RESUMO

PURPOSE: The aim of the present study was to evaluate the sealing performance, expressed as microleakage (ML), of two root-end filling materials when used at different retro-preparation lengths. METHODS: Fifty single-rooted human teeth were collected for the study. The teeth were cut at the cement-enamel junction and endodontic treatment was performed. Each root was cut at 3 mm from the apex and then stored in wet condition. The teeth were divided into three groups according to the retro-preparation length: control group (no retro-preparation); group 1 (retro-preparation of 3 mm); group 2 (retro-preparation of 9 mm). The teeth were equally allocated to either Biodentine or Super EBA treatment group. The teeth were immersed in 3% methylene blue dye solution for 24 h. The samples were split longitudinally and the depth of dye penetration was examined through light microscopy. RESULTS: No significant statistical differences were found at different retro-preparation lengths (P > 0.05). Differences were found between materials (P > 0.05). CONCLUSION: Biodentine showed significantly lower ML when compared to Super-EBA and no statistical significative differences were observed when samples were retro-prepared at 3 mm or 9 mm.

BACKGROUND: To assess the influence of multiple uses on the cyclic flexural fatigue and torsional resistance of Reciproc R25 (REC25) instruments. METHODS: Overall, 256 canals (two for each mandibular molar) were prepared using Reciproc R25 instruments. A total of 96 25-mm-long Reciproc R25 instruments were divided in six groups (N.=8) on the basis of different number of canals shaped: new instruments (control group), one, two, three, four, or six canals shaped during simulated clinical use for other groups respectively. Sixteen files were used for each of the six groups (eight instruments for cyclic fatigue test and eight ones for torsional test). Times to fracture (TtF) for cyclic fatigue and ultimate torsional strength and angle of rotation to fracture for torsional resistance were recorded. Data were statistically evaluated by the analysis of variance (ANOVA) with P<0.05. RESULTS: As for cyclic fatigue resistance, there was no significant difference between new REC25 and REC25 used in one, two and three canals respectively (P>0.05). REC25 used in four and six canals showed significant lower times to fracture when compared with other groups (P<0.05) with REC25 used in six canals showing the significant lowest times to fracture (P<0.05). Considering torsional resistance parameters, no significant difference was observed between the new and used instruments (P>0.05). CONCLUSIONS: REC25 files showed a significant decrease in cyclic fatigue resistance only when used in four or more molar canals while their torsional behavior was not affected by multiple uses.

The aim of the current meta-analysis was to assess the impact of methodological variables in performing fracture strength tests of upper premolars. Medline (Pubmed), Embase and Google Scholar were screened for studies performing ex vivo fracture strength tests of intact upper premolars or premolars with 0, 1 or 2 walls lost. The outcome variable for each study was the maximum breaking load expressed in Newton (kg × m/s2). Methodological variables (i.e., simulation of the periodontal ligament, load inclination, tip position, tip diameter and thermocycling) were registered to perform subgroup analyses and meta-regression. Overall, 25 studies and 78 study groups were included in the meta-analysis. Intact premolars (17 study groups) were not significantly influenced by any of the methodological variables considered. Subgroup analysis for load inclination (30°/45° vs. 90°/150°) was significant for premolars with 0 (10 study groups), 1 (6 study groups) and 2 (45 study groups) walls lost; thermocycling was significant for premolars with 1 and 2 walls lost. A strong methodological heterogeneity across studies evaluating the fracture strength of upper premolars was highlighted, especially when 0, 1 or 2 walls were lost. Further studies are needed to standardize the methodology used in order to allow for across-studies comparisons.

OBJECTIVE: The present review aimed to systematically evaluate the occurrence of caries in patients with inflammatory bowel disease (IBD), either Crohn's disease (CD) or ulcerative colitis (UC), compared to healthy controls. MATERIALS AND METHODS: MEDLINE (PubMed), Embase, Google Scholar, LILACS, and Cochrane Library electronic databases were screened. Caries experience was measured through the Decayed, Missing, Filled Teeth (DMFT) index. The weighted mean difference (WMD) with 95% confidence interval was calculated between IBD patients and healthy controls. RESULTS: Six studies were selected for the inclusion in the systematic review, 5 of which were also included in the quantitative synthesis of data. The WMD in the DMFT index between IBD and healthy subjects was 3.04 (1.52, 4.56) (p = 0.10). Subgroup analysis showed no difference (p = 0.31) between CD (2.52 [0.54, 4.49]) and UC (4.01 [1.52, 4.56]) subjects. CONCLUSIONS: There is a remarkably higher past and present occurrence of dental caries in subjects with IBD than healthy controls. This result should encourage clinicians to include oral health preventive programs in the overall treatment plan of IBD patients.

RESUMO

In order to reduce the spread of COVID-19 (Coronavirus Disease 2019), it is crucial to take extraordinary prevention and safety measures in dental offices, and to defer all elective and non-urgent procedures. Dental emergencies are defined through oral symptoms but, the systemic and psychological conditions of each patient should be considered. The present short communication proposes a multilevel evaluation (oral, systemic and psychological) and risk assessment score for the management of dental emergencies following the SARS-CoV-2 (severe acute respiratory syndrome coronavirus-2) outbreak. A comprehensive categorization and score scale assessment for dental emergencies allows a better identification of patient's treatment needs and avoids unnecessary contact between dental health care providers and patients during the SARS-CoV-2 pandemic.

INTRODUCTION: The aim of the study was to assess cyclic fatigue resistance of reciprocating (Reciproc and WaveOne) and continuous rotating (ProTaper) nickel-titanium files after immersion in different irrigation solutions with or without surfactants during several short time periods. METHODS: A total of 270 new Reciproc R25, WaveOne Primary, and ProTaper F2 files were tested. Instruments of each brand were divided into 1 control group (n = 10) formed by new files and 4 test groups (n = 20) formed by instruments dynamically immersed at 37°C for 16 mm in 5.25% sodium hypochlorite (NaOCl), Hypoclean (5.25% NaOCl with surfactant), 17% EDTA, or EDTA Plus (17% EDTA with surfactant). Each test group was subdivided into 2 subgroups (n = 10) on the basis of the time of dynamic immersion in the endodontic irrigant solution (45 seconds or 3 minutes). Resistance to cyclic fatigue was determined by recording time to fracture in a stainless steel artificial canal with a 60° angle of curvature and 5-mm radius of curvature. The fracture surface was examined by using scanning electron microscopy. RESULTS: Immersion in NaOCl did not reduce the cyclic fatigue of reciprocating or continuous nickel-titanium files. The 17% EDTA reduced the fatigue resistance of all instruments after 3 minutes. The immersion in irrigants with surfactants did not influence the cyclic fatigue of instruments except for Reciproc immersed in Hypoclean solution. CONCLUSIONS: EDTA immersion reduced the cyclic fatigue resistance of all instruments after 3 minutes. Addition of surfactants did not influence the cyclic fatigue of files except when added to NaOCl when it contacts Reciproc instruments.

INTRODUCTION: The shaping capacity of nickel-titanium (NiTi) rotary instruments is often assessed by photographic or micro-computed tomography (micro-CT) measurements, and these instruments are often used more than once clinically. This study was conducted to compare photographic and micro-CT measurements and to assess if the repeated use of NiTi instruments affected the shape of canal preparation. METHODS: Ten new sets of ProTaper Universal instruments (Dentsply-Maillefer, Ballaigues, Switzerland) were used in 60 resin blocks simulating curved root canals. Groups 1 to 6 (n=10) represented the first to sixth use of the instrument, respectively. Digitized images of the prepared blocks were taken in both mesiodistal (MD) and buccolingual (BL) directions and area measurements (mm(2)) were calculated using AutoCAD (Autodesk Inc, San Rafael, CA). The volumes of the same prepared canals were measured using micro-CT (mm(3)). Statistical analysis was performed to detect differences between photographic and volumetric measurements and differences between uses. RESULTS: Two-way repeated-measures analysis of variance revealed significant differences between groups (P < .001). Regarding measurement type, there were no significant differences between BL and MD measurements, but there were significant differences between micro-CT and BL measurements (P < .001) and micro-CT and MD measurements (P=.001). Significant differences were also noted between uses. CONCLUSIONS: Within the limitations of the present study, micro-CT scanning is more discriminative of the changes in canal space associated with repeated instrument use than photographic measurements. Canal preparations are significantly smaller after the third use of the same instrument.
