RESUMO

Water-electrolyte homeostasis and its basic regulatory hormones have been studied in 36 patients with neurosurgical brain pathology during surgical interventions performed under balanced anesthesia (sodium hydroxybutyrate combined with NLA drugs). The study has revealed 3 types of reactions in hormones regulating water-electrolyte homeostasis: type I--a decrease in vasopressin (VP) concentration and renin-angiotensin-aldosterone system (RAAS) activity; type II--VP increase and RAAS activation; type III--an increase in VP content and RAAS disbalance. It has been shown that type I reaction is accompanied by marked osmotic disturbances. The impact of sodium hydroxybutyrate on RAAS is manifested in aldosterone secretion suppression.

RESUMO

A scheme of differential diagnosis of mono- and multifactor compression of the brain was formed on the grounds of 18 most frequently encountered signs of the injury. The work is based on 693 cases which were verified on operation. The results were checked on a large independent selection. Accuracy of recognition was 87.5%. The Table suggested by the authors may prove valuable in emergency situations when the patients are admitted in a serious condition, in impetuous development of the compression syndrome, and when there is lack of time for carrying out instrumental examination.

RESUMO

From analysis of 304 clinical cases with trauma of the spine and spinal cord combined with various extravertebral injuries the authors discuss the specific features of the clinical course and diagnosis of the condition and the surgical tactics in the management of this severe contingent of patients. They substantiate the use of emergency instrumental methods for recognizing injuries to the viscera and brain as well as the tendency to conduct early operations for fractures of the spine and limbs whenever indicated.

RESUMO

The diagnostic possibilities of intraoperative comparative contact thermoencephalometry were studied in 54 patients with various forms of traumatic brain compression. Various temperature characteristics of the dura mater and cerebral cortex which depended on the rate of the clinical course of intracranial hematomas, as well as in foci of contusion and crushing of the brain, were detected. Consequently, this method can be employed during operation for determining the exact character and extent of the comprising substrate in craniocerebral trauma.

RESUMO

Analysis of thermograms recorded through intact skin and during operations in 61 patients with brain tumours differing in histobiological properties and in 63 patients with compressing forms of craniocerebral injury as well as analysis of the results of angiography and rheoencephalography and operative findings disclosed the mechanisms of the formation of pathological luminescence of the skin on the head.

RESUMO

From the analysis of 123 cases in which operations were performed after relieving compression of the brain the authors give a classification of operations according to their etiological trend: relief of persisting compression of the brain, correction of respiratory disorders, provision for enteral feeding, control of purulent inflammatory complications, rehabilitation. Indications for repeated operations and specific features of their performance were developed.

RESUMO

The author used the Soviet-made fast-operating TV-03 thermovision set to prove in principle the possibility of mediated thermovision of intracranial processes through intact coverings of the head. On the basis of the examination of 71 patients with craniocerebral trauma the thermovision characteristics of brain compression are described for the first time. The author distinguish thermovision signs of acute, subacute and chronic intracranial hematoma, brain contusion of convexital, parabasal and basal localization, which contribute to their topical and differential diagnosis.

RESUMO

On the basis of 109 cases with foci of brain crushing their definition and working classification is given according to localization, spread and depth of the damage to the brain matter and the degree of compensation of the function of the brain and the organism as a whole. The concepts of "focus of crushing" and "focus of contusion" are distinguished. Four parameters are distinguished according to which decompensation in foci of crushing may develop: the condition of the vital functions, general cerebral and focal neurological symptoms, the findings of auxiliary examination methods. Attention is drawn to the need for early active diagnosis and the performance of operation in the phase of moderate clinical decompensation, i.e. before pronounced signs of compression and dislocation of the brain develop. Microsurgical techniques promote radical removal of the foci of crushing and thorough hemostasis.

RESUMO

It is shown from the analysis of 38 cases with injury to and compression of the sagittal sinus that these traumas are often attended by contusion of the brain in the sagittal-parasagittal area and are characterized by hemiparesis with predominance of motor disorders in the distal parts of the limbs, often with ischuria. They are managed surgically. Emergency surgery is indicated in hemorrhage and when there is a clinical picture of progressive compression of the brain. In the other cases trephination of the skull should be postponed until shock is completely relieved and wound infection cured and should be performed by a skilled neurosurgeon and anesthetist when a sufficient amount of stored blood is available. Compression of the sagittal sinus for a period of up to 6 weeks did not cause any clinically marked disorders of cerebral circulation.

RESUMO

In order to early recognize intracranial hematomas and contusions, differential diagnostic criteria for analysis of neurological symptomatology and EEG signs are considered from the standpoint of a phasic clinical progress of traumatic pathological conditions of the brain. The EEG syndromes of initial, pronounced and gross synchronization correlated with the clinical phases of subcompensation, mild and deep decompensation. The study demonstrates a contrary trend in the dynamics of the clinical and EEG data in compression and contusion of the brain.

RESUMO

On the basis of examination of 130 patients, the authors describe for the first time the thermovision syndromes of cerebral tumors, intracranial hematomas, posttraumatic cranial defects, tumors of the spinal cord, Schmorl' body, injuries to the peripheral nerves, and some other neurosurgical pathologic conditions. The mechanisms of the origin of thermoasymmetry are discussed. The diagnostic possibilities and the prospects of thermovision in neurosurgery are disclosed.
