RESUMO

The aim of this study was to evaluate the effects of 7-epiclusianone (7-epi) on specific virulence attributes of Streptococcus mutans in vitro and on development of dental caries in vivo. 7-Epi was obtained and purified from fruits of Rheedia brasiliensis. We investigated its influence on surface-adsorbed glucosyltransferase (Gtf) B activity, acid production, and viability of S. MUTANS in biofilms, as well as on caries development using a rodent model. 7-Epi (100 µg/mL) significantly reduced the activity of surface-adsorbed GtfB (up to 48.0 ± 1.8 of inhibition at 100 µg/mL) and glycolytic pH-drop by S. mutans in biofilms (125 and 250 µg/mL) (vs. vehicle control, p < 0.05). In contrast, the test compound did not significantly affect the bacterial viability when compared to vehicle control (15 % ethanol, p > 0.05). Wistar rats treated topically with 7-epi (twice daily, 60-s exposure) showed significantly smaller number of and less severe smooth- and sulcal-surface carious lesions (p < 0.05), without reducing the S. mutans viable population from the animals' dental biofilms. In conclusion, the natural compound 7-epiclusianone may be a potentially novel pharmacological agent to prevent and control dental caries disease.

7-Epiclusianone (7-epi), a novel naturally occurring compound isolated from Rheedia brasiliensis, effectively inhibits the synthesis of exopolymers and biofilm formation by Streptococcus mutans. In the present study, the ability of 7-epi, alone or in combination with fluoride (F), to disrupt biofilm development and pathogenicity of S. mutans in vivo was examined using a rodent model of dental caries. Treatment (twice-daily, 60s exposure) with 7-epi, alone or in combination with 125 ppm F, resulted in biofilms with less biomass and fewer insoluble glucans than did those treated with vehicle-control, and they also displayed significant cariostatic effects in vivo (p < 0.05). The combination 7-epi + 125 ppm F was as effective as 250 ppm F (positive-control) in reducing the development of both smooth- and sulcal-caries. No histopathological alterations were observed in the animals after the experimental period. The data show that 7-epiclusianone is a novel and effective antibiofilm/anticaries agent, which may enhance the cariostatic properties of fluoride.

The intratypic variability of HPVs 16 and 18 has been extensively studied and has been used as an important tool in epidemiological studies of viral transmission, persistence and progression to clinically relevant cervical lesions. Infections by non-European variants of HPVs 16 and 18 are associated with an increased risk for the development of high grade squamous intraepithelial lesions (HSIL). Our aim was to correlate the intratypic molecular variability of both HPV types and risk of persistent infection and lesion outcome in a cohort study conducted in Brazil. We characterized molecular variants of HPV types 16 and 18 by sequencing a fragment of the LCR, and of the E6 and L1 genes, for HPV-16 variants only. For both types, European variants composed the most prevalent and diverse group. Persistent infections with HPV-18 were associated with continuous detection of European variants. However, risk for simultaneous detection of HSIL and HPV DNA was higher in women harboring non-European variants of HPV-16. The same trend was observed with HSIL detected during follow-up. Our study confirms the association between non-European variants and risk of cervical neoplasia, and highlights the importance of their geographic distribution for cervical cancer risk assessment.

BACKGROUND: Besides an established role for certain human papillomavirus (HPV) genotypes in the etiology of cervical cancer, little is known about the influence of multiple-type HPV infections on cervical lesion risk. We studied the association between multiple HPV types and cervical lesions among 2,462 Brazilian women participating in the Ludwig-McGill study group investigation of the natural history of HPVs and cervical neoplasia. METHODS: Cervical specimens were typed by a PCR protocol. The cohort's repeated-measurement design permitted the assessment of the relation between the cumulative and concurrent number of HPV types and any-grade squamous intraepithelial lesions (SIL) and high-grade SIL (HSIL). RESULT: At individual visits, 1.9% to 3.2% of the women were infected with multiple HPVs. Cumulatively during the first year and the first 4 years of follow-up, 12.3% and 22.3% were infected with multiple types, respectively. HSIL risk markedly increased with the number of types [odds ratio (OR), 41.5; 95% confidence interval (95% CI), 5.3-323.2 for single-type infections; OR, 91.7; 95% CI, 11.6-728.1 for two to three types; and OR, 424.0; 95% CI, 31.8-5651.8 for four to six types, relative to women consistently HPV-negative during the first year of follow-up]. The excess risks for multiple-type infections remained after exclusion of women infected with HPV-16, with high-risk HPV types, or persistent infections, particularly for any-grade SIL. Coinfections involving HPV-16 and HPV-58 seemed particularly prone to increase risk. CONCLUSION: Infections with multiple HPV types seem to act synergistically in cervical carcinogenesis. These findings have implications for the management of cervical lesions and prediction of the outcome of HPV infections.

Recently, two chemically different types of Brazilian propolis (type-3 and -12) were shown to have cariostatic properties. This study aimed to evaluate the influence of their isolated fractions on mutans streptococci viability, glucosyltransferases (GTFs) activity and caries development in rats. The ethanolic extracts of propolis (EEPs) were serially fractionated into hexane (H-fr), chloroform, ethyl acetate, and ethanol. The ability of the four fractions and EEP to inhibit Streptococcus mutans and Streptococcus sobrinus growth and adherence to a glass surface was examined. The effect on GTFs B and C activity was also determined. For the caries study, 60 Wistar rats infected with Streptococcus sobrinus were treated topically twice daily as follows: (1) EEP type-3, (2) H-fr type-3, (3) EEP type-12, (4) H-fr type-12, and (5) control. In general, the H-fr from both types of propolis showed the highest antibacterial activity and GTFs inhibition. Furthermore, the EEP and H-fr type-3 and -12 were equally effective in reducing dental caries in rats. The data suggest that the putative cariostatic compounds of propolis type-3 and -12 are mostly non-polar; and H-fr should be the fraction of choice for identifying further potentially novel anti-caries agents.

OBJECTIVE: Retinoids (natural and synthetic derivatives of vitamin A) have cancer chemotherapeutic and chemopreventive activities. Retinoic acid (RA) treatment has been associated with significant regression of preneoplastic lesions. However, serious toxicity associated with some therapies has made long-term chemoprevention in healthy populations unfeasible. Recently, serum RA has been shown to increase in response to oral retinol (vitamin A) supplementation. Here, we assess the variability of circulating RA levels and the lifestyle, demographic, and nutritional factors that explain such variability. METHOD: Total RA concentration and the concentrations of RA isomers (all-trans-RA, 13-cis-RA, and 9-cis-RA) were measured by high-pressure liquid chromatography in serum samples obtained 4 months apart from 502 women participating in the Ludwig-McGill Cohort (Sao Paulo, Brazil). RESULTS: The relative abundance of the three RA isomers was similar for each visit (baseline and month 4), with 13-cis-RA having the highest concentrations followed by 9-cis-RA and all-trans-RA. The within-person variability of total RA and individual isomers was low. Using multivariate logistic regression models (upper tertile versus low/middle tertile of serum RA), we found that age, race, oral contraceptive use, total number of pregnancies, and season of initial blood draw were significantly associated with at least one endogenous RA isomer level. All endogenous RA isomers were positively associated with serum retinol, beta-carotene, and beta-cryptoxanthin levels. CONCLUSION: These results have implications for the design of future epidemiologic studies focused on assessing RA-disease association and intervention studies aimed at modulating RA levels.

The association between dietary intake and persistence of type-specific human papillomavirus (HPV) infection, during a 12-month period, among 433 women participating in the Ludwig-McGill HPV Natural History Study was evaluated by use of a nested case-control design. Dietary intake was assessed by a food-frequency questionnaire at the month-4 visit. HPV status was assessed at months 0, 4, 8, and 12 by polymerase chain reaction (MY09/11). Only women who ever tested positive for HPV were included in the present study: 248 had transient HPV infections (1 of 4 positive tests or nonconsecutively positive), and 185 had persistent HPV infections (> or =2 consecutive tests positive for the same HPV type). Risk of type-specific, persistent HPV infection was lower among women reporting intake values of beta-cryptoxanthin and lutein/zeaxanthin in the upper 2 quartiles and intake values of vitamin C in the upper quartile, compared with those reporting intake in the lowest quartile. Consumption of papaya > or =1 time/week was inversely associated with persistent HPV infection.

The authors studied the time-dependent association between human papillomavirus (HPV) infection and squamous intraepithelial lesions (SIL) among women enrolled in a cohort study in Brazil (1993-2002), using repeated Papanicolaou cytologic examination and HPV testing by polymerase chain reaction. Through simulation with conceivable alternative cohort designs, they investigated different regression modeling approaches using time-varying covariates, time-varying hazard ratio functions, and repeated events to assess the effect of delay in lesion detection. Associations between HPV and early SIL were of high magnitude. The age-adjusted hazard ratios for the association between HPV at enrollment and low-grade SIL decreased gradually with time until 72 months for both oncogenic types of HPV (hazard ratio = 3.96, 95% confidence interval (CI): 2.5, 6.4) and nononcogenic types (hazard ratio = 2.37, 95% CI: 1.3, 4.3). The hazard ratio for incident high-grade SIL remained constant, ranging from 7.15 (95% CI: 2.0, 25.1) at 12 months to 6.26 (95% CI: 2.7, 14.5) at 72 months for oncogenic types of HPV. With oncogenic HPV as the time-dependent predictor variable, the hazard ratios for incident SIL and high-grade SIL events were 14.2 (95% CI: 8.7, 23.1) and 32.7 (95% CI: 8.4, 127.3), respectively. Investigators may underestimate the prognostic value of HPV detection using designs that rely on HPV ascertainment at a single time point. The waning in hazard ratios should be considered in the implementation of HPV testing-based screening programs.

Cervical cancer morbidity and mortality have decreased substantially during the last 50 years mostly due to successful organized or opportunistic screening with Pap cytology in high and middle income countries. In many low income countries Pap cytology screening is yet to be effectively implemented or has failed to reduce cervical cancer rates to an appreciable extent. The fact that infection with certain human papillomavirus (HPV) types is now recognized as a necessary cause of this disease has led to new research fronts on prevention of cervical cancer. Testing for HPV DNA has shown great promise as a screening tool with better sensitivity but somewhat lower specificity than Pap cytology. In combination with the latter, HPV testing has the potential to improve the negative predictive value of cytology, thus allowing for increased testing intervals, which would lower program costs with acceptable safety. Advances in cytology processing and automation have also led to new screening approaches that are increasingly gaining acceptance in high and middle income countries. For low income countries, visual inspection with acetic acid has proven to be an effective alternative to conventional Pap cytology, especially in settings where no screening programs have been implemented. Concerning primary prevention of cervical cancer, recent research on the safety and efficacy of candidate prophylactic vaccines against HPV have shown very promising results with nearly 100% efficacy in preventing persistent infections and development of cervical cancer precursors. However, policy makers are strongly cautioned to avoid deferring decisions concerning the implementation of cervical cancer screening under the expectation that a successful vaccine could obviate the need for secondary prevention strategies. HPV infections are believed to be a necessary cause of cervical cancer. Viral burden, as a surrogate indicator for persistence, may help predict risk of subsequent SIL. We used results of HPV test and cytology data repeated every 4-6 months in 2,081 women participating in a longitudinal study of the natural history of HPV infection and cervical neoplasia in São Paulo, Brazil. Using the MY09/11 PCR protocol, 473 women were positive for HPV DNA during the first 2 visits. We retested all positive specimens by a quantitative, low-stringency PCR method to measure viral burden in cervical cells. Mean viral loads and 95% CIs were calculated using log-transformed data. RRs and 95% CIs of incident SIL were calculated by proportional hazards models, adjusting for age and HPV oncogenicity. The risk of incident lesions increased with viral load at enrollment. The mean number of viral copies/cell at enrollment was 2.6 for women with no incident lesions and increased (trend p = 0.003) to 15.1 for women developing 3 or more SIL events over 6 years of follow-up. Compared to those with <1 copy per cell in specimens tested during the first 2 visits, RRs for incident SIL increased from 1.9 (95% CI 0.8-4.2) for those with 1-10 copies/cell to 4.5 (95% CI 1.9-10.7) for those with >1,000 copies/cell. The equivalent RR of HSIL for >1,000 copies/cell was 2.6 (95% CI 0.5-13.2). Viral burden appears to have an independent effect on SIL incidence. Measurement of viral load, as a surrogate for HPV persistence, may identify women at risk of developing cervical cancer precursors.

La morbilidad y mortalidad de cáncer cervical ha decrecido sustancialmente durante los últimos 50 años, debido principalmente a programas organizados de detección oportuna de cáncer basados en citología, particularmente en países con altos y medianos ingresos. Sin embargo, en muchos países de bajos ingresos el programa de detección oportuna de cáncer basado en citología apenas está siendo implantado correctamente o tiene fallas, por lo que no se puede apreciar el alcance para reducir las tasas de este cáncer. El hecho es que la infección con ciertos tipos de virus de papiloma humano es ahora reconocida como una causa necesaria de la enfermedad, lo que ha conducido a nuevas investigaciones frente a la prevención del cáncer cervical. Las pruebas de ADN del virus del papiloma humano se han mostrado como una herramienta prometedora con gran sensibilidad, pero con especificidad más baja que la citología. En combinación con citología ginecológica, la prueba del virus del papiloma humano tiene potencial para mejorar el valor predictivo negativo de la prueba convencional, permitiendo incrementar los intervalos de periodicidad de la misma y poder disminuir los costos del programa con seguridad aceptable. Los avances en el procesamiento de la prueba de citología y automatización de pruebas que determinan ADN del virus, han permitido nuevas propuestas de tamizaje lo que incrementa cada vez más su aceptación en países de altos y medianos ingresos. Para países de bajos ingresos, se ha probado que la inspección visual con ácido acético es una alternativa efectiva para la citología convencional, especialmente observada en sitios en los que los programas de tamizaje no han sido organizados. Referente a la prevención primaria del cáncer cervical, investigaciones recientes sobre seguridad y eficacia de la vacuna profiláctica contra el virus del papiloma humano han mostrado resultados muy prometedores con una eficacia cercana a 100 por ciento en prevención de infecciones persistentes y desarrollo de lesiones precursoras de este cáncer. Sin embargo, las políticas generadas son fuertemente cautelosas para evitar decisiones aplazadas, concernientes a la implantación del tamizaje del cáncer cervical bajo la expectativa de que la vacuna podría obviar la necesidad de estrategias de prevención secundaria.

Este estudo avaliou o potencial cariogênico de fórmulas infantis e do leite de vaca, usando um modelo de alto desafio cariogênico em animais. Sessenta ratas Wistar infectadas com Streptococcus sobrinus e dessalivadas foram aleatoriamente divididas em 6 grupos os quais receberam ad libitum:(1) água destilada e esterilizada (ADE) com 5 por cento de sacarose; (2) leite bovino tipo A; (3) Nan 2; (4) Nestogeno 2; (5) Ninho coadjuvante de crescimento; (6) ADE. Os grupos 1 e 6 também receberam dieta essencial NCP#2, através de gavage, 2 vezes/dia. Após 21 dias, os animais foram sacrificados e avaliados quanto a microbiota oral recuperada e o índice de cárie de Keys modificado. A análise dos carboidratos nos leites foi feita através de CLAE. Os resultados foram submetidos aos testes de Shapiro-Wilk e Krusal-Wallis. O leite bovino apresentou o menor potencial cariogênico quando comparado aos outros grupos testes, porém não diferiu estatisticamente do grupo 6. As porcentagens de S. sobrinus dos grupos 1 a 5 não apresentaram diferenças estatisticamente significantes entre si, exceto em relação ao grupo 6. Concluímos que o leite bovino não foi cariogênico e as fórmulas infantis mostraram potencial cariogênico.

Sixty female Wistar rats infected with Streptococcus sobrinus and desalivated were randomly divided into 6 groups, which received ad libitum: 1) sterilized deionized distilled water (SDW) with 5% sucrose; 2) cow's milk; 3) Nan 2; 4) Nestogeno 2; 5) Ninho growth supporting; 6) SDW. Groups 1 and 6 also received essential diet NCP#2 by gavage, twice a day. After 21 days, the animals were killed and evaluated according to recovered oral microbiota and caries score by using a modified Keyes method. The analysis of the carbohydrates in the milk samples was performed using HPLC. The results were analyzed by Shapiro-Wilk and Kruskal-Wallis tests. Cow's milk had the lowest cariogenic potential compared to the other test groups, but it was not statistically different from group 6. The percentage of S. sobrinus obtained from the oral cavity of the animals was not statistically different among the groups studied, except for the SDW group. It was concluded that cow's milk was not cariogenic and infant formulas showed some cariogenic potential.

RESUMO

O conceito de que a mulher deva ter suas necessidades básicas de saúde atendida no setor público como um todo, foi incorporado pela Secretaria de Saúde do Estado de São Paulo (SES) em 1985, traduzindo-se na prática, pela implantação paulatina em todos os seus Centros de Saúde (CS), das Ações Integradas de Assistência à Saúde da Mulher (AIASM). Para tanto, foram contratados recursos humanos e os CS equipados com material permanente e de consumo. O programa de prevenção e controle dos cânceres de cérvix uterino e de mama é um dos elementos deste conjunto. Dentro do sistema de saúde vigente em 1985, somente poucas unidades de saúde da SES ofereciam serviço de prevenção do câncer ginecológico à população feminina do Estado. Entretanto, os dados disponíveis revelam que os cânceres do cérvix uterino e da mama têm relativamente alta prevalência na área, com elevada morbidade e mortalidade relativas para mulheres em idade fértil. Sendo assim, temporaria ou permanentemente, estes tumores têm retirado do lar as mães numa época em que sua presença é fundamenteal para o desenvolvimento de seus filhos. Da mesma forma, é perdida mão de obra importante nos locais de trabalhos visto que, hoje em dia, a mulher contribui significativamente para a economia doméstica e da nação. Com a melhor compreensão acerca da história natural do câncer de cérvix uterino verificou-se que este apresenta lesões precursoras, as displasias, que com o passar dos anos podem evoluir para carcinoma "in situ" e dai para carcinoma invasor. O tempo de evolução de um estágio para o seguinte varia de uma população estudada para outra. O diagnóstico de qualquer destes estágios é bastante simples devido...
