Acute sensorineural hearing loss is an uncommon phenomenon. We describe the first case of a 42-year-old lady who presented with acute sensorineural hearing loss occurring immediately after a dental procedure. Possible mechanisms are discussed. She was treated with high dose oral steroids, low molecular weight dextran and vasodilators with benefit.

In this report we present a case of squamous cell carcinoma developing in a mastoid cavity after prolonged exposure to the chemical disinfectant, Eusol. The efficacy and safety of Eusol and other chloric acid (hypochlorous acid) derivatives in clinical use is debated.

The experiences of an otological surgical team during a brief visit to northern Romania are discussed. The importance of careful assessment of local deficiencies and needs is emphasized and sources of funding and equipment for use during such a trip are discussed.

Following the introduction of self-recording audiometers into regular use in non-hospital Royal Naval medical facilities, there has been an increase in the rate of detection of hearing losses and consequent referral for formal audiometry and ENT evaluation at Naval Hospitals. Forty-two sets of audiograms have been examined and the hearing thresholds obtained by the two methods compared. The value of self-recording audiometry even in the often imperfect audiometric conditions available in a Naval sick bay has been confirmed and the midpoint of the tracing established as a reliable indicator of the hearing threshold.

A case is described in which upper airway obstruction was successfully treated using cricothyroidotomy in association with high frequency jet ventilation. This procedure allowed time for the patient to be transferred to the operating theatre for formal tracheostomy under optimal conditions. It is suggested that the Portex 'Mini-trach' kit, though primarily designed for tracheobronchial toilet, can be used to provide a satisfactory and safe airway during the resuscitation of an obstructed patient, particularly if jet ventilation is available.

Experience in the use of free vascularised tissue flaps in reconstruction following major head and neck resections for malignant disease has been described. The benefits of the use of free flaps are improved function and cosmesis with more rapid rehabilitation and reduced in-patient stay in a group of patients, many of whom had a limited life expectancy. The disadvantages of this technique are the need for special equipment and for medical and nursing staff trained and experienced in this type of work.
