RESUMO

Within the past 5 years panretinal photocoagulation was performed in both eyes of 36 patients (Type I diabetics) with proliferative retinopathy. The age of these patients was between 21 and 38 years. The average follow-up period was 3.6 years. The effects on visual acuity, visual field, and dark adaptation were examined: in 65.3% of the cases (47 eyes) visual acuity remained unchanged; in 34.7% (25 eyes) central visual acuity was reduced. Depending on the coagulation technique and the number of coagulates applied, the visual field narrowed in a more or less pronounced way. Moreover, in all patients the dark adaptation threshold was elevated after panretinal photocoagulation.

RESUMO

The hormonal regulation of normal pregnancy by the pituitary gland is summarized: during pregnancy the pituitary temporarily becomes enlarged and usually exerts pressure on the anterior angle of the chiasm from behind and below. The report then compares the results of quantitative kinetic perimetry obtained by one investigator using the same Goldmann perimeter with findings obtained at the same time at the 2nd Clinic for Obstetrics and Gynecology of Vienna University.

RESUMO

According to traditional opinion the basophile adenoma, in contrast to the eosinophile or chromophobe tumours of the hypophyse, practically never causes lesion of the chiasm. However, the case of a 16-year-old girl suffering from adiposis, hypertonia, striae distensae and a "full moon face" is used to demonstrate that through the precis examination of the visual field on the Goldmann perimeter even with normal disque of the optic nerve, normal sella turcica established by X-ray, and uncharacteristic hormon condition, a slight chiasmal lesion caused by pressure from below can be established.

RESUMO

In a short survey the technical difficulties occurring at the intubation of stenotic tear passages are presented. In conclusion, a simple 'blood free' method is advised as promising; consisting of the putting through of a 'Silastic tube' through both lacrymal points with the help of a curved canula probe (Fulmek).

RESUMO

After a discussion of the pathomechanism of skull trauma by a blow leading to chiasmal lesion, frontobasal injuries of the skull are considered; it is shown that typical defects of the visual field resulting from chiasmal damage can be revealed not only in cases of serious industrial or traffic accidents with brain concussion and skull base fracture, but also following minor injuries without unconsciousness and radiologically visible bone fractures.

RESUMO

With reference to a chiasma contusion, proved at autopsy, sustained by a football player after a temporal impression fracture due to contact of a knee with his skull, the pathological mechanism causing chiasma injuries in blunt head injuries is explained. Finally, from our own experience we report on 2 water-sports accidents sustained by young men (a jump from the trampoline, and a fall during water-skiing) where chiasma-lesions were diagnosed from typical field defects.

RESUMO

After a discussion of the pathomechanism of frontobasal cranial injury with a consecutive chiasmal lesion and diabetes insipidus due to a fracture of the basis of the skull, a personal observation of bitemporal hemianopia with a transient diabetes insipidus after a frontal motorcycle crash is recorded. A short survey of the literature shows the controversial opinions on traumatogenic lesions of the chiasma, which may be injured directly or the bitemporal hemianopia is due to a posttraumatic vascular disturbance. For the case described by the author primary tearing of the chiasma due to sudden applanation of the skull in the frontal region with burstfractures in the anterior cranial fossa is assumed.

A survey of the present knowledge of the pathology, clinical corues and prognosis of arachnoidosis optochiasmatica is given. Furthermore a case is described where the imminent loss of sight could be prevented by close cooperation with the neurosurgeon and well-timed operation.

RESUMO

Report on a case of advanced siderosis bulbi. Besides the typical alterations in the anterior segment of the globe, siderotic changes of the retina could be revealed. The ERG showed a reduction of the b-wave. After removal of the foreign body, a partial regression of the siderotic changes took place. In addition, the ERG became normal within 8 weeks postoperatively. The indication for removal of metallic intraocular foreign bodies despite specific alterations of the ERG is emphasized.
