RESUMO

As ondas de calor relatadas no climatério apresentam alta prevalência e afetam de forma significativa a qualidade de vida das mulheres acometidas. O climatério pode cursar com obesidade, hipertensao, diabetes mellitus tipo II e outros fatores de risco cardiovascular, que, em conjunto, compõem a síndrome metabólica, e que pode estar relacionada à maior ocorrência de ondas de calor. O presente estudo tem por objetivo estimar a prevalência de ondas de calor, bem como verificar sua relacao com a síndrome metabólica, em mulheres no climatério residentes em Pindamonhangaba, SP. Assim, em estudo transversal realizado à partir de um banco de dados de um projeto denominado PROSAPIN (PROjeto de SAúde de PINdamonhangaba) foram analisadas 900 mulheres de 35 a 65 anos - período conceituado como climatério (Notelovitz 1988). Todas as participantes foram avaliadas por meio da escala de Kupperman para quantificar os sintomas climatéricos e pelos critérios do ATPIII, para firmar o diagnóstico de síndrome metabólica. A prevalência de ondas de calor foi de 53,89% (IC95%: 50,62% - 57,15%). Por meio de um modelo de Regressao de Poisson múltipla, ainda se constatou nítida associacao da presenca de ondas de calor com síndrome metabólica, insonia, depressao, estresse e raca negra e parda. Em conclusao, as ondas de calor apresentaram alta prevalência na populacao estudada e se associaram a distúrbios emocionais, à raca e à síndrome metabólica


Vasomotor symptoms or hot flushes are very prevalent in the climacteric period and significantly worsen the quality of life of the women affected. Climacteric can occur with obesity, hypertension, type II diabetes mellitus and other cardiovascular risk factors, which, together, represents the metabolic syndrome, that appear to be related to a higher occurrence of hot flushes. The present study aims to estimate the prevalence of hot flashes, as well as to verify its relationship with the metabolic syndrome, among climacteric women resident in Pindamonhangaba, SP. Thus, a cross-sectional study from a database of a project called PROSAPIN (PINdamonhangaba`s Health Project) analyzed 900 women from 35 to 65 years-old - a period known as climacteric (Notelovitz 1988). All participants were evaluated through the Kupperman scale to quantify the climacteric symptoms and through the ATPIII criteria, to establish the diagnosis of metabolic syndrome. The prevalence of hot flushes was 53,89% (CI95%: 50,62% - 57,15%), and, by a multiple Poisson regression model, it was observed a clear association between the presence of hot flushes and metabolic syndrome, insomnia, depression, anxiety and afrodescendence. In conclusion, the hot flushes presented high prevalence in the study population and were associated with emotional disorders, ethnicity and to the metabolic syndrome

INTRODUCTION: Polycystic ovary syndrome (PCOS) is a prevalent disease affecting women of reproductive age. It may be associated with metabolic disorders such as insulin resistance (IR) and obesity. The anti-Mullerian hormone (AMH) levels seem to be higher in patients with PCOS. OBJECTIVE: The present study was designed to evaluate the association between AMH and insulin in women with PCOS with and without IR. METHODS: Cross-sectional study, including 86 patients, selected and divided into three groups: Group A: 26 women with PCOS and IR; Group B: 30 women with PCOS and without IR; and Group C: 30 controls without PCOS. RESULTS: We found significant difference between serum AMH levels in the group of women with PCOS and without IR when compared to the control group, thus showing that PCOS and IR play an important role in elevating the levels of this hormone. When the groups were compared with each other following adjustment for BMI, serum AMH levels were significantly higher in the group of women with PCOS and IR. CONCLUSION: AMH levels are significantly higher in patients with PCOS, particularly in those women with PCOS and IR. Nevertheless, larger samples are required to confirm these findings.

The evidence suggest that androgen deficiency in women induces sexual dysfunction as its main clinical manifestation, in particular reduced libido. However, other factors may be involved in the genesis of this disease, such as interpersonal relationships, social stressors, physical inactivity and the partner. The prevalence of sexual problems among women ranges from 9 to 43% and, recently, many studies have reported that androgens are beneficial not only for women's sexual function, but also for mood disorders and vasomotor symptoms. This is why physicians should include androgen deficiency syndrome in their differential diagnosis, even in women with adequate estrogen levels. Our objective was to present the practical aspects of this disease, emphasizing diagnosis and focusing on treatment. This review searched PUBMED for publications from the last 51 years, up to May 2010, including consensus statements and expert opinions and identified 105 articles. We concluded that the androgen deficiency syndrome in women is overlooked in clinical practice. There is no consensus in the literature regarding diagnosis or treatment, including choice of drug, route of administration and time of application.

As evidências sugerem que a deficiência androgênica na mulher exibe como principal manifestação clínica a disfunção sexual, especialmente a queda da libido. Entretanto, outros fatores podem também estar implicados na gênese da disfunção sexual, como o relacionamento interpessoal, os estressores sociais, o sedentarismo e o próprio fator masculino. A prevalência da disfunção sexual feminina oscila entre 9 por cento e 43 por cento e recentemente muitos estudos têm mostrado que a reposição com androgênios não só melhora o desempenho sexual, mas também os distúrbios do humor e sintomas vasomotores. Por isso, o profissional de saúde deve sempre incluir no diagnóstico diferencial da disfunção sexual a Síndrome de Deficiência Androgênica, mesmo em mulheres com concentrações séricas normais de estrogênios. O presente artigo tem como objetivo revisar os aspectos práticos da Síndrome de Deficiência Androgênica, enfocando especialmente o diagnóstico e tratamento. Para tanto, nos valemos da análise de 105 artigos publicados em revistas indexadas no PUBMED nos últimos 51 anos (até maio de 2010), incluindo consensos e opiniões de especialistas. Como conclusão, a Síndrome de Deficiência Androgênica na mulher é negligenciada, existindo ainda muitas controvérsias quanto ao seu diagnóstico e terapêutica, especialmente no tocante à escolha do androgênio, a via de administração e o tempo de duração de uso.

Prevalence of sexual problems among women ranges from nine to 43 percent and, recently, many studies have reported that androgens are beneficial not only for the sexual function of women, but for mood disorders and vasomotor symptoms. That is why the physician should include androgen deficiency syndrome as differential diagnosis, even in women with adequate levels of estrogen. Our goal was to present practical aspects of this disease, emphasizing diagnosis and focusing on treatment. This survey covered all the publications indexed in PUBMED in the last 51 years ending May 2010, including consensus and expert opinions; 105 articles were identified. We conclude that the syndrome of androgen deficiency in women is overlooked in clinical practice. There is controversy in literature regarding diagnosis and treatment including choice of drug, route of administration and time of application.
