RESUMO
Interstitial brachytherapy of intracerebral tumours was stimulated through the introduction of computed tomography and afterloading technique. Biopsies for their histologic verification as well as the stereotactic implantation of plastic tubes and adjusted to the extent of the tumour can be carried out with a high degree of exactitude by means of CT-supported localization and puncture. The small-sized 198-gold seeds are then introduced according to the manual afterloading technique and remain in situ until the planned dose in the target volume of 50 Gy is achieved. Report on our manual afterloading brachytherapy in high-grade astrocytomas III and IV and in adenomas of the hypophysis. Indications of the diagnostic and therapeutic procedure, irradiation planning, calculation of the dose and discussion of our tentative clinical experience are explained.

The piriformis syndrome is characterized by pain in the buttock radiating to the leg and to the coccydeal region. It is an isthmus syndrome of the N. ischiadicus and/or N. cutaneous femoris posterior. The most frequent causes are blunt traumas of the buttocks and spasms of the N. piriformis. More often than not the piriformis syndrome is the cause of a "postlaminectomy syndrome" or of a coccygodynia. The diagnosis can be established by a clinical examination with sufficient reliability.

In 1981, percutaneous transluminal angioplasty (PTA) of the subclavian artery was performed in 10 patients. 8 stenotic lesions were localized in the first segment and two in the third segment respectively. Dilatation was performed with the Grüntzig balloon-catheter ranging from 4 to 7 French. PTA was successful in 9 of the 10 cases. Success of dilatation was documented by recording blood pressure, normalization in the pre- and poststenotic segment and by clinical results. Success of PTA depends on exact angiographic studies, adequate indication and subsequent anticoagulation treatment. Early results after PTA are excellent. PTA of stenotic subclavian arteries has proved an alternative to surgical management.

By means of the division of the conserved blood into cellular and plasmatic constituents it became possible to produce also plasma fractions with enriched coagulation factors. The factor-VIII-preparations got a particular importance for the substitution treatment of patients with haemophilia A, also under the viewpoint of the prophylaxis for the decrease of the frequency and the intensity of haemorrhages. Apart from the plasma fraction cryoprecipitate a preparation with a medium degree of enrichment on factor VIII could still be developed. However, with increasing enrichment the losses on factor VIII increase progressively. Despite the search for substances which substitute the factor VIII in the course of coagulation, release from the vascular wall and increase its activity, the substitution therapy with factor-VIII-preparations cannot be abandoned for the time being.
