INTRODUCTION: Guatemalan lay midwives are well-respected community leaders in a country that lacks the institutional capacity to meet healthcare needs related to pregnancy, newborns, and COVID-19. Thus, Guatemalan lay midwives, who attend the majority of births in their country and who attend most births at home, are in an optimal position to offer frontline support to pregnant women and newborns regarding the global pandemic. The primary objective of this program of study was to dispel myths about COVID-19 and to provide culturally relevant educational activities to low-literacy Guatemalan lay midwives about issues related to the virus, such as signs and symptoms, risks to the mother and fetus, which patients are most vulnerable, appropriate responses, benefits and side-effects of the vaccine, timing of the vaccine, how the virus interacts with breastfeeding, and breastfeeding recommendations. METHODS: In a partnership among the Guatemalan Ministry of Health, expert faculty at the University of Utah College of Nursing, and Madre y Niño, a non-profit organization from the US, evidence-based educational activities about COVID-19 were offered orally in the native language of participants. Two hundred and ten lay midwives attended educational sessions at 11 locations throughout the remote Peten department. Educational activities included repetition, storytelling, and role plays. A pretest-post-test evaluation of 10 questions with 24 correct answers was used to determine if the educational activities changed lay midwife knowledge about COVID-19. Participants were given essential birth supplies and laminated COVID Reminder Cards, which were designed to increase visual literacy, to encourage knowledge retention after the educational sessions. RESULTS: Participants showed a significant increase in knowledge scores (possible 0-24) from prescores 7.09 (standard deviation (SD)=3.06) to 15.20 (SD=4.61), Student's t-test p<0.001. In addition, a significantly higher proportion of participants mistakenly thought COVID-19 passed through breast milk on the pretest (70.1%) compared to post-test (8.4%) (McNemar test, p<0.001). Regarding breastfeeding, 12.6% of participants knew on the pretest that women with COVID-19 who breastfeed should wear a mask and wash their hands compared to 74.3% of participants who knew these recommendations on the post-test (Wilcoxon signed-rank test, p<0.001). Finally, 2% of participants knew on the pretest that pregnant women with COVID-19 should take a low-dose aspirin compared to 67% of participants on the post-test (χ² (1)=194.7, p<0.001). CONCLUSION: These essential primary care providers misunderstood many critical issues related to COVID-19, pregnancy, and newborns. Culturally relevant educational activities provided orally in the native language of participants dispelled myths about the virus and significantly improved lay midwife knowledge. Providing evidence-based educational activities in a culturally relevant format is critical to protecting remote, vulnerable populations, such as pregnant Guatemalan women and newborns, during a global pandemic.

Se describen las características esenciales de Colombia, Chile, México, Venezuela y se particulariza en el modelo cubano. Se realiza una reflexión final respecto a la contradictoria situación prevaleciente y se analiza su contexto político (AU)


The characteristics of Public Health in Latin America and the Caribbean through the present health state of the region is considered. The evolution of these characteristics and the facts that justify the processes of reformation of the sector, as well as their theoretical sources ; the form in which they are developed, and the problems they face are also regarded. The essential characteristics of Colombia, Chile, Mexico, Venezuela and particularly those of the Cuban model are described. A final reflection regarding the contradictory prevalent situation and its political context is made (AU)

Un aspecto importante del progreso científico es el experimento. Por lo tanto, las ciencias médicas tienen que apoyarse en este para incorporar nuevas tecnologías, diagnósticas y terapéuticas a la práctica clínica; validando su seguridad y eficacia en animales y seres humanos. La ética de la experimentación con seres humanos no es sólo una de las áreas fundamentales de la bioética, sino también una de las problemáticas que le dieron origen. Se describen en este trabajo algunas implicaciones éticas y aspectos sociales de la experimentación en seres humanos, particularmente en el marco de la incorporación de una tecnología de punta como es la distracción osteogénica en la cirugía maxilofacial, para la reconstrucción de defectos combinados en tejidos duros y blandos; se analizan el origen y evolución de la bioética, los aspectos regulatorios para la realización de ensayos clínicos y su valoración moral, los tipos de experimentación; la realidad mundial y cubana en cuanto a investigación clínica y tecnología y el consentimiento informado. Se concluye que el criterio ético fundamental de las investigaciones en seres humanos es su disposición al servicio del hombre como fin y nunca como medio (AU)


An important aspect of the scientific progress is the experiment. Therefore, the medical sciences have to lean on this to incorporate new technologies, diagnostic and therapeutic to the clinical practice; validating their security and effectiveness in animals and human beings. The ethics of the experimentation with human being is not only one of the fundamental areas of bioethics, but also one of the problems that brought about its origin. Some ethical implications and social aspects of the experimentation in human beings are described in this work, particularly while talking about the incorporation of a high technology in the ostegenic distraction in maxilofacial surgery for the reconstruction of defects combined in hard and soft fabrics; the origin and evolution of bioethics are analysed, the regulatory aspects for the realization of clinical rehearsals and their moral valuation, the experimentation types; the world and Cuban reality as for clinical investigation and technology and the informed consent. As a conclusion it is said that the fundamental ethical approach of the investigations in human beings is its disposition to the man's service as a goal and never as a way (AU)

Antecedentes: el virus de la hepatitis E es el agente causal de la hepatitis E. Las propiedades biológicas y moleculares de las cepas asiáticas del VHE ya han sido exploradas en cultivos celulares. Objetivos: aislar y propagar una cepa cubana del virus de la hepatitis E en diferentes líneas celulares. Métodos: la monocapa de las células A549 fue inoculada con una suspensión de heces obtenida de un paciente con diagnóstico serológico y molecular de hepatitis E. Estas células fueron observadas hasta el décimo pase. Mientras que, las líneas celulares MRC5, LLCMK2, HEP-2, FRhK4 y HeLa fueron utilizadas para propagar el virus de la hepatitis E, a partir del sobrenadante obtenido del tercer pase en A549. Estas células fueron seguidas hasta el tercer pase. El ARN y los antígenos de la cepa ECV/2349-03 fueron identificados por TR-RCP e inmunofluorescencia indirecta. Resultados: en las células A549 el ECP apareció desde el primer pase, a los 3 d de posinoculación. El genoma y los antígenos del virus fueron identificados en todos los pases seriados. En el resto de las líneas celulares estudiadas no se observó el ECP. En estas células el material genético del virus de la hepatitis E se detectó desde el primer pase y los antígenos a partir del segundo pase, excepto en las HeLa. Conclusiones: estos resultados confirman que las células A549 pueden ser utilizadas para aislar y propagar el virus de la hepatitis E, mientras que las células MRC5, LLCMK2, HEP-2 y FRhK4 son capaces de mantener el crecimiento viral(AU)


Background: hepatitis E virus (HEV) is the causative agent of hepatitis E. Biological and molecular properties of Asian HEV strains have been explored in cells cultures. Objetives: the aim of this investigation was to isolate and propagate a Cuban HEV isolate in different cell lines. Methods: A549 cells monolayer was infected with faeces suspension from patient with sporadic hepatitis E and followed up until tenth passage. Lately, the supernatant harvested from third passage in A549 cells was inoculated in MRC5, HEP-2, LLCMK2, HeLa y FRhK4 for propagation study. These cells were observed up to third passage. RNA and viral antigen of ECV/2349-03 HEV strain were identified by RT-PCR and indirect immunofluorescence. Results: CPE appeared since first passage, at third day of post-inoculation in A549 cells. HEV antigens and genome were detected in all serial passages. CPE was not observed in the rest of cellular cultures. In the cells used for propagation the viral genome was observed from first passage, while the antigens were detected since second passage, except HeLa. Conclusions: these results confirm that A549 can be used to isolate and propagate HEV. Meanwhile, the MRC5, HEP-2, LLCMK2 and FRhK4 were able to support viral growing(AU)

The main objective of this study is to evaluate the prevalence of resistance-associated mutations to reverse transcriptase (RT) and protease (PR) inhibitors (I) 2 years after the introduction of antiretroviral treatment in Cuba, analyzing the mutations corresponding to different HIV-1 genetic forms circulating in Cuba. A total of 425 plasma samples were collected in 2003, corresponding to 175 (41.2%) subtype B and 250 (58.8%) non-B genetic forms, including 56 (22.4 %) non-B subtypes, 112 (44.8%) circulating recombinant forms (CRFs), and 82 (32.8%) unique RFs (URFs). Of these, 175 (41.2%) patients were under highly active antiretroviral therapy (HAART) and 250 (58.8%) were treatment-naive. The presence of RT and PR resistance-associated mutations was established by sequencing. Levels of resistance were evaluated according to the Stanford Database program (http://hivdb.stanford.edu). The prevalence of resistance to RTI was 52.2% among RTI-treated patients, 51.5% for subtype B, and 53.2% for non-B genetic forms, including CRF18_cpx, CRF19_cpx, subtype C, and BG URF. In treatment-naive patients it was 6.4% in subtype B and 4.2% in non-B subtypes and RFs. The prevalence of resistance to PRI was 30% among PRI-treated patients, 28% in subtype B and 31% in non-B genetic forms, and 3.2% among treatment-naive subjects, mostly BG recombinants. In conclusion, significant differences in the prevalence of resistance to RTI and PRI were not detected among the most frequent genetic forms from treated patients, suggesting that the genetic diversity of HIV-1 in Cuba does not play a main role in the development of resistance to antiretroviral drugs. The presence of transmitted resistance mutations supports the study of resistance at baseline of treatment.

BG intersubtype recombinants represented 11.6% of HIV-1 isolates in a recent survey in Cuba based on pol sequences, most of them forming a single clade further subdivided into 3 subclades. Here, we analyze 8 near full-length genomes and 1 gag-pol sequence from epidemiologically unlinked Cuban BG recombinants from these 3 subclades (3 from each). Near full-length sequences were also obtained from 3 subtype G and 2 subtype B Cuban viruses. Phylogenetic relationships were estimated via maximum likelihood, and mosaic structures of the recombinants were inferred with the bootscanning, MaxChi, Genconv, and GARD methods. For the near full-length genomes, all recombinants formed a strongly supported clade further subdivided into the same subclades previously defined in pol. Mosaic structures were identical within each subclade and different among subclades, although 5 breakpoints were coincident among all recombinants. Individual phylogenetic trees for nonrecombinant fragments (concatenated B and G subtype segments) indicated a common ancestry for the parental viruses and their relationships to local subtype B and G strains. These results allow us to identify 3 new BG intersubtype circulating recombinant forms in Cuba derived from a common recombinant ancestor, which originated from B and G subtype parental strains circulating in Cuba.

Highly diverse HIV-1 genetic forms are circulating in Cuba, including subtypes B and G and two recombinant forms of African origin (CRF18_cpx and CRF19_cpx). Here we phylogenetically analyze pol sequences from a large collection of recent samples from Cuba, corresponding to 425 individuals from all Cuban provinces, which represents approximately 12% of prevalent infections in the country. RNA from plasma was used to amplify a pol segment by reverse transcription-polymerase chain reaction; phylogenetic analyses were performed with neighbour-joining trees and bootscanning. The distribution of genetic forms was subtype B, 41.2%; CRF19_cpx, 18.4%; BG recombinants, 11.6%; CRF18_cpx, 7.1%; subtype C, 6.1%; subtype G, 3.8%; B/CRF18 recombinants, 2.6%; subtype H, 2.1%; B/CRF19 recombinants, 1.7%; and others, 5.4%. Seventy-five (17.6%) viruses were recombinant between genetic forms circulating in Cuba. In logistic regression analyses, adjusting by gender and region, subtype B was more prevalent (OR 5.0, 95% CI 2.0-12.3) and subtype G less prevalent (OR 0.1, 95% CI 0.0-0.5) among men who have sex with men (MSM) than among heterosexuals. Within the main genetic forms of Cuba there were phylogenetic subclusters, several of which correlated with risk exposure or region. BG recombinants formed three phylogenetically related subclusters, corresponding to three different mosaic structures; most of these recombinants were from MSM from Havana City, among whom they have expanded recently, reaching 31% HIV-1 infections diagnosed in 2003. This study confirms the high HIV-1 diversity and frequent recombination in Cuba and reveals the recent expansion of diverse related BG recombinant forms in this country.

Nine Adenovirus (Ad) strains isolated in Cuba, from 128 nasopharingeal swab specimens of children below five years old, with acute respiratory diseases, during 1996 and 1997, were studied by restriction enzyme analysis of genomic DNA with two endonucleases BamH I and Sma I. All different fragment patterns were compared with the respective prototypes. The identified adenoviruses were Ad 1 (n=4), Ad 2 (n=1) and Ad 6 (n=4). Males were more frequently infected than females. The analysis of the occurrence of these Adenovirus strains of subgenus C revealed that Ad 1 and Ad 6 were the predominant serotypes in 1996 and in 1997, respectively

RESUMO

Se presentó el caso de una paciente que ingresó en el Hospital General Calixto García por tener síntomas respiratorios y estado de choque hipovolémico o séptico. Se diagnosticó neumonía hemorrágica viral. A partir de la secreción endotraqueal tomada como muestra se aisló el virus de influenza como agente etiológico, y se caracterizó como una cepa perteneciente al subtipo H3N2, parecida antigénicamente a la cepa A/Johannesburg/33/94, mediante la técnica de inhibición de la hemaglutinación. La paciente evolucionó hacia la curación luego de recibir tratamiento con rivabirín(AU)

RESUMO

The paper presents the case of a female patient who was admitted to "Calixto GarcÝa" General Hospital with respiratory distress and hypovolemic or septic shock. She was diagnosed with viral hemorrhagic pneumonia. From the endotracheal secretion taken as a sample, the influenza virus was isolated as etiological agent, which, through the hemaglutination inhibition technique, was characterized as a strain belonging to H3N2 subtype, very similar to strain A/Johannesburg/33/94 from the antigenic viewpoint. The patient recovered satisfactorily after being treated with rivabirin.

RESUMO

Se detectó un incremento en los casos reportados de meningoencefalitis viral (MEV) en los meses de octubre y noviembre de 1995 con respecto a igual período de 1994. Se recibieron en el Laboratorio de Enterovirus del Instituto de Medicina Tropical "Pedro Kouri" 43 muestras de heces fecales procedentes de niños con este diagnóstico, y se obtuvieron 23 aislamientos (53,4 por ciento) identificados como Coxsackievirus B5. Además, en 43 sueros pareados investigados por la prueba de neutralización frente a varios Enterovirus, se obtuvieron 21 positivos (48,8 por ciento) al agente aislado. Esto nos permitió afirmar, apoyados en el cuadro clínico y en la epidemiología, que estábamos en presencia de un brote de MEV producido por coxsackievirus B5(AU)

La línea continua NCI-H292 de células mucoepidermoides de pulmón humano ha sido reportada ser de utilidad para la propagación de muchos virus, principalmente Adenovirus y Paramyxovirus. Se plantea la posible sustitución de cultivos primarios de riñón de mono por NCI-H292 para el aislamiento de dichos agentes. En el presente trabajo se evalúa la utilidad de esta nueva línea para la multiplicación de los virus sincitial respiratorio. Adenovirus 3 y 7 y los virus parainfluenza 1,2 y 3 en comparación con las líneas celulares continuas utilizadas tradicionalmente para la propagación de éstos: para lo cual se inocularon cepas de los virus en cuestión en las líneas Vero, HEp-2 y HeLa, según sus sensibilidades conocidas, y en NCI-H292 paralelamente. La multiplicación viral se detectó por aparición de efecto citopático o por hemadsorción. Como resultado se corroboró la capacidad de multiplicación de la línea NCI-H292 para los Adenovirus 3 y 7 y el parainfluenza 3, siendo más útil para la multiplicación de éstos que las líneas tradicionalmente usadas(AU)

The NCI-H292 continual line of mucoepidermoid cells of the human lungs has been reported to be useful for the propagation of many viruses, mainly Adenovirus and Paraxymovirus. It is stated the possible substitution of primary cultures of monkey kidney for NCI-H292 in order to isolate such agents. In the present paper it is evaluated the utility of this line for multiplying the respiratory syncytial viruses Adenovirus 3 and 7, and the parainfluenza viruses 1, 2, and 3, in comparison with the continual cellular lines traditionally used for the propagation of these viruses, whose strains were inoculated this time in the Vero, HEp-2, and HeLa lines, according to their know sensitivities as well as in NCI-H292 simultaneously. The viral multiplication was detected by the appareance of the cytopathic effect or by hemaadsorption. As a result, it was demostrated the multiplication capacity of the NCI-H292 line for Adenoviruses 3 and 7 and parainfluenza 3, being more useful for their multiplication than the tradicionally used lines.
