free period, psychological characteristics and quality of life are provided. Factors determining course severity and quality of life in migraine are discussed.

RESUMO

Clinical-psychophysiological characteristics of tension-type headache are analyzed. Comparison of some revealed peculiarities with both the form of tension-type headache (episodic, chronic) and manifestation of anxious or depressive disorders as well as with the duration of ache syndrome was performed. The problems concerning diagnosis of tension-type headache and factors contributing to its chronization are discussed.

A study was made of the psychoendocrine factor of pathogenesis of patients suffering from cerebral obesity with a hyperphagic reaction to stress and without it. The purpose was to study the hypothalamo-hypophyseo-adrenal system, emotional-personality features and motivation sphere in patients with cerebral obesity as compared to a control group. The following methods of examination were used: cliniconeurological examination, psychological methods--MIL test, Spilberger's test, a radioimmunoassay for determination of the blood levels of ACTH, STH, prolaction and cortisol at rest and under experimentally simulated stress. These methods were followed by mathematical processing of the obtained data with a correlation analysis of psychological and hormonal indices. The conclusions were based on statistically significant results only. A variable psychohormonal complex of reaction to stress was revealed in the patients with relation to the presence or absence of a hyperphagic reaction to stress. In a hyperphagic reaction to stress an increase in the body mass under an emotional stress resulted mainly from inadequate psychomotivation shifts with further overeating. In the absence of such a reaction a body mass increase under a stress was associated mainly with the neuroendocrine factor with retardation of lipolysis.
