[History of plastic surgery and its effects on our specialty]. / Geschichte der plastischen Chirurgie und ihre Auswirkungen auf unser Fach.

The life and work of the first three chief surgeons at the University of Berlin are portrayed: Carl Ferdinand von Graefe (1787-1840), Johann Friedrich Dieffenbach (1792-1847) and Bernard von Langenbeck (1810-1887). Especially that of Dieffenbach and his time is pointed out. Similarly the founder of cosmetic surgery, Jaques Joseph (1865-1934), is also mentioned.

[Risks of serial intramuscular injections]. / Die Risiken intramuskulärer Serieninjektionen.

A recently reviewed case underlined the danger of serial intramuscular injections. More recent results about muscular response to injection treatment and its morphological, anatomical, and pathophysiological effects on muscular tissue are described. Our studies about the optimum length of the needle for intramuscular injections and the thickness measurements of tissue and fat layers in the area of the most important injection points (upper outer quadrant of Hochstetter point) were supplement by animal experiments to prove that the use of short needles might lead to subcutaneous misinjections causing acute drug-induced dermal embolism. On the other hand these animal experiments demonstrated that even correctly placed serial injections might cause severe necrosis of the deep muscular structures accompanied by a major increase in serum creatinin phosphokinase activity. With intragluteal serial injections one must think of repair-induced muscular fibrosis which led to an intragluteal implantation cyst in the case reviewed by us.

[Allogeneic lymph vessel transplantations in the early phase]. / Allogene Lymphgefässtransplantate in der Frühphase.

Syngeneic and allogeneic grafts of segments of the thoracic duct from rats were investigated in the first fifteen days after transplantation. The patency rate--evaluated by the passage of blue dye--of the allogeneic transplants was almost equal to that seen in the syngeneic grafting. By direct exploration there were no changes to be seen in the syngeneic and allogeneic transplants. Investigations by electron microscopy showed no major reactions to the syngeneic grafts during the observation time: The endothelium always was completely detectable and the subendothelial layer of collagen fibrils was not dislocated. By contrast, the allogeneic transplants showed considerable alterations as disintegration of the collagen fibrils and a partial loss of endothelium occurred after eight to ten days. After fifteen days, however, the formation of a neointima could be demonstrated. Despite the observed alterations there was no loss of function in the allogeneic transplants.

Tooth pulp stimulation: a method of determining the analgesic efficacy of meptazinol in man.

A dose-ranging study of meptazinol was carried out using the pain threshold to electrical tooth pulp stimulation in healthy volunteers as the pain model. A well defined dose-response curve was found for oral meptazinol (50 mg, 100 mg, 150 mg and 200 mg) and placebo. The methodology and results are discussed in terms of subsequent clinical experience with meptazinol.

Symptomatic prophylaxis for pregnant women in whose relatives facial clefts have appeared has been carried out since 1958. This report of over 20 years' experience shows the difference between data from clinical histories which give a heredity ratio of about 17% and the careful family investigation in families with cheilognathopalatoschisis. These family investigations show (and this is understandable) a considerably higher heredity ratio, because in certain respects they represent a negative selection. Thus we find in the F1-F4 generation of families subject to clefts, values of about 40 to 50%. In contrast there is a treatment group which has undergone symptomatic prophylaxis by administration of vitamins and an oxygen-activating medicament in which a significant lowering of the deformity rate is shown in "cleft-positive" mothers treated in early pregnancy.

Rhinoplasty for preventing nasal cartilage growth.

The physiologic growth of the nose, which continues, particularly in females, throughout life, prompted us to develop a technique for preventive surgery. The method consists in producing a scar in the cartilaginous portion of the nose to prevent continued growth of that tissue and to preserve the nose without deformities.

Clinical evaluation of Warthin's tumor. An analysis of 43 cases.

Forty-three patients with Warthin's tumor (26 males and 17 females) were analyzed and the following results were obtained: All tumors were related to the parotid gland with no bilateral involvements. The ages of the patients were 20-84 years old and the average age was about 62. The interval between tumor recognition and first examination was 1 week to 16 years with the average interval of 20-22 months. The method of treatment was complete enucleation. However, radiation therapy was also done in three cases, two of which were irradiated after operation. The other received palliative radiation therapy because of old age and poor general condition. Recurrence was observed in two cases in the course of the follow up, but the examination at the end of June 1977 revealed neither recurrence nor metastasis in the cases followed. A rare case of synchronous occurrence of warthin's tumor and another histological tumor was included in this series and similar cases are reviewed briefly in the literature.

[Analgesic and anti-inflammatory efficacy of biarison. Clinical experimental studies (author's transl)]. / Analgetische und antiphlogistische Wirksamkeit von Biarison. Klinisch-experimentelle Untersuchungen.

The analgesic time-effect profile of 600 mg Biarison (clinical experimental number: RU 43-715) compared with 100 mg indometacin, placebo and a blank test was recorded for 12 healthy subjects as an intra-individual single-blind trial by means of electrical measurement of thresholds in the teeth. Biarison shows a powerful analgesic effect with a maximum increase of the threshold by 22.3 mA after 75 minutes. The maximum increase in mA with indometacin and placebo was 18 and 9 mA respectively. The differences are confirmed statistically. Under double blind conditions, no significant differences in antiphlogistic activity could be established between Biarison (900 mg/day) and indometacin (150 mg/day) with reference to the edema appearing after maxillary sinus operations.
