RESUMO

Vedolizumab is used in the treatment of inflammatory bowel disease and is generally well tolerated. We report a 20-year-old man who presented with right flank pain on a background of ulcerative colitis. He was on vedolizumab with his last dose 1 week before the onset of symptoms. Kidney function tests revealed a serum creatinine of 171 µmoL/L and a C-reactive protein of 74 mg/L. Kidney biopsy demonstrated focal acute interstitial nephritis. He was prescribed oral prednisolone and achieved complete recovery of renal function within 3 weeks. At the follow-up after 4 months, his renal function remains normal.

RESUMO

Hibernoma is a rare benign tumour that was first described by Merkel in 1906. It arises from remnants of brown fat and has a differential diagnosis of lipoma and liposarcoma. This is a case report of a 31-year-old male with a slow-growing mass in the left flank that produced constant pain radiating to the groin. Computerised tomography localised the mass within the external oblique muscle, which showed some heterogeneity and low attenuation. The mass appeared hypodense to muscle on T1 and hyperdense to muscle on T2 weighted magnetic resonance images. Prominent vascularity of the mass was noted. Finally, the lesion was found to be a 'typical' hibernoma on core-needle biopsy. It was surgically resected with a cuff of muscle. He recovered without complication, and there is no clinical evidence of recurrence at 6 months.

RESUMO

Incidental diagnosis of haematolymphoid disorders in prostate tissue is rare, with the largest study in the literature reporting a 0.37% incidental lymphoma diagnosis in prostate and associated lymph node tissue. B cell lymphocytosis (MBL) is a relatively recently defined disease entity. The authors present the diagnosis and management of a patient diagnosed with incidental MBL on transurethral resection of prostate (TURP), performed for symptomatic lower urinary tract symptoms refractory to pharmacological therapy. Initially thought to represent chronic lymphocytic leukemia, repeated flow-cytometric analysis confirmed the diagnosis of MBL, and the patient remains under surveillance without the requirement for further medical treatment.
