[Sjögren syndrome associated with renal tubular acidosis type I]. / Un caso de Síndrome de Sjögren primario complicado con acidosis tubular renal tipo 1.

Primary Sjögren's Syndrome complicated with a renal tubular acidosis type 1 and hypocalcemic paralysis, as the principal clinical manifestation, is uncommon. Although the initial manifestations of the nephropathy are not well understood, it is believed that the invasion of mononuclear cells and the high level of circulating antibodies, play an important role in the pathogenesis of the disease. We present a patient with hypocalcemic paralysis as an initial manifestation of a latent Sjögren's disease. The glandular biopsy was normal, suggesting a mayor participation of an immunological humoral factor in the renal lesion.

[Arteriovenous fistula of the superficial temporal artery]. / Fístula arteriovenosa de la arteria temporal superficial.

A case of an post-traumatic arteriovenous fistula of the right temporal superficial artery in a diabetic child is reported. In absent of a cranial fracture, the lesion was notorious for the first time, ten months after a simple head injury. The progressive increase in size and the beginning of a frequent parietal right headache were the initial symptoms and important signs. The malformation was clipped on both sides and removed completely. The diagnosis and treatment are discussed as well as the theory on the pathogenesis of the lesion.

Fiebre reumatica en los adultos: caracteristicas observadas en los pacientes hospitalizados en el Hospital Santo Tomas, de 1973 a 1979. / Rheumatic fever in adults: characteristics observed in hospitalized patients at the Hospital Santo Tomas, from 1973 to 1979

Se estudiaron los expedientes clinicos de todos los pacientes adultos que habian sido admitidos en el Hospital Santo Tomas (HST) con el diagnostico de fiebre reumatica, en el periodo comprendido entre 1973 y 1979, y se seleccionaron los de trece pacientes adultos que fueron atendidos en su primer episodio y que se ajustaban estrictamente a los criterios revisados de Jones. La mayoria (85%) de los pacientes seleccionados eran adultos jovenes, entre los 8 y los 28 anos de edad; siete (54%) eran del sexo femenino y seis (46%) eran del sexo masculino. Nueve pacientes (69%) procedian del area metropolitana y cuatro (31%) procedian del interior de la Republica. La artritis fue la manifestacion clinica mas frecuente y se presento en el 100% de los casos. Se caracterizo por afectar las grandes articulaciones, principalmente de las extremidades inferiores, y por se poliarticular, simetrica y aditiva, en el 85% de los casos. La clasica migratoriedad solo se presento en el 15% de los pacientes. La carditis fue leve y se observo en el 38% de los casos. En ninguno de los pacientes se presento eritema marginado, nodulos subcutaneos o corea
