RESUMO

Immune thrombocytopenia (ITP) was reported as a rare complication of COVID-19 vaccines. We conducted a retrospective single-center analysis of all ITP cases detected in 2021 and compared the quantity with the pre-vaccination years, from 2018 to 2020. In 2021, a two-fold increase in ITP cases was identified compared to previous years; 11 of 40 cases (27.5%) were considered COVID-19-vaccine related. Our study highlights an increase in ITP cases at our institution, probably related to COVID-19 vaccinations. Further studies are needed to investigate this finding globally.

OBJECTIVES: Compared to other senses, temporal perception of odors seems fairly slow. In addition it has been shown in previous studies that even not consciously perceived odors could influence our behavior. Aim of the current study therefore was to study the interstimulus interval (ISI) length, which is necessary between two repetitive stimuli to be able to perceive them separately. The additional aim focused on observing central odor processing of not perceived odorous stimuli. MATERIALS AND METHODS: The study was divided into three parts. In each part healthy, normosmic volunteers were included. In part I and II stimulus pairs (CO2, H2S, orange and phenyl ethyl alcohol (PEA)) were presented to the subjects via a computer-controlled olfactometer with short ISI of 0.6-9s. The decision whether one or two stimuli were perceived was recorded. In addition the influence of odor valence, trigeminallity and concentrations was observed. In part III olfactory event-related potentials (OERPs) to perceived and not-perceived odors were recorded. RESULTS: The two stimuli of a stimulus pair were perceived separately more often with increasing ISI length. This increase was significant until an ISI between the stimuli of 4s. Odor intensity, pleasantness, trigeminallity and sex had no major influence on this. In addition we were able to observe that OERPs are less often detected in response to not perceived olfactory stimuli. However, the presence of OERP in response to not perceived stimuli in more than half of the cases indicated that even not perceived stimuli are centrally processed.

BACKGROUND: Although evidence for their therapeutic efficacy is limited, herbal traditional Chinese medicine (TCM) preparations increasingly gain popularity. In contrast to other herbal products, adverse effects by herbal TCM including liver toxicity were rarely reported. In recent years, more cases were published, providing new clinical challenges. AIM: To summarise comprehensively the literature on herbal TCM hepatotoxicity since 2011. METHODS: PubMed was searched using key words related to TCM, the results were restricted to full English-language publications and abstracts published since 2011. In addition, the database of the National Institutes of Health (NIH) and LiverTox was accessed under the topic 'Drug record: Chinese and other Asian herbal medicines'. RESULTS: Since 2011, new case reports and case series provided evidence for herbal hepatotoxicity by TCM, focusing on nine TCM herbal mixtures and four individual TCM herbs with potential health hazards. These were the TCM products Ban Tu Wan, Chai Hu, Du Huo, Huang Qin, Jia Wei Xia Yao San, Jiguja, Kamishoyosan, Long Dan Xie Gan Tang, Lu Cha, Polygonum multiflorum products, Shan Chi, 'White flood' containing the herbal TCM Wu Zhu Yu and Qian Ceng Ta, and Xiao Chai Hu Tang. Other developments include the establishment of a new and early diagnostic serum marker for hepatotoxicity caused by pyrrolizidine alkaloids, assessed using ultra performance liquid chromatography-mass spectrometry analysis, and new regulatory details to improve herbal TCM product quality and safety. CONCLUSION: Stringent evaluation of the risk/benefit ratio is essential to protect traditional Chinese medicines users from health hazards including liver injury.

INTRODUCTION: Continuous bedside brain tissue oxygenation (p(br)O(2)) monitoring using the Licox system is an established method for detecting secondary ischemia in comatose patients with acute brain injury. The purpose of the current study was to compare the newly introduced Raumedic p(br)O(2) probe with the established standard. METHODS: Eighteen patients with acute traumatic brain injury or aneurysmal subarachnoid hemorrhage had p(br)O(2) probes of both types implanted side by side in the same vascular territory at risk of ischemia. Data were analyzed by the Bland-Altman method as well as random effect regression models to correct for multiple measurements per individual. RESULTS: Both types of probes were able to display spontaneous fluctuations of p(br)O(2) as well as reactions to therapy. Mean measurement difference between the Licox and Raumedic probes was -2.3 mmHg, with corresponding 95% limits of agreement of -32.3 to 27.5 mmHg. Regarding an ischemia threshold of 15 mmHg, both probes were in agreement in 78% and showed disparate results in 22%. CONCLUSIONS: Our data suggest that the p(br)O(2) measurements of the two systems cannot be interchanged. Although we were unable to determine which system delivers more valid data, we do think that more rigorous testing is necessary before implementing the new probe in clinical routine.

OBJECTIVE: Validation of a bedside test to measure the monocular and binocular subjective visual vertical (SVV). METHODS: Monocular and binocular measurements of the SVV were made in 30 healthy subjects (mean age 46.1 +/- 16.4 years) and 30 patients with acute peripheral or central vestibular lesions (mean age 51.3 +/- 17.1 years). The established method uses a hemispheric dome (adjustment of a bar to the vertical without spatial orientation clues in a motor-driven, hemispheric dome randomly covered with colored dots). It was compared to a simple, self-made bucket method, in which the subject estimates true verticality by attempting to properly align a straight line visible on the bottom of a bucket that is rotated at random by the examiner. RESULTS: Comparative measurements of the whole group (n = 60) showed no difference in the distribution of monocular or binocular SVV values while using the hemispheric dome or the bucket method. Intertest reliability was 89% for monocular and 90% for binocular SVV measurements. Intratest reliability of the bucket method in 10 repetitions was 92% for binocular and monocular SVV determinations. With the bucket method, the range of absolute deviations of SVV values from true verticality in healthy subjects was 1.1 +/- 0.7 degrees for monocular and 0.9 +/- 0.7 degrees for binocular measurements (mean +/- SD). There was no significant effect of age or gender. CONCLUSIONS: The bucket method is an easily performed and reliable bedside test for determining monocular and binocular subjective visual vertical. The bucket can be modified within 1 hour and costs less than $5.

Gap junction channels are essential for intercellular electrical communication in the heart. The most important cardiac gap junction proteins are connexin43 (predominantly) (Cx43), connexin40 (Cx40), and in early developmental stages connexin45. Since catecholamines play an important role in cardiac (patho)physiology, we wanted to elucidate whether catecholamines may affect expression of Cx43 and Cx40. Cultured neonatal rat cardiomyocytes were exposed for 24 h to increasing concentrations of noradrenaline (1-10000 nM) (physiological agonist at alpha and beta-adrenoceptors), resulting in significantly increased Cx43-expression, while Cx40 was unaffected. In further experiments cells were incubated with either phenylephrine (alpha-adrenergic agonist) or isoproterenol (beta-adrenergic agonist) (0.1-1000 nM) for 24 h. Both catecholamines lead to a concentration-dependent increase in Cx43 protein and mRNA expression (EC50: 10-20 nM). Inhibition experiments showed that the phenylephrine effect was transduced via PKC, while the isoproterenol effect was mediated by PKA. Dual whole-cell voltage clamp demonstrated that increased Cx43-expression was accompanied by significant increases in gap junction current. In additional in vivo experiments, adult rats were subjected to 24-h infusion of isoproterenol or phenylephrine showing again significant increase in Cx43 but not Cx40. Adrenergic stimulation of cardiomyocytes can enhance Cx43 expression thereby increasing cellular coupling, indicating a possible role for catecholamines in the regulation of cardiac gap junction expression in cardiac disease.

Preparations of stress protein gp96 from tumor cells are active as tumor vaccines by eliciting immune responses against mixtures of individual tumor peptide antigens which are complexed to gp96. Due to the individual antigenicity of tumors, a vaccine consisting of tumor-derived gp96 has to be prepared individually for each patient from autologous tumor tissue. So far, gp96 expression by human tumors has not been analyzed. Here, we report stable and mostly homogenous expression of gp96 by colorectal cancer, which was enhanced compared to surrounding tumor stroma in 70% to 80% of colorectal cancer specimens. Fewer non-metastatic than metastatic primary cancer specimens showed enhanced gp96 expression. Glucose deprivation increased gp96 protein and RNA expression in the human colon cancer cell line HT-29 in accordance with the role of gp96 as a glucose-regulated stress protein. Additionally, TNF-alpha, interferons and other cytokines induced an increase of gp96 RNA expression in HT-29 cells, suggesting that gp96 expression by colorectal cancer cells can be influenced by different methods of immunomodulation. The stable and homogenous expression of gp96 in 19 primary and metastatic colorectal cancer specimens and the up-regulation of gp96 in colon cancer cells by glucose deprivation point to an essential role of this stress protein in colorectal cancer, presumably by protecting against hostile conditions of the tumor micro-environment like glucose deprivation. In view of these results, loss of gp96 expression by colorectal cancer cells as an immune escape mechanism is unlikely.

In 1993, J.Y. Chevalier described a single needle venovenous extracorporeal membrane oxygenation (ECMO) system using a nonocclusive roller pump and alternating clamps for pulmonary support in neonates. We modified this system to use it in older children as well and for additional indications. Introducing a double raceway and 2 different sizes of tubing sets and performing percutaneous approach, we treated 21 children (age 1 day to 49 months) using this system. Indications for treatment were hypoxia and hypoxic induced myocardial dysfunction resulting from pulmonary failure, sepsis, and congenital defects. Of the children treated for neonatal indications, 7/9 survived. For 2 children ECMO was terminated because of intraventricular hemorrhage (IVH). In the pediatric group 5/7 of the children could be weaned from ECMO, and 2 children died after more than 30 days on ECMO. Two of the children who had been almost completely weaned died later because of therapy withdrawal following a brain death diagnosis. In the cardiac group, 3/5 of the children survived. We conclude that the described system is an effective venovenous ECMO system that reduces invasivity and expenditure.

We evaluated a bedside method of assessing respiratory compliance from chest expansion, which is judged by eye and classified into three tidal volume categories, and from the inflation pressure read on the respirator's manometer. Compliance assessed by this method was compared with the compliance measured by the injection technique in 45 randomly chosen newborns ventilated for various diseases. The compliance assessed from chest expansion and inflation pressure correlated significantly with the measured compliance (r = 0.86, p < 0.001). Interobserver reliability was acceptable for clinical practice and improved within 6 months of training (weighted kappa = 0.67 versus 0.86). Within ten individuals, changes of compliance assessed by this method were also significantly correlated with those of measured compliance (r = 0.85, p < 0.01). This method, termed "optical compliance," is instantaneously and always available for all patients on ventilators without any additional apparatus and thus might improve the assessment of respiratory function in routine care and emergency situations.

An ultrahigh vacuum chamber is described which is being used to study laser Raman spectra associated with gas adsorption on clean metal surfaces. The system enables sample temperatures from -85 degrees to 600 degrees C to be obtained, and besides surface cleaning capabilities associated with high-temperature oxidation or reduction, it incorporates an Ar(+) sputter gun for surface cleaning. Initial experiments of CO+H(2) adsorption on crystalline Ni surfaces are described and the effects of H(2) and O(2) exposure shown for the 80 cm(-1) band which results from CO+H(2) adsorption.

RESUMO

The influence of dimethylsulfolane on active avoidance conditioning with simultaneous registration of EEG was investigated. Dimethylsulfolane, which shows some pharmacological properties of pentylenetetrazole causes a decrease of the amount of conditioned reactions. There was no strong correlation between the impairment of learning ability and the number of EEG spindle discharges after dimethylsulfolane.
