RESUMO

BACKGROUND: Full thickness skin grafting is a common technique for reconstructing defects in the head and neck area. We propose the use of an addition-cured silicone as an individually moulded silicone dressing to keep the vulnerable skin graft in place, prevent shearing forces and create a moist environment. METHOD: The silicone dressing is applied directly on the graft, using a double cartridge system. The silicone is moulded to the skin graft and hardens quickly, integrating thread knots into its material and creating good adherence to the graft. Charts of 24 patients who had undergone reconstruction with full thickness skin graft from the neck after surgery for skin tumors in the head from November 2017 to October 2020, were reviewed retrospectively to quantify the degree of post-operative graft loss and durability of the dressing. CONCLUSION: Medical silicone based on venylpolysiloxane is a safe and durable dressing which makes postoperative dressing changes expendable. Trial registration The study was approved by the institutional review board of the Brandenburg state medical association (S 4(bB)/2021).

RESUMO

PURPOSE: The right choice of dressing after skin grafting, especially in the anatomically complex and aesthetically important head and neck area, is difficult. It is important to have a dressing which is durable and doesn't need a lot of dressing changes. This study introduces a novel, individually moulded silicon dressing; and investigates the number of dressing changes, durability of the dressing and the aesthetic outcome of patients receiving this new type of dressing, in comparison to a control group. MATERIALS AND METHODS: The present retrospective single center controlled case series study reports our experiences using two different types of dressing on patients undergoing full thickness skin grafts in the head during the period 01 May 2016 to 01 May 2020. Data were analysed according to the type of dressing with either a transparent moulded silicone dressing or an established silicone sheet dressing. RESULTS: 52 patients were included in the study. We found no difference in the aesthetic outcome and complication rate (p > 0.05) between the two groups. The number of days until first dressing change after surgery, the number of dressing changes or follow up visits in the outpatient care until complete wound healing, and the overall number of dressing changes were all statistically significantly reduced (p < 0.05). CONCLUSION: Addition-cured silicone used as a moulded dressing in full thickness skin grafts has longer durability and leads to a significant reduction of dressing changes with equal aesthetic outcome compared to an establishes silicone sheet dressing.

RESUMO

Pulmonary hypertension (PH) is characterized by the increase of the mean pulmonary arterial pressure in the lung circulation. Despite the large number of experimental and clinical studies conducted on pulmonary hypertension, there is no comprehensive work that analyzed the global research activity on PH so far. We retrieved the bibliometric data of the publications on pulmonary hypertension for two periods from the Web of science database. Here, we set the first investigation period from 1900 to 2007 (t1) due to the cited half life of articles and the relating difficulties to interpret the citation parameters. The second evaluation period (t2) covers the time interval from 2008 onwards including the year 2015. The data were analyzed and processed to density-equalizing maps using the NewQIS platform. A total number of 18,986 publications were identified in t1 that come from 85 countries. The US published the highest number of publications (n = 7,290), followed by the UK, Germany, Japan and France. In t2 19,676 items could be found worked out by 130 countries. The raking started just the same with the USA as most publishing nation with 7,127 publications on PH, followed by the UK and Germany. Japan fell back on 6th place, whereas China came into view on the 5th position. Analyzing the average citation rate as a parameter for research quality, Mexico reached the highest value in t1 and Ireland in t2. While, the country specific h-index underlined the leading position of the US research in both evaluation periods again. The average number of international collaboration items was expanding from none in 1978 to 530 items in 2015 with the USA as the country with the highest number of collaboration articles. The present study is the first large scale density-equalizing mapping and scientometric analysis of global PH research activity. Our data draw a sketch of the global research architecture in this field, indicating a need for specific research programs in countries with a lower human development index.

RESUMO

Due to an increasing awareness of the potential hazardousness of air pollutants, new laws, rules and guidelines have recently been implemented globally. In this respect, numerous studies have addressed traffic-related exposure to particulate matter using stationary technology so far. By contrast, only few studies used the advanced technology of mobile exposure analysis. The Mobile Air Quality Study (MAQS) addresses the issue of air pollutant exposure by combining advanced high-granularity spatial-temporal analysis with vehicle-mounted, person-mounted and roadside sensors. The MAQS-platform will be used by international collaborators in order 1) to assess air pollutant exposure in relation to road structure, 2) to assess air pollutant exposure in relation to traffic density, 3) to assess air pollutant exposure in relation to weather conditions, 4) to compare exposure within vehicles between front and back seat (children) positions, and 5) to evaluate "traffic zone"-exposure in relation to non-"traffic zone"-exposure.Primarily, the MAQS-platform will focus on particulate matter. With the establishment of advanced mobile analysis tools, it is planed to extend the analysis to other pollutants including NO2, SO2, nanoparticles and ozone.

RESUMO

We present a biologically motivated architecture for object recognition that is capable of online learning of several objects based on interaction with a human teacher. The system combines biological principles such as appearance-based representation in topographical feature detection hierarchies and context-driven transfer between different levels of object memory. Training can be performed in an unconstrained environment by presenting objects in front of a stereo camera system and labeling them by speech input. The learning is fully online and thus avoids an artificial separation of the interaction into training and test phases. We demonstrate the performance on a challenging ensemble of 50 objects.
