RESUMO

In 49 of 365 clinical and patho-anatomical intracranial tumour cases the tumours were indicated in the anamnesis only by mental disturbances before acute signs of brain pressure appeared. With reference to the relations between the psychopathological syndromes and the types of locations of tumours attention is drawn to factors that may complicate the early detection of tumours.

RESUMO

The paper describes the neuropathological findings in the seven-year case history of a male patient who died at the age of 29 of Behçet's disease with initial iridocyclitis and neurological complications. In the author's opinion, the presence of any one of the three cardinal symptoms of the muco-cutaneo-ocular syndrome and its manifestation on the CNS is sufficient to suppose Behçet's disease. Affectation of the eye is a significant and usually the first symptom. The case history and the typical morphological findings give rise to the assumption of a special ophthalmomeningoencephalic variety of Behçet's disease.

During the past ten years, treatment of delirium alcoholicum was almost exclusively by means of chlormethiazol (distraneurin), an agent which has sedative, hypnotic, and antiepileptic effects. A total of 111 deliriants with single and multiple deliria were subjected to 151 treatments. In the majority of cases the state of delirium was overcome after three days of specific treatment. The case histories of three patients, where the course of disease was extremely severe and additionally complicated by secondary diseases and who died during treatment (the percentage being 0.7% relative to 151 cases of treatment), are discussed in detail. In the case of severe states of delirium accompanied by repsiratory diseases it is necessary that the patient receiving specific treatment be supplied with additional amounts of oxygen because distraneurin, like other sedatives, trends to reduce the utilization of oxygen under certain conditions. The majority of deliriants were in their third to fourth decades of life. Accordingly, our patients were younger than those included in the reports presented by other authors. Also discussed by the present authors are attacks accompanying the state of delirium as well as concomitant diseases complicating the clinical picture and adverse effects of alcohol on the nervous system.

A comparison is made of the pathological changes of the median line structures such as corpus deficiency, enlarged cavum septi pellucidi (Csp.) and septal defects in 10,457 pneumo-encephalography (PEG) examinations of neurological and psychiatric patients, and in 11,016 general autopsy cases. Clinical peculiarities in corpus deficiency and other development anomalies of the median line structures are discussed along with case reports. A predominance of the male sex in cases of enlarged Csp. together with frequent combinations with neurological and psychiatric illness were noticeable, particularly oligophrenia, behavioral disorders, epilepsy and cerebral tumours. Thus, certain inferiority of the dysrhaphic mutation of the central organ can be assumed.
