Topical steroids are among the most frequently prescribed medications in the treatment of skin disease. Hundreds of preparations are currently available in a variety of vehicles and potencies. This article details the mechanism of action, determinants of clinical effectiveness, vehicle and potency selection, special considerations, and side effects of topical steroids.

Granuloma gluteale infantum, a benign granulomatous eruption involving the gluteal region, histologically shows a nonspecific dermal inflammatory infiltrate composed of neutrophils, lymphocytes, histiocytes, plasma cells, and eosinophils. It is important to recognize this condition, as it may clinically simulate a neoplastic process. It arises as a complication of primary irritant diaper dermatitis, however, and typically resolves without treatment.

Cutaneous leishmaniasis is endemic in south central Texas. There have been at least eight cases, four of which have been reported since 1980. An additional case of cutaneous leishmaniasis is reported in a 56-year-old woman from Sandia, Texas. The diagnosis was confirmed by culture of Leishmania mexicana var. mexicana.

Exogenous ochronosis resulting from the topical application of hydroquinone-containing bleaching creams has been reported to occur almost exclusively in black subjects, and only after use of high concentrations of hydroquinone (greater than 3 percent) for many years. A Mexican-American patient is described who experienced exogenous ochronosis after using 2 percent hydroquinone cream for less than six months.
